A semi-subtle detail about this scene that I really appreciate is the presence of the security guards and people milling about at the start that end up leaving before the big moment. This could have been one of those scenes where we're all rolling our eyes at this lady being alone in a horror movie, this little lamb to the slaughter, gee I wonder what's going to happen to her. But the addition of the guards on patrol and being silently called away reminds the audience of just how quickly and quietly someone can be snatched away. You were with them not moments ago and suddenly they're gone forever. And that whole time that there were people around who could have helped her, there was a killer just waiting and biding their time in the shadows for the moment to strike and none of them had any idea.
Damn, the build up is immense!!! The fake out jump scare, the fact that the actual screen is the same as the scare, but it changes ever so subtly several times, the acting with everyone leaving the last second and the dreaded zoom in and the sound effect! *FUCKING GOLDEN,* i never really saw this before, but after watching the video about the history of jumpscares, it's just beautiful really
This movie deserves much more credit and recognition than it gets and this is a typical example of how studios can screw up a project out of greed. The title “The Exorcist III” upon which the studio insisted heavily, did a great disservice. It would have been much more successful as a standalone project if they had kept the original title “Legion” which is the original title of the novel that was based on. Again, written by Blatty. What’s done is done, but lessons should be learned. In my native country we have a very cruel proverb: “Greedy a*s eventually sh*ts blood “
If you have the eye for it, you can tell by the cinematography that it wasn't directed by a traditional "movie director". For the better, in my opinion.
Normally today, loud noises are considered a cheap trick to make unscary jumpscares scary. But in this scene, I almost think that the jumpscare is only enhanced by the sting, because you already have such a great build-up to the final scene, it leaves you both tense and confused, with multiple misdirections and fakeouts, and then when the scare finally comes it's a genuine shock. The sting just drives that horror in even deeper.
This film was pretty underrated by most I'd say----for my tastes I thought it was a worthy successor to the first classic(skip the dreadful 2nd film), and this particular scene was one of the most startling, unsettling, and frightening scenes in film history.
Me and my friend agree that aside from it being a good 'jump' scare, it makes you feel more than that, kind of shivers down the spine kind of scare and a disturbing image. Vey unique.

I'd be willing to bet that most of the people commenting here that this scene wasn't scary, or was a waste of time, jumped out of their own tits when the nurse was reaching for the glass of ice water and the patient popped out and started yelling.

She keeps getting frightened by little noises, but the scene is mostly very quiet, except for at one point when she screams after waking up a patient who yells at her. At the very end of the scene, she’s just checked one room, turns her back to return to the nurses station when suddenly a man charges at her from behind, draped in white sheets brandishing a knife as a burst of strings from the soundtrack interrupts the quiet.
It's pretty damn crazy and sad to think that while this movie was playing several people were being tortured and dismembered by the despicable Jeffrey Dahmer. R.I.P to all of the victims
I don't find this scary. It's like The Blair Witch Project. You don't see her being decapitated. Then you see her headless without her red sweater. And her nurses uniform isn't bloodied. Yawn.

1st of all, this movie is 32 years old; but to respond to your comment, the bar for horror is pretty damn low these days. It seems kids these days want to see the monster/demon/killer in practically every scene. Horror film history says that the creature should be shown sparingly, but modern audiences want nothing left to the imagination. The slow build-up to the reveal creates tension. The tension is diffused after the 1st jump scare, and then it begins to build up again before the "monster" is finally revealed, and only for a few seconds before it cuts to the next scene. Practically all modern horror movies are "loud noises", whether it's the non-diagetic music that screeches just as a "scary" moment happens, or the annoying shrieking sounds the creature makes that has become almost ubiquitous in this generation of horror films.
It’s the lead up to it with the cop coming and going, thereby creating a false sense of security, and the previous crackling sound turning out to just be ice, and the patient surprising her at first as well.
This lulls you into a state where you're just beginning to get bored, and then, just like that, it happens. Simply brilliant.
