    These videos are FANTASTIC. I am in the early days of studying for the ICU primary exam and am following advice to start by reading G&H cover to cover - quite a dry task until I found these! Such a great adjunct to my study, thank you so much.

    In the vicious cycle of cardiac detoriation in cardiogenic shock it,the last sentence says " for this reason , in treating myocardial infarction is extremely important to prevent even short periods of hypotension" ,made me wonder,
    First.doesnt this make vasodilatators kinda dangerous considering that vasodilation decreases blood pressure ?
    Second. Does this mean that this hypotension thing applies to big myocardial infarctions where raised sympathethic activity and urine fluid retention arent enough to kinda beat hypotension ,or does cardiogenic shock hypotension happen after those phases in the decompensating phase ?
    I hope i didnt make the question confusing

      when it comes to the vicious cycle of cardiac detoriation in cardiogenic shock, the heart is uncapable to contract as well meaning, less blood flow, less blood pressure and less nutrients reaching the coronary arteries that maintain itself which intail worsens the hearts condition even further, thats where the vicious cycle part got its name. the reason you want to suminstrate vasodilators is because you need higher blood pressure caused by laminar flow in the arteries, its all about autoregulation in this specific scenario because you dont want your blood pressure lower than 80-90 mmHg when it comes to myocaridal infarctions. hope i didnt make the answer any more confusing
