    It's sad I unfortunately didn't have that option of getting my overbite fixed because my mother never gave a shit so I'm 32 trying to get it fixed I only pray I don't have to get surgery so I want to say thanks mom.

    A friend of mine had an deep overbite all his life and decided to close it in his early 40's. He wore elastics which pushed his bottom teeth forward. Took over a year.

    My 9 year old has this.. he has an overbite, crossbite, overjet, crowding, and retained baby teeth.. His orthodontist recommends 2 phases of treatment phase 1 is going to be first an RPE, which we are starting that whole process, he has spacers in now. Then, after 4 months of the RPE, he's going to get 2x4 braces on the upper and lower teeth with a Herbst appliance for about 12-18 months. Then, we'll finish phase 1 with bands and then possibly a retainer before phase 2. Unless a vast majority of his baby teeth are gone by phase 2, then we will just move directly into a full set of braces and potentially power chains to just straighten, align, and hold his teeth and bite into position. And finally, a retainer.. I hope we can end his braces' journey by 12-13, and by junior high, he'll have a perfect smile. 🤞

    I had my top premolars extracted by my father when I was 19 because of my overjet. He was a dentist. Unfortunately I am in braces again because I now have severe deep bite at 36 and my profile looks sunken in. I wish I knew that certain appliances at the right time could’ve saved me from this 😢

      Thanks for sharing! Sometimes taking out top premolars is an excellent choice and other times it results in a sunken in profile over time, which of course is a bummer!

      I even asked an oral surgeon who suggested jaw surgery, but my orthodontist said I would either need implants on my top teeth or extractions on my lower jaw which together with jaw surgery is way too invasive for my liking😬

      @@bracesintexas I have such a deep over jet that I had my two bottom premolars taken out and the gaps closes which gave me a even deeper overbite and will be getting my lower jaw moved 1.5cm forward, do you think this could of all been avoided if I acted earlier

      The extraction of healthy teeth needs to be criminalized. I had the same thing. I wish I had been given the option of expansion and proper tongue posture, which would've saved me years of health issues.

    Hi doc, I have recently got my braces removed apparently I had an over jet.. so my orthodontist had removed 4 teeth’s from upper jaws and 2 wisdom tooth from bottom jaws.. the braces treatment went on for 6 years. However, even now my teeth’s have more than 3mm distance btw the upper and lower jaws
    & it is a class II condition. I referred another doctor he said to either get jaw surgery instantly to modify it more or leave it as it is now and have retainers. As, of now I wear retainers and don’t wish to go for surgery. Is this ideal for me?

      Somewhere between 1-2mm is what we shoot for. 3mm is pretty close! Depending on tooth anatomy, 3mm can still function and look really nice. 6 years is a long time. I wish I could say that moving teeth was an exact science, but we definitely encounter situations where the teeth don’t move like we anticipate. Sometimes surgery can be a great option. I’m not sure why you would need jaw surgery instantly. It might be good to clarify this and see why. I think after 6 years of treatment, I would probably just sit on your current situation for a bit and then, if down the road, the extra overjet still bothers you, you could find a new provider to discuss the alternatives to try and reduce your overjet to 1-2 mm. I wouldn’t do the surgery “instantly” unless there was some major medical concern. Good luck! -drRP

    Yeah, my case. I have an overjet. My parents did nothing at the age of 10-12. As an adult I wore braces because I also had crooked teeth with gaps and an overbite. I had been warned by my orthodontist that nothing could be done with my overjet at my age (except for surgery. Even teeth extraction wouldn't give the stable result). However, all other issues were solved.

      @@dijaco15 I hear ya, but to be honest if it's really excessive overjet/overbite, it's skeletal in nature and an orthodontic fix would be camouflage orthodontics (you can look up what that is)

    Back in the 1970s, I wore an Andresen activator from age 10 for about 3 or 4 years (it felt like forever at the time), then an expander to sort the posterior crossbite and relieve the crowding. It reduced the overjet by over 10 mm and the molars went from a class 2 to class 1 occlusion. It didn’t help that my wisdom teeth appeared at age 11 and destroyed the roots of the second molars - the latter were quickly extracted.The final result is an even, if narrow smile, but I did develop TMJ problems at age 19, which was not too surprising as I am hypermobile.

    I have a 6 mm overjet and it does not affect my side profile I have a strong jawline still… I’m seeing a lot of people that have jawline problems and I wonder why I don’t maybe it’s just because my overjet is mostly only with my front middle teeth and ones on side of them?

      Thanks for sharing! Bites can be influenced by teeth and bone position. There are different combinations of these positions that can lead to a similar result. For example if your bones are in good support positions, but your teeth are in poor positions, you can end up with excessive overjet still. Come find us, if you want to chat more! Good luck! -drRP

    I’m in puberty, and man Im just depressed, yeah I did went to an orthodontist but they just put me braces and that was all, I have overbite, my lower jaw is underdeveloped and I seriously can’t take it anymore. I’m just eating and girls laugh at me, I’m not even a weirdo. I try to be funny and be the best version of my self but when I see my self in the mirror I cry. Where is your dentist located? What appliances could I get?

