It would be helpful to do a vid on TRT and dutasteride. I was waiting for that as endocrinologist would often advise dut on TRT
The message would be the same. Only since Dutasteride blocks most of the DHT, concerns would be far greater.

Hello Doctor, question about Finasteride and TRT Acne. I've been taking Testosterone since last August. I got blood work last week and my total and free testosterone are a little too high but that was to be expected. My estradiol is completely normal. But my DHT levels were very high. I keep getting flares of acne on my back and I believe it's DHT induced acne. Do you think taking Finasteride can lower/clear body acne when someone is on TRT?
The unfortunate thing is that Finasteride increases the level of testosterone by 15%. It is since it prevents conversion to DHT.
Sir could you please make a long explaining video about how to come off of TRT safely and start an effective Post Cycle Therapy "PCT " to re-start natural production of Testosterone within the body.
Because we get confused to receive many types of answers from the local clinics. And our purpose is to be on exogenous Testosterone for 2 to 3 years to add more muscles to ourselves and then to completely come off of the use of the steroids

Hi Doctor! Amazing video, thanks for posting. One question -- Related to your discussion of Boron. I recently had my free Testosterone tested, and it came in on the lower end (51, normal range 49-150). I am on finasteride. Would it be safe to supplement Boron, or will it likely accelerate my hair loss?
You can supplement Boron. It does help in hair regrowth.

More great information. Acknowledging that many athletic people are using testosterone. Many steroids especially testosterone can be used safely. The abuse of exogenous testosterone and steroids is dangerous. Testosterone just like finasteride is very safe when used correctly.

Amazing content Dr. Bhatti. I'm about to start TRT cream. I've had several hair transplants. Just about to Start topical minoxidil & Finasteride I also get PRP hair treatments every 6 months. Anything you suggest to combat the DHT from the TRT? Thank you!
I would only suggest Oral Finasteride. Rest will nto make any significant difference.
Very wise. I saw a hair transpant doctor who gave me the same advice regarding minoxidil and PRP as I'm already on finasteride.
It is HPTA axis. Hypothalamus pituitary gonadal axis.

Hi Dr. Amazing content, I’m 40yo 5’10” 190lbs, I workout regularly and have been experiencing hair loss over the last few years. I am considering finasteride and my total testosterone is 795ng/dl and free is 26.4ng/dl, I haven’t gotten my estradiol tested, but based on this information would I be in the lower risk category for side effects? I plan on starting with 1mg EOD / 3mg week
This is an amazing level. With the information you provided it will be surprising if you have a side effect with Finasteride.

Dr. - Can you elaborate further on topical finasteride and how effective it is at binding with DHT receptors in the scalp - is it localized in scalp tissue or does it go systemic? To what extent?
The technology that can make topical Finasteride reach the hair follicles direct is still in its nascent state. For now Finasteride reaches follicles through the blood stream whether consumed orally or applied locally.
As a first time watcher I really appreciate your content. It's quite informative. Doctor I am 22 and have been on TRT since 19 when I was diagnosed with isolated Hypotropic hypogonadism. I take 250mg Testosterone enanthate every 20 days. Since a few days I have an itchy scalp and am also losing hair from the front. I don't understand whom to consult in such a case because my endocrinologist says its rarely due to TRT while I firmly believe it's due to TRT itself. Should I consider another endocrinologist or try a dermatologist or maxillofacial surgeon?
Maxillofacial surgeon? What will this speciality know? You need an in depth consultation from a hair loss specialist (dermatologist/plastic surgeon)

Thanks for your insight, I have been on testosterone cyp for about 7 years. I am 37 and have had a great experience. I take 180mg and divide it up by taking 4 injection in a two week span. So each injection is .45 of a diabetic needle. I take .5 Arimadex only on the day following the injection. My levels are always in check and have nevermind had any ED with treatment. my hairline has been going so thin lately so I decided to do a topical finasterode with minoxadil mix. Is this a bad idea? Thank you
Thanks for sharing your experience. Experiences like your are more valuable than the lectures I give! By taking topical you will be wasting time. Oral Fin is the only way forward.

