By Kerbal standards, nothing actually went wrong with this mission, just another routine mission, only a few unscheduled explosions. And ONLY ONE Rapid Unscheduled disassembly, quite amazing if you ask me.

Believe it or not, there is something akin to an atmosphere on the Moon. While it's not nearly as dense as the atmosphere of the Earth, due to lower gravity combined with incredibly low pressure, it still does exist. However it may not be properly modeled in KSP due to being as thin as it is. 39:10 you get 2000 points instead of 1000 for each: a) very efficient instead of efficient (>700m/s left) mun landing b) very efficient instead of efficient kerbin return landing. Problem in your case - at beginning of video 9:00 you undocked from second stage fuel tank to dock the lander into upper main capsule module. Had you instead undocked main module, turned it around and docked it into lander module+second stage fuel tank, you could've use second stage fuel tank fuel to fly to moon. You also used upper main capsule module to catch kerbals in orbit, instead of using lander rcs to approach upper main module. That would have given you lots of fuel in the main capsule module, and you would get gold.
