Now by watching this video I identified it as Malabsorptive (I have all points and iron deficiency as well). Can you give me any recommendations how to make my pancreas work better? Food/supplements? I don’t want to take pancreatic drugs but that’s what doctors would recommend

Told to take Imodium (which doesn’t work any more). Went to Gastro Doctor. All he recommended was colonoscopy. Completely clear. Parting words, it’s something you’re eating. See you in 10 years. Went to nutritionist. Did food diary. Food sensitivity blood test. He put me on pancreatic enzymes, colostrum and probiotics. Seemed to get worse so I discontinued all but enzymes. Went to urgent care. X-rays and fecal analysis all clear. I’m beside myself!!! I’m a senior athlete with world records in power lifting. I work at being healthy! No prescriptions and very careful diet. Have an appointment with a different gastrointestinal practice in 2 weeks. (Had to wait almost 2 months). Is there anything else I can do???
I'm sorry to hear about what you've been experiencing. Unfortunately, I can't give specific, individualized medical advice on here. Regarding the appropriate work-up for patients, the table @14:40 in the video is intended to be a summary of everything remotely common. Luckily, a normal colonoscopy helps to rule some things out (e.g. IBD) for most people. A food diary can be helpful, particularly if paired with a trial of removing certain foods from your diet and reintroducing them (e.g. 6 week food diary with 2 weeks eating dairy products, followed by 2 weeks strictly dairy-free, and then 2 weeks eating diary again - or whatever other food you think might be a trigger). A diagnostic fast is usually not necessary to make a diagnosis, but can also be useful in particularly challenging cases. As you indicated, getting a second opinion from another GI doc is a good idea too. Good luck!

And it did seem like I was getting close until a couple days ago. Complete setback. Not sure what caused it but I’ve got suspicions and will experiment. Ok....This sounds ridiculous, but it's worth a shot...I was pretty much unable to leave the house for six months or so... My issue cleared up soon after my brother started inviting me over every Sunday for steak dinner. Not a fan of meat, or beef - but it's my brother offering dinner, so I felt obliged to go. Anyway, it seems that the slow digestion of beef did 'something' beneficial for this condition in me. I'm still not a big meat eater - maybe a burger or two a month, but this problem is definitely gone now. I tried all types of things - even began to accept the false idea that my system was changed forever. No doctor or anybody suggested this - but circumstances lead me there. Today, this problem is a memory.
It's not as ridiculous as you think. Carnivore diets have helped many people solve many different issues, like "leaky gut," inflammation, autoimmune disorders, and more.
what about for those who cant tolerate fatty meat? i used to tolerate fatty meat quite well. but recently ive had diarrhea everytime i eat fatty stuff. im fine with lean meat for as long as the fat content isnt too high. stuff like pork belly, avocadoes or stuff cooked in coconut oil would give me diarrhea. i want to be able to eat fatty meat again..

Oh and make sure you are not consuming sweeteners that have laxative effects. They can cause serious problems for IBS sufferers. Look up online which sweeteners have laxative effects. I know Xylatol is one of them.
You only touched on the connection between diarrhea and fecal incontinence. I think you said that diarrhea can unmask or worsen a pre-existing problem with incontinence. But it seems to me that fecal incontinence can cause diarrhea since fecal incontinence can be associated with rapid transit time, which does not allow enough time for water reabsorption in the colon, causing loose stools. Is there a condition of idiopathic rapid transit time that causes fecal incontinence, in which stools are loose, which looks like diarrhea? (Edit: I do see that you mentioned hypermotility.)
What a shitty topic, but an important one. I was wondering if my problem, which comes and goes, could be caused by any of the pills such as blood thinners I've been taking for my heart. But eating fresh fruits, especially bananas, is sure to send me to the toilet.
Unfortunately the only way I can think of is to look and see the consistency and you'll probably have to look for several days. If it is consistently normal firmer consistency then it's incontinence, if it's consistently loose then it's diarrhea.
Best dissertation of chronic diarrhea. I think my problem at age 83 is lactose intolerance is worsening. Went form lactose free whole milk to skim milk for awhile. Seemed to help but now not so much. I use the pills then you guessed it; the opposite.
I am vegan whole family vegan,some are firness trainers and dance teachers one is body builder,we do not need dairy profucts at all they are bad for us.Milk from cows is For cows, milk from goats is for goats dog milk is for dogs etc..

Wait, why is Microscopic Colitis under non-inflammatory? I mean, it’s an itis which means that there’s inflammation, so wouldn’t it make more sense to be under inflammatory?
I know it seems contradictory to include it as a "non-inflammatory" cause of diarrhea when there's inflammation by definition. However, the disease clinically resembles a non-inflammatory diarrhea - which is what is most important for organizing a diagnostic framework (unless you're a pathologist of course!). For example, unlike a disease like ulcerative colitis, the diarrhea of microscopic colitis is watery and non-bloody; and patients do not have systemic signs of inflammation (e.g. elevated ESR, elevated WBC, fever). Also, colonoscopy in microscopic colitis is usually either normal or "near normal"; the diagnosis is only evidence on histology (i.e. with biopsy) An analogy is how osteoarthritis is not considered to be an "inflammatory arthritis" like rheumatoid arthritis or psoriatic arthritis.