    Am 26.my upper & lower incisors r mild proclined & little bit gap between them. My Dr. Suggested me that i have 2 option. First one is I will have to go through Non-extraction in which the space will be decrease & teeth move backward only for 2 mm within one year... 2nd option is extraction, in which my teeth move more backward within 2.5 year n it will be long process... He recommended me that for my case there is no need to extraction without this I can get a good profile... Sir could u plz suggest me which option should I choose.... What's ur opinion on this plz reply...

      Hello! Here are my thoughts:
      1. Extraction and non extraction treatment options are often both viable.
      2. In my office we probably lean towards non-extraction more often than extraction. Most patients/parents prefer this.
      3. Hard to make recommendations without seeing the case. For mild crowding in either arch and a bite that is mostly class 1 then moving forward with non-extraction seems to be the best option.
      4. Find an orthodontist you trust and make your decisions based off his/her recommendations. This is the best option.

    Hey doctor, im 20 right now. When i was 15 i had my braces when i had massive deepbite and crowded lower teeth. For context i had 3 tooth extracted , 2 upper and 1 lower. I had it for around 2 years and few months of retainers.
    After that i was a little careless and didnt pay attention to my teeth, i now have a minor overbite and crowded low teeth again. Im 20 and im not able to close my teeth like i wanted and my chin is a little receding again. Should i get braces to fix the issue, this time for good ? ( I also hit puberty pretty later than most people )

      Hello!
      1. Unfortunately the only way to fix your teeth “permanently” is through diligent retainer wear. At this point, as soon as you stop wearing retainers you risk your teeth moving.
      2. Based on your description only, it sounds like you might be able to reduce your excess overbite (overjet), by aligning your bottom teeth.

    I'm 13 and have very severe overbite. My bottom teeth when I bite down will sink into my upper gums which hurts a lot. I started mewing when I was 12 and got braces around the same time. My mom was sick during pregnancy which is why my teeth were messed up. I believe what is happening is my jaw is angled upward which is why elastics wouldn't work. I hope my overbite gets solved

      Hello! Thanks for sharing. That does sound pretty deep. Braces are definitely a common approach to make your bite less deep. Sometimes you need something more than just braces. Mewing has not been researched as a means for deep bite correction. Good luck! -drRP

    I have a 10 mm overbite and I’m 20 years old . What do you recommend should I get braces where doc removes two teeth in r should I go for the surgery?

      Hello! Good question! Here are some thoughts:
      1. Trying to decide between the two options you mentioned is very personal.
      2. More than 70% of people who have two teeth extracted to resolve the extra overjet seem to be quite satisfied with the outcome.
      3. 10mm overjet is quite large and surgery may be the only option that gets you the closest to the ideal cosmetics. Especially if you are I satisfied with the way your face presents (small to no chin/low jaw projection) then surgery is the way you should go. If you struggle with sleep apnea, then surgery is also the way to go.
      4. Tradeoffs though: surgery can be a brutal recovery. This is a huge change that can be mentally and emotionally challenging to cope with. Taking out teeth, likely will not improve your facial presentation, but just make your teeth/smile look better.

    Hye, I have 5mm overjet and missing first molar due to decay. Can orthodontist fix it without extraction? It's not like because of those harmful effect of extraction going around, but I do not wish to lost any teeth anymore. I can't sleep for 3 days because of the first molar extraction.
    Can orthodontist use my missing molar's space to correct the overjet?

      Hello! Any upper space can be used to reduce your excess overjet; however, la large space on only one side of your mouth may end up creating an asymmetric eyesore. Your upper midline may pull to that side you have a missing molar when you close the so space. Keep that in mind.

    Hi! What abt molar distilization with TAD? I have an overheat thats pretty significant and got a 3rd opinion today where he mentioned moving my top teeth back one by one until the overjet improves

      Hello! Here are some thoughts:
      1) Distali5ation with TAD's, extractions, or springs will lead to the same result = pulling back your upper teeth.
      2) TAD's in theory allows you to keep all of your teeth and get a similar result to extraction of upper premolars and retraction.
      3) Moving teeth back one by one is quite the long process. Not impossible.

I brought up the distilization with TADS and she said I’m not a candidate bc of my age 33..and that wouldn’t yield the result I need :/ says it’s either extractions..continue with my rubber bands or just have the braces removed :( my teeth are straight but the correction caused more of an overjet..I wasn’t expecting this

    I am 14 and have had orthodontic treatment for over 6 years. I still have a bad overjet and deep bite even after all these years because my ortho is refusing to give me rubber bands and only tightens my braces every 2-4 months. SInce I am 14, is is too late to fix an overjet with just rubber bands when my ortho finally gives them to me? Im worried that my ortho waited too long to fix it.