Yes if you take Finasteride it should be fine. You will know.

Thank you for watching. I do not prescribe TRT since this is not my field. I would advise you to see an andrologist.
Upping the injection frequency lowers the peaks and raises the troughs (you are more consistent). Usually, guys have less E2 issues with higher injection frequency. With Test C or E, I think ED is overkill for most guys. EOD should provide almost all the higher injection frequency benefits, without all the shots.
I am currently doing EOD, with FIN and Adex (I am blasting, otherwise no Adex). For the blast, I take 250 mg test EOD, .5 mg Fin ED, and .25 mg Adex EOD. I'll get blood work in a few weeks to see if I am about right in my guesses. I haven't used FIN before now, and I haven't used Adex in a long time (Almost crashed when on TRT dosages with only a little bit). When I go to cruise (50 mg of Test E EOD), I'll drop the adex, and I'll use 0.5 mg of Fin EOD with my Test shots. I have some experience and previous blood work to go off of. Lots of variables, but I have some of it solved with that previous experience and blood work, but will need some Test, DHT and E2 measurements to confirm my informed guesses.

Yes you can. Sice in mot cases the side effects are attributable to hypogonadism and not to Finasteride per se- Finasteride causes these in less than 0.5% cases with normal testosterone levels.

Dear Doctor, I’ve gotten a hair transplant 3 weeks ago and I am on 125mg of testosterone enanthate once a week while taking 0.5mg of Finastride.. is that sufficient enough to prevent any negative effects towards my hair transplant outcome?
YOu can ask your doctor if he will suggest 1 mg a day for 6 months. he will know your situation better and also the state of miniaturisation you have.

Yes we can wait and watch and see how much dosage will be sufficient. But usually it is a losing battle when you have progressive baldness and you crave for supplements.
Dr. - Can you explain why estrogen blockers - such as arimidex - may cause hair loss?
Because in women, oestrogen and progesterone help to keep hair in the anagen phase for longer.
Hi Dr. very informative video. I’m in a quandary. I’m 51. Been on TRT for 10 years through endocrinologist. I started because my testosterone levels were 174. I’m now very much noticing quite a bit of thinning on top of my head only. Not the crown.
It seems your saying I can take Finastride and continue TRT? So I won’t experience ED issues? The TRT nullifies this? I’ve been trying to get answers and don’t know path to take.

Thanks for being on my Channel. If you have ED issues, they will remain. Since ED is of many types and not always due to low testosterone. If not, you can go ahead and start Finasteride. Most people have no issues with both.

I had no idea armidex was toxic. I have been prescribed this for the last 7 years with my trt. If I stop armidex can any damage be reversed?
What damage do you think you have? Your treating doctor will be better able to titrate the dose.

So if I take 1mg of testosterone a week and 1mg of Finasteride every two days. I will see the benefits on testosterone and the finasteride will stop my hair thinning further. Sounds like a perfect combo then?
You will need to observe this yourself. Increase the dosage if this dose does not help in consultation with your doctor
Hi Doctor, thanks for sharing all the valuable info. Regarding boron supplementation, my free testosterone is in the middle range and I take 1mg finasteride per day. Recently, I decided to supplement with 6mg boron per day as I feel that my erections got slightly weaker. After 2 weeks, my free test is slightly above normal range. Is it possible to get increase in DHT levels due to increased Free Test and hairloss begins again? I am 31 years old, very lean(weightlifting), E2-mid range, SHBG-low midrange, TotalT 550ng/dl, FreeT 36pg/ml. I am on finasteride for 1.5years. Weightlifting 10+years.

Hair loss is not die to quantity of DHT circulating. It may be a factor but not the major factor. It is susceptibility to DHT that counts

If your baldness is not progresive and you are above 35, no need to be on Finasteride. 5% Minoxidil should be enough.

