  • Postural drainage is an airway clearance technique in chest physiotherapy that uses the effect of gravity to clear secretions from different segments of the lungs. This video demonstrates the different positions used to remove secretions at different parts of the lungs.

    you should never make a patient cough in supine lying ever. either turn them sideways or bring the head to upright position!

    What about putting people face down on an inversion table and then using a vibrating massager to vibrate the phlegm out of the lungs?

    nice video for the postural drainage guys , but it hurts my eyes to see the demonstrations of huff and couphing while completely supine.

    Why is wheeze contraindicated?

      If someone is having an asthma exacerbation, they may not be able to tolerate postural drainage positioning. Wheezing, in this case is due to bronchoconstriction.
      However, the presence of mucous in the lungs can cause wheezing as well, and postural drainage can help facilitate removal of the phlegm.
      So, if the patient can't tolerate positioning, sit them up and modify as needed, or, discontinue the therapy altogether.
      If they're really struggling, then you wouldn't even consider postural drainage. Your focus would be on bronchodilators, and possibly oxygen therapy if necessary.

      Chest examination signs may show but a Chest CT would....i had a patient who came with a chest CT

      Try putting books under the bed posts to elevate the top part or bottom part. Also, try using several pillows to position your body.

    How does one know from which area of lung secretions are coming from? Should a patient do each position for 15 minutes each?

      Tools like a Stethoscope (Listening in segments of the lungs for secretion) and/or CRX (Chest X-Ray) are best.