      Oh man! 6 years is tough. Be sure to follow all instructions, maintain good oral hygiene, keep appointments, and all other aspects of your agreement with your provider to try and help move things along! We usually see our braces patients ever 4-8weeks for adjustments. Overjet is best reduced during a signficant growth spurt. Assuming you still have that coming then there is still hope!     Hey Dr. Iam 17yrs old and i have actually a noticeable over bite and a little bit facial assymetry.. Not in my lower jaw that's with upper.. I think,
    Also i have those adinoid problems i think that was the Result when my 13s and 14s i sleeped with open mouth. With out knowing these thing's..
    Now I have overbite, Adinoid enlargement, and facial assymetry -(but it's not that big)
    And what i can do brother..
    Do these thing's can be cured by braces ? At 17

      Thanks for the reach out! To your thoughts regarding excessive overbite (overjet), adenoid enlargement, and facial asymmetry, these are influenced by multiple factors. Science has NOT been able to isolate single causes/etiologies in any reliable way that is agreed upon by orthodontic community at large.
      1. One potential factor in developing a receded or set back lower jaw has to do with a poor growth pattern. For example, sometimes there is insufficient vertical growth of the mandibular ramus. If your lower jaw is “short” then it has a hard time rotating forward to “catch up” to the upper jaw and you end up with excessive overjet.
      2. Adenoid/tonsil tissue from a growth and development standpoint is reported to follow a pattern of rapid increase in our childhood years and then naturally reduces it size without any treatment.
      3. “Mouth Breathing” can refer to different things. It remains unclear, insufficient evidence through repeatable and well designed studies, what the exact relationship is between “mount breathing” and development. Yes mouth breathing, in extreme cases can have affects on development, but there are people who “mouth breathe” that have good jaws, no issues with tonsils, and fairly symmetrical faces and then vice versa.
      I’d visit an orthodontist you trust (get multiple opinions if needed). 17y/o probably means you’re past your peak growing years which will allow you a few options: 1) leave jaws as is, evaluate your tonsil tissue with ENt provider, consider allergy contribution to mouth breathing and other anatomical obstructions, 2) extract upper premolars to camouflage the excessive overjet, or 3) consider surgical advancement of the mandible.

    hey doc, I am 15 years old and have a 5mm overjet and my ortho said extracting 4 premolars will be better but he aso suggested to use tads without extraction but it wont be 100% correct is this true?

      Hello! Great question. Perhaps your doctor feels that in your case using the extractions will get you closer to the ideal than just using TADs. When you remove teeth, you create about 7mm of usable space and then new space is usually closer to where it’s needed, vs. with TADs you may have to work harder to create similar results. It depends on your orthodontist’s plan! Good Luck! -drRP

    I’m 28, and I have a 12mm over bite :( jaw surgery is out of the question& they don’t want to do extractions, but they said rubber bands would correct it by 50 % 🙁 what’s your opinion on this?

      Hello! Good question. Here are some thoughts:
      1. 12mm is significant.
      2. If they don’t want to take out teeth it maybe because they feel like your lip support will suffer post extraction and space closure.
      3. Sometimes you can use TADs to help support your tooth movements, increasing your ability to accomplish desired movements.
      4. To say it will improve 50% is questionable to me. Could your overbite get better? Yes! Will it get all the way better? Don’t bet on it! Will you be able to improve it by 50%? Seems unlikely. Part of the success will depend on your ability to wear the rubberbands more than 20hrs everyday for several months to year(s) and how well your adult teeth/mouth respond to that pressure. Maybe they are considering using a motion device, which could increase your chances of a moderate improvement, but again, so much will depend on if you can wear the rubber-bands sufficiently.
      5. It’s not a bad thing to try with just rubberbands, but surgery is definitely the most predictable option, in a case like this, to get to the most ideal cosmetic/functional/stable result.

    My daughter is 10. Severe overbite and v shape pallate.
    Ortho is correcting vshape first with expanders and after 6 months remove expanders and put braces on the top. She didn’t mention about any other tools to help with the overbite . My daughter also has JIA. Juvenile arthritis but it’s under control . Her condyles are flat but there’s no more inflammation for now. I don’t want surgery and I’m afraid she will miss out on a treatment that will really benefit while she’s growing . Wish I was in your state . So overwhelming . I will appreciate any advise from you . Thank you so much

      Hello! Excessive overbite can be hard to fix without excellent compliance or a device that is fixed in place. You may want to review your plan with your provider to confirm the tool used to address the overbite (overjet). If it is rubberbands, just know that it’s all dependent on your daughter to make it happen. JIA is tough and difficult to predict how condyles will respond. Another reason to stay close to your provider in order to monitor closely. Good luck! -drRP

    I have my upper jaw outside like overbite and all of the dr said that there will be the one and only solution that is surgery and yeah I am a college student and I know how much I suffer 😢 can this help me I m very scared of surgery even I put braces cause my teeth are also 😢

      Once you have assumed your adult form and bone structure, the only predictable way to change your bones is surgery. Depending on the severity of your overbite (overjet), you may be able to resolve it by taking out teeth or with the help of temporary anchorage screws.

    I was wondering if my orthodontists wanted to remove two upper premolars due to overjet that was created to make space for a impacted tooth should i get the extractions, and what will this do to my impacted tooth coming in and my crossbite of the back teeth?