It varies form person to person. You will need to try out 0.5 mg on alternate days and then scale up if hair loss continues, to maximum 1 mg a day under supervision and direction of your doctor locally.
What do you think about oral minoxidil?
Yes good drug but side effects are a good amount. It has to be taken on prescription afer medical examination of cardiovascular status.
Does Biotin supplement or Hair growth multivitamin supplements necessary?
Marginal utility in tose with male pattern baldness. But whatever the effect, we need to recruit all medications that have been scientifically prove to be beneficial for hair. After all Vitamin B7 or Vitamin H as Biotin is called, does not have adverse effects. But it is never an alternative to a DHT blocker.
Even with Trt my shbg is 99 , free is 35.8 which is very low and i`m on propecia . what am I doing wrong how to raise my free testosterone? i was prescribed trt by my doc.

Yes testosterone in excess will be converted to estrogen. However this doe not necessarily mean erectile dysfunction. Go on it for 3 months on trial basis.

You need to go on a month long trial with Finasteride 1 mg on alternate days and see if all is well. Get in touch after that. What is the total testosterone level?

Usually not. But after 3 months on, you can do some blood work and consult your doctor.
Hello Doctor, this is a great video with lot of good information. I have been on TRT for more than 2 yrs now and I have seen very few videos like this. Thank you. I have 2 questions if you can answer pls.
1. I am on TRT (140 mg/week), my energy has improved. However without HCG my libido is almost dead. With HCG, I feel better and my libido improves. Can you explain why is this so? The problem with HCG though it increases Estrogen I guess and hence some side effects like bloating, gas etc, for me (particularly on the days when I inject HCG)
2. My Estrogen and DHT are both at higher level. Side effects are bloating, gas, (no nipple issues luckily) and hair loss. I have tried AI before but I felt so bad with AI that I stopped it. For my hair loss I am being suggested by take Finasteride but again I am scared about its side effects like it will shoot up my estrogen further and Libido issues. To your point I don't want to take either AIs or Finasteride and keep my TRT protocol simple and manageable in long run. With this in mind, do you have any advice (any supplements, natural herbs etc.) which can help reducing my estrogen and DHT little bit? Your advice would be greatly appreciated. thank you again.

I have no experience with HCG and this question can be asked from your treating endocrinologist/andrologist.
What is your age? Is the baldness stable? Do you do rigorous gym exercises.
@@hairtransplantinindia I am 40+, yes I go to gym regularly. My hair loss has started for last 1 year. I am using a hair cream which has 10% minoxidil. But I guess that is not enough. I am afraid to take Finasteride for Libido (as I already have issues with high estrogen).
@@hairtransplantinindia HCG will keep the testicles from atrophying if one is on TRT. It can also in some cases restore fertility on TRT. I believe it can boost testosterone additionally on TRT. It is a luteinizing hormone analog.
Have you tried a very small amount of anastrozole, like 0.5 mg every 10 days? That may be enough to keep your estradiol in the 30 to 40 range but not produce any side effects.
im 58 years old, full head of hair so I must not have the hair loss gene. my T levels are 550, my doctors prescribed TRT. 120 mg a week split in two a week. all was great but the hair thinning especially at the crown. got off trt. would you recommend finasteride?
The TRT has unmasked your androgenetic alopecia it seems. Whey protein may have helped in this process as well! I would suggest you get evaluated and get a prescription for Oral Finasteride.

If you quit finasteride after years of taking it does the hair loss come back to what it was before taking fin or does the hair loss become as bad as it would’ve progressed if you haven’t taken fin at all?
It would be the latter. While taking Finasteride the salubrious effect is not seen. You are unmindful of the progression of baldness. The hair are being maintained due to the DHT blocker. The day you stop it, the baldness you would have had at this time if you had not taken Finasteride, would get unmasked. It will be usually far more than when you started taking Finasteride.

PRP is fine for maintaining good hair quality but deos not help in androgenetic alopecia much since it is not a DHT blocker. Generic Finasteride is a vast term encompassing companies that are reputd and those that are shady. Whichever you take, note the effect and then if you benefit, do not change brands.

