Pretty much every member of the band had issues with drugs and alcohol at some point as you eluded to everything from alcohol, weed, acid, heroin, coke, so most likely it was written based on there experience's But this album marked a big refinement in there music and really help boost them into the mainstream. Although it really wasn't until the black album that they became mainstream at which time many of there hard core fans felt they sold out. But in reality it was just the band evolving into what they wanted to do. when you listen to there early albums then each newer album you can hear this transformation.

Needle work away, never you betray. That and the line chop your breakfast on a mirror. The first part of the song is the drug speaking to the addict. You pretty much got the solos right. The euphoria of the high with the calm melodic guitar. Then the heavier guitar when coming down. Then the addict pleading to the drug. And then back to the drug taunting the addict.
I always seen it as the beginning is like the temptation the choice to travel that path. Then the middle section is as you said...getting high and mellow. The final heavier section the trap has been sprung and the hook is in deep with little chance of gaining your freedom again. Nice reaction. Metallica's first four albums are absolute gold, fire and brilliant.