      Great questions! Sometimes due to the amount of crowding it makes sense to consider removing a premolar to make space for an impacted canine.
      Sometimes it also makes sense to remove upper premolars to reduce excess overjet.
      Crossbites are correctible regardless of if you extract premolars.
      My recommendation is that you find a provider you can trust and whose treatment plan you can get on board with! Your description definitely sounds like lots of work!

    i'm 14 turning 15 this year and i got braces 6 months ago with elastics, in my opinion i don't think i have an overbite, i think i have a deep bite because my lower teeth kept touching my upper gums, is deep bite and over bite the same thing? anyways i'm not sure if it's gonna correct my bite but i'd love to hear your opinion about it

      Technically, overbite refers to the depth of your bite (how much your top teeth come down over your bottom teeth, not how far forward your top teeth are compared to your bottom teeth). Sometimes by fixing any the "overbite" (how far in front your top teeth are compared to your bottom teeth = overjet), you can make your actual overbite (depth of your bite) less!     from India ,I’m 18 year old and my dr gave me twin block it’s been 1 month , ☹️and I look like I’m on growing stage cz my height is 5’ and my 🦶🏼 look’s little big nowadays and my class 7 shoes still lets me but nowadays I notice my fut is little bigger, and whenever i ware my twin block and then remove it , it feels like my jow is little dif place , hopefully everything will be okay

    hello dr. Im 22 years old and I came to know about mewing a month back amd all these years my tongue used to down on teeth and Im a mouth breather since 4 standard. Due to which now I have a very long face with asymmetry and very poor jawline extremely receding chin and I have a v shape pallate and over crowded teeth so there isnt space for tongue to rest on roof of the mouth. I have visted so many dentist nobody ever told me about me mewing or how to cure mouth breathing . Please tell me what procedure i should go through please sir all my facial features are ruined

      Hello! Mewing is not a well studied technique. There is no peer reviewed evidence that it does what it claims to do (that doesn’t mean it has no effect at all…).
      Have you visited a local orthodontist? Mouth breathing is challenging to fix. How do you know that you have been “mouth breathing” since you were 4? It’s more complicated than just practice keeping your lips together and you’ll never mouth breathe again. When you sleep, you can lose neuromuscular tone and your airway can collapse or your lower jaw can relax, just like you have no control over your bicep muscle while you sleep (you can’t keep it contracted).
      Check in with an in person orthodontist to evaluate you bone growth/direction and tooth position to decide if there is an adequate way to improve overall facial balance/harmony. Good luck! -drRP

    What should i follow until age of 24 years to not worsen current existing position.should i Follow perfect jaw position like u mentioned to put upper teeth slightly over lower teeth or stay with my current mild overbite position and not worsen the scenario.i feel like whenever I try to practice the perfect jaw position mentioned I feel like my jaw strength is weak.what should I do doc?Also I tried to start mewing few years back and now my default position of teeth is in such a way that both upper and lower teeth does not clinch anymore during reting my teeth

      Hello! Thanks for sharing! Everyone should have mild overbite, somewhere between 1-2mm. You can do jaw exercises to try and strengthen your jaw, but that may or may not make a difference. Mewing is a posturing technique that hasn’t been proven to be effective in the peer reviewed research. Good luck! -drRP

    I have 9 mm overbite and I am 27 years old. I wanted to fix my overbite without surgery that’s why I consider Vivos oral appliance . I have it for 6 months and my concern is that my space between lower and upper jaw is bigger than before . My dentist is saying that is as a result of widening the upper jaw faster than the lower jaw . I am little bit concerned and I am not sure if I should continue with this process , to considerate fact that I have paid half of the cost for the Vivos . Please give me your opinion if you have hear before for this pallet expander device

      Hello! The Vivos oral appliance for those who struggle with sleep apnea? Unfortunately, for an adult there isn’t any appliance, that has been studied and peer reviewed, that will fix an overbite that large. Surgery, extractions, TADs, would be required to reduce the overbite to ideal.

      Hello! Thanks for the comment! Here are some thoughts:
      1. What does it mean to be a life time mouth breather? Do you breathe only through your mouth? Do you breathe through your mouth sometimes? We all breathe through our mouths at some points in our lives, if not daily, based on our need for air. Some individuals have great vertical growth potential of the lower jaw which has the potential to overcome “effects of mouth breathing”. Even without great growth, we don’t know how much/what percentage for what amount of time of “mouth breathing” is required to actually cause a change in craniofacial growth. We have seen through animal studies that a complete, 100% blockage of nasal airway can lead to changes in head/jaw posture that can be reversed once nasal passage is opened back up.
      2. Nasal breathing is a healthy practice.
      3. Mewing is a posturing technique that is reported to help with growth and development. Currently there is no repeatable scientific studies that are well designed and peer reviewed that support this technique to grow jaws as an adult.
      4. That being said, orthodontists (Dr. Mew is/was a licensed orthodontist) use the idea of posturing the lower jaw forward during the years of puberty, in particular, to modify growth. We aren’t growing jaws, we try to help control the direction of growth and hope that the available growth potential stabilizes the new jaw position. Growth potential appears to be the key. Current evidence suggests that we aren’t growing the lower jaw beyond what is programmed in the genetic code, at least in any significant way.
      5. Age does not always correlate with growth potential; however, on average girls are likely done with growth at age 17 and boys are getting close to finishing, but again, it all depends on the individual. If you are done with your major growth spurt, it is unlikely that any posturing technique or exercise will reduce your overbite in a stable/unchanging way.
      6. If it’s possible, I’d visit with a specialist to see where you are at on your growth curve.