I wish I could give an estimate on this, but unfortunately, I just can't. First, I need to find good ECG examples for each topic, which becomes incrementally harder as I go through the series and get to the more rare conditions. And then I need to remaster the ECGs into a format that looks good when reproduced in the video, which is time consuming and painful. (Some of these tracing are given to me by colleagues as faded black and white photocopies that look like garbage if I import them into the video as is.) Not sure what "NB" stands for, but colon cancer is not a common cause of chronic diarrhea. One situation in which it might occur is with something called overflow diarrhea or paradoxical diarrhea, in which an intraluminal obstruction in the colon is bad enough that hyperstimulation of the gut starts to push liquid stool around the obstruction. When this happens, it is usually from severe constipation and something called a "stool ball" (which is exactly what you are probably imagining), but it could also potentially happen from colon cancer. In addition, neuroendocrine tumors that cause chronic diarrhea via secretion of hormones could potentially occur in the colon, but I wouldn't label that occurrence as "colorectal cancer", which to most physicians specifically implies adenocarcinoma (a specific cancer subtype) rather than a neuroendocrine tumor.
Is it possible to have more than one? My doctors always want to throw antibiotics at me and that just makes everything worse. Fasting stops it, but not eating is not sustainable…
More than one cause of chronic diarrhea at the same time? Sure, but it would be unlikely, unless one of the causes is something particularly common like lactose intolerance. I can't give specific, individualized medical advice here, but as a general rule, chronic diarrhea in immunocompetent patients (i.e. people with normal immune systems) in the developed world is only rarely caused by a chronic infection. I honestly can't think of a situation in which I would personally treat a patient with chronic diarrhea (i.e. duration longer than 3 weeks) with antibiotics without confirming the specific pathogen first. Diarrhea is the most common side effect of antibiotics. Once again, this should not be interpreted as personalized medical advice for you, and I would discuss your concerns with your own physicians.

Ive had diarrhea for 28 days now. When it first started it had mucus but it was clear. I dont feel anything wrong or pain it just happens. I can hold it now its not an emergency everytime now. It sucks because i want it to go away by the time school starts again.

I never have gas I never have bloating. I never have abdominal pain. It appears like my colon just does not remove the water or liquid from the fecal matter. And it just all comes out. I rarely have a formed poop. I know what triggers me and I try to stay away from it.
That’s pretty funny. You refer to chronic diarrhea or loose spells as I have, in weeks. Try years over 20 years. I don’t understand why I can’t. Just have a regular formed bowel movement. Yes, some foods do trigger. But I’ve had accidents in public before and it is very embarrassing.
I'm sorry to hear about your situation. The reason for that particular time course is simply to categorize the causes of diarrhea into acute vs. chronic. The list of things which can cause diarrhea lasting >3-4 weeks and that which causes diarrhea for 20 years is more-or-less the same. Unfortunately, this doesn't always mean a diagnosis will be identified.

I am currently on Docusate tablets and glycerin suppositories does anyone know how long it takes to improve for hard stools as i am severly constipated doctors reckon its overflow diarrhea and i have had it for nearly 2 weeks non stop??
I can't give specific, individualized medical advice here. However, having said that, you should speak to your doctor about switching from docusate to something else. Many experts now believed, based on numerous studies, that docusate is entirely ineffective for soften stool and treating constipation. Our hospital doesn't even carry it anymore. That's a great question! It's because the distinction between inflammatory and non-inflammatory diarrhea is a clinical one. Blood and/or mucus in the stool +/- fever and/or leukocytosis = inflammatory diarrhea. With microscopic colitis, even though the underlying mechanism is inflammatory, it doesn't usually cause those inflammatory features.

Maybe you are intolerant to casein (milk protein) too. So you can't take anything with milk, because the lactase enzyme only works for lactose (milk sugar). Prefer coconut milk.
It took me years to realize I was also casing intolerant from milk products. So if you're having loose bowels because of lactose no doubt you're also having a problem with the casein in the milk and lactose pills don't help that. When I do any milk products I also get very mucousy in my lungs down my throat sinus infections. Just stop the milk

Sounds like yours might have been malabsorption. Maybe you had iron deficiency / anemia?

Skim milk has more lactose in it than whole milk. Make you are allergic to the protein in dairy?