I am on generic finasteride here in the UK bought from a legitimate and reputable pharmacy via prescription from the Dr. I started with brand name propecia but it was getting very expensive with Minoxidil too so Dr gave me a prescription for generic finasteride. I also have a prescription for dutasteride so will add one dutasteride once a week on day 7 like you suggested and finasteride for the 6 days. Would PRP be useful or not in stimulating the existing miniaturized hairs whilst taking finasteride and Dutasteride or is adding the PRP pointless as its very very expensive? Many thanks.
@@acquiesce100 Yes that seems like a good way forward if it is a reliable pharmacy. PRP does not help mostly in people with aggressive baldness. But you can take 2 treatments and see for yourself. It is likely that what you interpret as a benefit from PRP is due to the DHT blockers you ill be taking.

There is no 'complete' stopping hair loss since we cannot block all DHT. Even if we do, nature has its own mechanisms of upregulation. But after one year of use you can expect the best results with Finasteride.

Dear Doctor, so in other words - I could do a light TRT to help me stay in shape and live my life to the fullest and take small dose of fin to keep my hair and protect my prostate? Till 6 months after hair transplant, Finasteride is mandatory. Long term Finasteride will depend upon several factors. Not the same for everyone. Some don't need it at all after 6 months.

Many reasons- if grafts planted have less double and triple hair grafts, if density i low or if you have not taken Finasteride after hair transplant. None of these factors can be corrected at this stage but in consultation with your doctor you can take Finasteride and this will help somwwhat.

So can you use topical fin and injectable test and not lose extra hair if taking like 100mg of test a week? I’m taking 1 mg of topical fin with 5% minoxidil a day and it’s working I’m 23 and my test level was 300. But I want to start TRT or am at least considering it.
Topical Finasteride is still to be effective. Research is going on. Till then those who use it use it for its placebo effect.

@@adonisb39 At 23 the baldness is usually aggressive. It might not be in your case, I am not discounting that. A test period while on Inj. Testosterone will decide this for you. Everyone is different. We will need to wait and watch and be vigilant.

@@harsharora6113 If you are using Fin on it, you will probably end up with a lower DHT level than you would have had naturally without Fin. If you get into blasting test for performance, then yes, it will most likely accelerate even using Fin. It depends though. Some providers go a bit crazy and have people on "performance" TRT. If you end up with 3X the Test level you had before, your DHT will probably be about the same on Fin as it was naturally without it.

The sort of infertility you could face is erectile dysfunction and not necessarily decreased number of sperms in semen. There are many artificial ways of conceiving for eg IVF. I would advise you to visit a fertility clinic or an endocrinologist for the same.

@@vanshadkitchloo3094 I do not believe this doctor is knowledgeable about fertility issues with TRT. hCG can restore or maintain fertility on TRT, from what I read, although experimentation with the dosage maybe you needed--it's usually injected every other day or two times a week, depending. Dr. Bhatti admits he doesn't know about hCG. Since the FDA pretty much banned compounded versions, the only available supply now may be the brand name Pregynl, and It's a bit expensive, but one vial can last up to two months in the refrigerator after mixing.

It is not about the amount of DHT level. It has no relevance since it is all about sensitivity of the receptors to DHT. Even a minute amount of DHT can cause hair fall if you have a proclivity borne out of genetic susceptibility. People who have high levels of DHT but do not carry the baldness gene will never bald!

The video is about hair loss when you're on TRT and how to deal with it. Patients who are hypogonadic are not initiated on Finasteride since they have a higher chance of ED, etc
i got 1500 futs in 2017. im nervous about going on trt, ive been suscbribed 50mg every 3 days ,been also giving dutasteride , im 38 years old... will i shed more is it worth the hassle?
TRT does cause hair loss in those who are susceptible- those who have the baldness gene. Not otherwise. Those who need TRT have low testosterone levels and are taking TRT for better libido amongst a host of other reasons. I would not start my patient on Dutasteride though. Finasteride would be sufficient for him.