    I have an overjet (or a recessed lower jaw). Orthodontists kept putting me on hold until I was 14, then admitted that my case was just too complicated and not worth their time. I went to a dental hospital and had to wear a full mouth brace overnight, every night. But it didn't realign in the slightest. I'm now 42 and what bothers me is that my neck looks bad because the lower jawline is so weak looking. You end up looking like you have a double chin.

      1. Bionator is another technique to try and reduce excessive overjet via growth modification.
      2. One of the challenges associated with the Bionator is compliance. Can you wear it enough to see the same type of results that you would likely see with an appliance that is glued into place, like the Herbst?
      3. At age 16, and as a female, another question is, how much growth do you have left? Not that it won’t work at all, but the results may be less predictable if you have already started your cycle.
      4. Sometimes, it is still worth trying to see what changes can be obtained, but if it is unsuccessful in reaching the desired results then you may have to consider removal of upper premolars or surgery down the road.

      Hello! Thanks for the question! Growth modification that I refer to on my channel is for children/adolescents who are going through their pubertal growth spurt. Adults cannot benefit from this type of treatment as they are beyond this growth spurt. Although we continue to grow throughout our lives, nothing in our adult lives compares to this amount of growth. At your age there is unlikely to be “growth modification”! What is the appliance? If the orthodontist or dentist (you sure it was an orthodontist?) suggested growth modification at your age, I find additional orthodontists to review your case with in person, like me! I have bunny teeth too. I really don’t want surgery or teeth extraction I know it’s hard to answer properly without seeing in person but is there a chance for me to fix it with braces and rubber bands?

      1. Significant overbite (overjet) can be a result of upper and lower jaws and teeth (in the back) that aren’t positioned correctly, or it can be a result of the top/bottom front teeth that are out of position while the back teeth are in good position.
      2. If it is the first issue, then rubberbands more than 20+hrs is likely needed, at least for some period of time.
      3. If it is the second issue, associated with the front teeth (such as flared upper front teeth, maybe there is spacing in between them), then rubberbands at night time may be all that’s needed to preserve the good position in the back.
      4. I would communicate your desire to correct your overbite (overjet) as much as possible too make sure you are all on the same page.

      Compliance is key with Invisalign!
      In addition, there are some providers of Invisalign (they may not be orthodontists), that just offer the Invisalign to help straighten teeth, but they don’t discuss overbite correction with their patients. You might spend the same amount of money or more with this type of provider and get less out of the treatment time.
      Definitely want to be open about your treatment goals with your provider.

      1. If you have concerns about your braces, definitely speak with your provider.
      2. Crooked teeth and/or an unresolved bite issue, never killed anyone nor has been documented via research as a primary factor in developing a major medical issue.
      3. That being said, crooked teeth and/or unresolved bite can contribute to issues with cavities, gum/bone loss, developmental jaw problems etc. Key word is “can”. It may not, but it can.

      Well!!! Lingual braces?! You are talking about braces that go on the backside/inside aspect of your teeth. If you can find the right provider, these appliances can help you get similar results to front braces. The trick is finding a provider who feels comfortable accomplishing this. These appliances are not as common in the states when you compare them to traditional front braces.
      As it relates to the herbst…you can use the appliance as a distalizing appliance at your age. It can push top teeth back to help reduce overjet, but you aren’t getting any growth modification. There are other appliances that you could consider that aren’t as big/bulky to accomplish “pushing top teeth back”.

    My almost 13 year old daughter has an overjet, perfectly straight teeth, a weak looking chin and her jaw is small. She’s becoming self conscious about her face bc she has a double looking chin. I have mixed emotions with her having braces but now that she’s becoming set conscious and seems to want the braces I’m leaning more towards wanting her to have it while she’s still young.       While she is young, my preferred option to address a lower jaw that is set back is through growth modification. The older a female is the more challenging to get predictable results with this technique because they usually finish growing sooner than than the boys. Other alternatives include surgery to specifically address a lower jaw bone that is back. There are other options that try to pull the top teeth back to the bottom, but that wouldn’t fix the lower jaw. Come find us if you want to discuss specifics! Good luck! -drRP

    Hey I have an overbite of 10 mm and my orthodontist referred my to do surgery when I went the consultant he said my soft tissue disguises it well and scared me about the surgery. so I went with the upper 2 extractions and now I’m regretting it. I looked at some options on getting a genioplasty after orthodontist treatment is done do you think that is possible .

      Hello! Yes, genioplasty is possible after ortho is done. It’s a bummer you regret your choice. Some individuals, like you said, have soft tissue that mask/hide the fact that your lower jaw is considerably back compared to your top jaw. In these cases taking out top teeth can be desired because you avoid the surgery. There is a large percentage of people that is usually just as satisfied with this option as people who go through the surgery. Hope you get what you want. Good luck! -drRP

      1. Not everyone who has teeth taken out on-top to fix excess overjets will have a sunken in face. It depends on your pretreatment face type and lip position.
      2. That being said, when you tip the top teeth backward to try and reduce the overjet, you are reducing the lip support and so the upper/lower lips can “sink in”.

      flattened cheeks, thinned lips, hanging columella, trapped recessed lower jaw, scleral show, premature wrinkles around mouth, deep nasolabial folds, marionette lines. Now add some health problems; snoring, sinusitis, rhinitis, eventually having difficult time to nose breathe, rapid breathing, post nasal drip, chronic bronchitis, tongue biting, cheek biting, eustachian tube blockage, tinnitus, migraines, compressed condyles. Eventually, you may pass the high bar for a diagnosis of sleep apnea, which will come with the added benefits of possible high blood pressure, diabetes and stroke.

