Safin spared Madeleine's life so that she could live and suffer while not having parents or a family anymore just like how Safin suffered with the death of his family.

You know if you notice in Mr. Robot, Rami's other character Elliot Alderson went through a much similar situation like Madeleine did where he had TWO psychotic men obsessed with him and fall for him for wrong reasons.

Lyutsifer Safin is a victim, who's mind is twisted by Ernst Blofeld himself, he wants revenge against them and that is an encouraging thought. Sometimes we see people as treacherous dangerous people capable of lies, false love, dark secrets, and the only way to deal with them before increasing terror with their own making is to annihilate them, as a messenger of death. No matter what he does, I sympathize his entire life in torment and suffering. So unfair. So horrid. I could only wish if there is a light within him that helped him understand there are good and honest people in the world who could help him to redeem himself, then the story of 007 and Lyutsifer would exist together to ensure further safety to other countless families against other criminal organizations than just SPECTRE.

He became the most uninteresting and boring Bond villain to this date and the worst final villian for Daniel Craig's bond to face.
He wanted to get revenge on Spectre and try to spread the virus throughout the whole world
Even when he was dying, he didn't show any emotions.

Tbh the whole movie was just a mock-up of the Wuhan lab leak theory, played over an End of the World style Bond finale.