Wait, why is Microscopic Colitis under non-inflammatory? I mean, it’s an itis which means that there’s inflammation, so wouldn’t it make more sense to be under inflammatory?
I know it seems contradictory to include it as a "non-inflammatory" cause of diarrhea when there's inflammation by definition. However, the disease clinically resembles a non-inflammatory diarrhea - which is what is most important for organizing a diagnostic framework (unless you're a pathologist of course!). For example, unlike a disease like ulcerative colitis, the diarrhea of microscopic colitis is watery and non-bloody; and patients do not have systemic signs of inflammation (e.g. elevated ESR, elevated WBC, fever). Also, colonoscopy in microscopic colitis is usually either normal or "near normal"; the diagnosis is only evidence on histology (i.e. with biopsy)
An analogy is how osteoarthritis is not considered to be an "inflammatory arthritis" like rheumatoid arthritis or psoriatic arthritis.

I wish I could give an estimate on this, but unfortunately, I just can't. First, I need to find good ECG examples for each topic, which becomes incrementally harder as I go through the series and get to the more rare conditions. And then I need to remaster the ECGs into a format that looks good when reproduced in the video, which is time consuming and painful. (Some of these tracing are given to me by colleagues as faded black and white photocopies that look like garbage if I import them into the video as is.) I'm hoping to have enough done for a couple more ECG videos this summer...we'll see.

You should mention Hypokalimea it will cause diarrhea too, as well as low magnesium.
Diarrhea causes hypokalemia, not the other way around.

Not sure what "NB" stands for, but colon cancer is not a common cause of chronic diarrhea. One situation in which it might occur is with something called overflow diarrhea or paradoxical diarrhea, in which an intraluminal obstruction in the colon is bad enough that hyperstimulation of the gut starts to push liquid stool around the obstruction. When this happens, it is usually from severe constipation and something called a "stool ball" (which is exactly what you are probably imagining), but it could also potentially happen from colon cancer.
In addition, neuroendocrine tumors that cause chronic diarrhea via secretion of hormones could potentially occur in the colon, but I wouldn't label that occurrence as "colorectal cancer", which to most physicians specifically implies adenocarcinoma (a specific cancer subtype) rather than a neuroendocrine tumor.

More than one cause of chronic diarrhea at the same time? Sure, but it would be unlikely, unless one of the causes is something particularly common like lactose intolerance. I can't give specific, individualized medical advice here, but as a general rule, chronic diarrhea in immunocompetent patients (i.e. people with normal immune systems) in the developed world is only rarely caused by a chronic infection. I honestly can't think of a situation in which I would personally treat a patient with chronic diarrhea (i.e. duration longer than 3 weeks) with antibiotics without confirming the specific pathogen first. Diarrhea is the most common side effect of antibiotics.
Once again, this should not be interpreted as personalized medical advice for you, and I would discuss your concerns with your own physicians.

However, I can say that there is a lot of overlap between carbs and gluten-containing foods, the later of which is responsible for celiac disease, which is a relatively common cause of chronic diarrhea.

I never have gas I never have bloating. I never have abdominal pain. It appears like my colon just does not remove the water or liquid from the fecal matter. And it just all comes out. I rarely have a formed poop. I know what triggers me and I try to stay away from it.
That’s pretty funny. You refer to chronic diarrhea or loose spells as I have, in weeks. Try years over 20 years. I don’t understand why I can’t. Just have a regular formed bowel movement. Yes, some foods do trigger. But I’ve had accidents in public before and it is very embarrassing.
I'm sorry to hear about your situation. The reason for that particular time course is simply to categorize the causes of diarrhea into acute vs. chronic. The list of things which can cause diarrhea lasting >3-4 weeks and that which causes diarrhea for 20 years is more-or-less the same. Unfortunately, this doesn't always mean a diagnosis will be identified.
Kindly suggest treatment.

I can't give specific, individualized medical advice here. However, having said that, you should speak to your doctor about switching from docusate to something else. Many experts now believed, based on numerous studies, that docusate is entirely ineffective for soften stool and treating constipation. Our hospital doesn't even carry it anymore.

That's a great question! It's because the distinction between inflammatory and non-inflammatory diarrhea is a clinical one. Blood and/or mucus in the stool +/- fever and/or leukocytosis = inflammatory diarrhea.
With microscopic colitis, even though the underlying mechanism is inflammatory, it doesn't usually cause those inflammatory features.

I have diarrhea, mild to severe stomach pain and abdominal discomfort, ALMOST EVERY DAY
I’d say 2 out of 7 days this happens
I eat ONE ice cream sandwich with dairy
And then I have diarrhea as if I drank 2 full fucking cups of milk
My lactose intolerance was never this bad and tbh I’m not even sure if lactose intolerance is to blame anymore lmfao

Maybe you are intolerant to casein (milk protein) too. So you can't take anything with milk, because the lactase enzyme only works for lactose (milk sugar). Prefer coconut milk.
It took me years to realize I was also casing intolerant from milk products. So if you're having loose bowels because of lactose no doubt you're also having a problem with the casein in the milk and lactose pills don't help that.
When I do any milk products I also get very mucousy in my lungs down my throat sinus infections. Just stop the milk