      @@bunb8541 i talked to my orthodontist about all these after effects. he closely examined my face and teeth. he told that all it would do is get my lip back, and thats the main goal because my lips are protruding and they look really thick, which doesnt suit my face. so maybe it wouldn't be so bad for my case? also about the flat cheeks- i have a baby face (chubby cheeks) so, who knows. i used to be called rat face when i was small, so honestly, i would love to get my teeth fixed... lets see now. thanks tho. if i have any health issues in the future, i will see to it.

    Hello. Thank you for the great video. Our son has Class II Div 1, early stage of puberty, has narrow dental arch, and was suggested the removal of first premolar thereafter MARA and braces to take advantage of the growth modification. Reason for teeth removal was that there is insufficient room to set MARA over the lower molar (forgot which molar). We understand that once the lower jaw is moved forward, his overjet could be controlled with follow up braces and would not look substantially out of proportion. However, is removal of teeth really necessarily before starting on MARA (or any functional apparatus)? If the narrow arch is the issue, can he try the expander (RPE or MAPRE)? We understand that the midpalatal suture density ratio is one essential factor for choosing the expander options. Because he is at the cut off timing for the traditional expander (11~12), what would be the our options for discussion if we want to avoid teeth removal? Also, we would appreciate hearing from you about the treatment plan where the expander is combined simultaneously with the functional apparatus. That sounds like a good plan to me, but does not seem to be a popular approach. Why is that? Also, we get that the expander widens the arch, so once the arch is widened which then is followed by the braces to achieve the ideal dental profile is understandable, but then would the functional apparatus (Herbst/MARA) alone also widen the dental arch? Because the functional apparatus seems to be more so for promoting the forward jaw development, but does not necessarily seem to widening the dental arch, we are just confused about the difference. Any feedback is deeply appreciated. Thank you.

      Side note, we addressed about the need for orthodontic treatment since he was 4 years old with our pediatric dentist. SINCE 4 years old!! And, Dr. kept stating you do not need to see orthodontist now, wait till close to or all permanent teeth are erupted. !!!! We should have taken more proactive approach, too late now, but we hope that it is not that too late…

      Hello! Thanks for sharing! Alot to unpack here. I personally have not heard of, in any common approach, the need to remove teeth in order to make a functional appliance fit in the mouth. If you did have a fit issue and needed to take out a tooth as a result, that seems like it is the exception rather than the rule. Sometimes tooth removal is recommended in addition to class 2 functional appliance therapy to put the lower anterior teeth in the best position for overall soft and hard tissue (profile, lips, smile, bite, etc) cosmetics and occlusion. In cases with lots of crowding or protruding lower incisors and a set back lower jaw, you can fix the bite with a class 2 functional appliance, but the profile may suffer (not the end of the world and maybe not even bothersome to the patient) without removal of certain teeth. It depends and you'd want to review with your provider on that.
      Expanders are not the key to limitless bone. Yes you can expand, but should you? How much extra space do you need? What are the effects of expansion on the teeth in the back next to the ramus of the lower jaw bone? What are the long term risks to your gums and bone by expanding now? All questions to consider or review with your provider because every case is different.
      Somtimes people do add an expander as part of their Herbst appliance, but it's not so much because we want space as much as we are trying to coordinate the upper jaw with the lower jaw in it's new position.
      Review with your provider. You want to make sure you find someone you can communicate effectively with to understand risks/benefits/why's etc. Good luck! --drRP

As you mentioned, we also thought that removing the teeth in order to fit the functional apparatus is out of norm. But we also know that his dental arch is narrower in the bottom, adjacent teeth is really tight touching each other (but no crowding). Anyway, we now understand that, he does not need to wear expander. We were really worried about missing the optimal timing to wear expander if he needed to wear one, as he is already at early puberty.
      He still has time to ride along with his growth spurt so we will be looking at the functional apparatus, whether it be Herbst, braces/band, or other apparatus. Now we were told MARA requires teeth removal, what about other apparatus, like Herbst? When the space between the teeth are so tight, can he even wear Herbst? Also, we read that Herbst combined with TAD shows promising result (to avoid undesirable inclination of lower front 4 teeth). If you have a patient in early puberty (Class II, Div 1, excessive overjet like 14mm and over, Deep bite, otherwise no crowding), would you suggest or be comfortable with Herbst with TAD treatment plan? Or, how do you, as a Orthodontist, decide on Herbst with TAD (this is not standard plan, correct?) for patient like our son?
      And, lastly, this is just my curiosity, that what I understood about mandibular advancement is to promote the condylar growth (the amount of new bones formed at the condylar by wearing the functional apparatus, that amount pushes the lower jaw forward, is this correct?). Is there any research paper out that you know of which compares the before/after condylar to show effectiveness of functional apparatus? That's just a geek in me wanting to read up on that subject.     Hello, I was hoping I could get some opinions here? I just had a consultation appointment, waiting on a call from my orthodontist. We haven't discussed treatment plans yet, but I wanted some opinions on what treatment to advocate for based on my wishes and concerns:
    * Moderate to severe overbite (dont know exact mm, front six teeth protrude quite a lot)
    * Hoping treatment can fix aesthetic profile, in my side profile my front lip protrudes over my bottom one noticeably, which puts pressure on my bottom lip causing me to have a pouty look to my face which I don't like.
    * My jaw doesn't look super recessed, but it does have pressure on it from my overbite which makes it look like I have jowls, and im only 18 :(
    * Jaw surgery out of the question, I dont want it
    * I had orthodontic treatment a long time ago but they just put braces on and didn't correct my bite, I almost got extractions but last minute my orthodontist said it would "ruin" my profile, I still don't know what he meant by that but it scares me.
    Would you suggest the rubber bands/springs or extractions for my case? This is my second time getting treatment and I wanna make sure I get it right this time. Thank you!
    EXTRA: my brother had very similar teeth to me, had orthodontic treatment for nearly 4 years, he had rubber bands and they did a bone graph on his lower gums in order for his lower teeth to be able to go out further to correct his overbite. However, I can't find anything online about this. Is it a relatively new treatment? I also wanna consider this option if possible bc he has a perfect profile and smile. ty!!

      Thanks for sharing! Here are some thoughts:
      1. Based on what you are describing, you have excess overjet and maybe even a deep bite.
      2. At age 18, growth modification to address a receeded lower jaw is likely out of the question.
      3. Surgery often has the greatest ability to improve profile, smile, and bite most predictably.
      4. If surgery is out of the question, taking out too teeth can help reduce excess overbite (overjet). If your upper lip is considerably forward relative to your nose, than this could be a satisfactory option, but if your lip sits back behind your nose, then this could lead to a “larger” appearing nose and sunken in profile over time (less lip support).
      5. The option you described is not common, although most orthodontics are aware of it. You’d just have to find someone who is familiar with the technique!

      I have a question, what do u mean "lip sits back behind nose"? My upper lip does forward a bit but i feel like it's mostly my bottom teeth id want pushed forward more than my front teeth being pushed completely back if that makes sense. I just feel like it'd look better on my face, but idk.
      Do u think the bone graph option would work well for me? I think it sounds like an easier version of the jaw surgery right? because it kinda builds the lower jaw forward if i understand correctly

      Some people have lips that come out beyond the point of the nose. If your lips are like this or somewhere near this point, then taking out top teeth to reduce the excess overjet could be a reasonable camouflage option for your profile. A lip that already is considerably behind the point of your nose (many old people) have flat looking profiles. Taking out top teeth in a situation like this would make the profile worse!
      Yes, often the lower jaw is the problem and is what needs addressing. A bone graft to allow lower anterior teeth to come forward is a technique orthodontists are aware of, but not commonly implemented.
      You’d have to visit a provider and have that discussion in person!

      1. Overjet can be challenging to reduce for sure.
      2. Surgery can sometimes be the most predictable route, particularly for adults, to achieve the best cosmetic, functional, and stable results. There is sometimes a surgical first option. This may or may not get you to what you need before your wedding (surgical first).
      3. Other option likely would include extracting upper premolars + rubber bands. This option is great for individuals whose lower jaw appears to be in a relatively good position compared to the entire face and has an upper lip that appears forward.
      4. All options are UNLIKELY to be complete in less than 1 year!

      If you want to just straighten your teeth, you could reasonably consider either braces or clear aligners.
      If you were trying to fix the excessive OJ, then aligners, in your specific case, are probably not the most predictable option to reach the desired outcome, unless you were down for surgery! -drRP

    Went to an orthodontist. They suggested taking out 4 premolars (2 at top and 2 at bottom) plus all 4 wisdom teeth. My overbite is not so bad but they say it's because my teeth at bottom are too crowded. I don't want to take that many teeth out and have sunken in profile. Plus my lower jaw should be moved forward and this option you mentioned in the video would've been perfect but I'm 23. I guess I would be fine with just straighter teeth if that means not taking out 8 of them in total :( heeelp?

      Hello! Here are some thoughts:
      1. 23 means no growth modification for you.
      2. The only other options to resolve any excess overjet is with surgery and or taking out teeth (depending on how excessive the overjet, using rubber bands to reduce the overjet via pushing top teeth backwards may be an option).
      3. There are many people who leave their excess overjet and go with straight teeth. None of these individuals, as far as I know in the research, have died as a result of the excess overjet.

      Hello! From a growth and development standpoint the actual chin tends to, on average, become more prominent as we age, particularly as we go through puberty. It’s not that the chin “grows more forward”, but rather the lips and bones adjacent to the chin point change giving the appearance that the chin is “growing forward.” This is under normal growth and development. If you think your lower jaw is getting further back, then it might indicate some sort of discrepancy in normal development that you could get looked at. Good luck! -drRP

Extraction of upper premolars would make it smaller, but surgery may be the best option for the greatest reduction in total overbite (overjet). Good luck!       In theory, if you could use all of the extraction space for reduction of the overbite then it would probably be about 7-8mm because that is the average size of a premolar. It is hard to get this amount without temporary anchorage devices (basically temporary implants). Surgery is expensive for sure. Be sure to weigh out all of your options. May take some time, but probably worth the evaluation. Good luck! -drRP

    I'm 22, I've a 10mm overjet and a narrow upper arch. My orthodontist is saying he's trying to expand my upper teeth. But he's just using braces and bite turbos on the lower 2nd molars. He used a bite plate for first 2 months. I've developed gaps between my incisors and canine. Now, he's using power chain on my first four upper incisors only. My smile looks far better in just 7 months of treatment. But, I'm still worried about my overjet. It's not reduced that much. He's doing it without extraction. He has not used any rubber bands yet. He's saying I might need to extract my upper wisdom teeth (3rd molar cause they are projected outwards).

      Sounds like you are working hard! Based on your description, an overjet of that amount is unlikely to resolve in a stable/predictable way without extracting upper premolars and/or surgery to bring the lower jaw forward. Perhaps your orthodontist is attempting to autorotate your lower jaw forward?? If you are concerned about resolving your excess overjet, don't let time go by without getting a solid understanding of the plan to resolve or attempt to improve. Ask about predictability. Wisdom teeth, generally, have no significant impact on correcting excess overjet. Good luck! --drRP

      Hello! Extractions and IPR both can be used to create space to retract (bring back) teeth. Extractions usually give you more space than doing IPR. Sometimes you don’t need as much retraction or patients are comfortable if 100% retraction is not accomplished.
      My recommendations are usually based off individual circumstances and goals for treatment outcomes. Either option could be beneficial, it just depends on you and your provider!

      That’s right! When you move your jaw forward your back teeth won’t touch limiting your ability to chew. There is a good chance you have mandibular retrognathia, which means that your lower jaw back compared to your top jaw (disharmony of your jaw bones, it may be big or small).     I’m 38 and the orthodontist wants to take to teeth out from the bottom and through invasaline, push the lower jaw backwards as I’ve overbite. My concern is will my jaw shift due to my age? As I don’t want to end up with missing teeth and gap. Two, my mouth majority of time is open with my tongue a bit out even though I’ve my lips closed. Will the procedure also help with this and jaw clicking? I need a second opinion and would appreciate your input.

      Hello! Here are some thoughts:
      1. Taking out teeth from the bottom jaw only is generally, not the solution to correct excess overjet (what people often refer to as overbite).
      2. Taking out teeth and aligning the rest of your teeth ideally can be mechanically challenging for Invisalign unless there is a lot of crowding involved.
      3. When trying to correct an underbite, taking out lower teeth is definitely an option. It’s not that your lower jaw will shift back, but rather the bottom teeth tip back to try and gain positive overjet, where the top teeth are in front of your bottom teeth.
      4. Orthodontic treatment with braces and/or Invisalign is not treatment for joint clicking.
      5. Correcting underbite or overjet to an ideal position can help with tongue positioning, but may require myofucntional therapy where you try to train the tongue/other facial muscles to be where they needs to be.

      I have class 2 overbite my dentist suggest DJS to correct my bite but another dentist suggest just to take off the premolars of the upper jaw and maybe go for genioplasty
      I really don’t know what to do most of people told me that I look pretty the way I’m but I’m so bothered by my side profile also the surgery is too expensive for me 😞 and challenging for sure
      My question is how can I know that the option of the extraction will be a good option for me ( cosmetic way) because I don’t have sleep apnea or medical issues

    What's the best you can hope for in terms of growth modification if you treat it before the growth spurt? Is there an average amount of movement in mm or cm?

      Hello! This is a great question! Here are some thoughts:
      1. To be clear, if growth modification is attempted too early, well before the adolescent growth spurt, then it likely wont work, and the lower jaw will move to its original backward position.
      2. If you time it correctly, then a 5-7mm overjet correction with growth modification is not out of the realm of possibility. This is based off of my personal office experience. There may be research I am unaware of that suggests an average.
      3. As the amount of overjet increases beyond 5-7mm, the predictability of correcting it with only growth modification goes down. Sometimes providers will suggest a combination of growth modification and extraction of teeth.
      4. 1 cm is a very surgical sounding movement, however, surgeries have been performed on people with less overjet.
      5. The greater the overjet to change, the more challenging it is to stabilize it.

      1. If you are an adult, the good news is there is not a rush to to get the orthodontic treatment, besides just wanting to fix the overbite, because your treatment options likely are the same now as they will be when you can save up enough money.
      2. Overbites aren’t a major medical issue. I know it looks better when they can be fixed, but at least you know you can still be healthy with an overbite.

      1. Extracting upper premolars to pull the top teeth back towards the bottom teeth. This may reduce your total lip support.

      3. Everyone should have a little overbite, depending on if your overbite is not as excessive as you think, you may be able to treat it predictably with rubberbands and some alignment tooth (braces, clear aligners).

      We have a high success rate if we get the device in during the adolescent growth spurt. We don’t claim to grow the mandible larger than the programmed genetics allow, but we try to control the direction of growth! It is definitely a great and quite predictable option!     I have like a very minor overbite/overjet. At the most 2.5 mm from normal (probably lesser) and it gives me a very small lip incompetence but no gummy smile and a cute tiny philtrum. But im 18 so lets hope the overbite isnt skeletal and its just dental . But if it comes to it I would rather have jaw surgery than get premolar extraction
