If your USB was formatted on a Windows device (using the NTFS file system, for example), your Mac may not be able to read the files. Macs can only read, but not write, NTFS partitions by default, and your iPad may not support NTFS at all.

It seems that you flash drive is under write protected. To remove the protection, you can first find out if your USB drive has a switch on its side. Locate and turn ON to OFF on your flash drive and then try again to see if it can work well. Try in that way definitely will work . Don't try with big object it will damage port try small pen or pencil or something thin enough to make contact this way both device won't damage
My san disk extreme has suddenly stopped working on my mac book. It's been working fine but all of a sudden when I unlock the drive the drive doesn't appear. However the unlocker tells me the card is now unlocked. It's vanished. So there's no chance of me retrieving anything because I can't even find it.

When I go to finder, utilities, system info and select USB on the left hand side there’s no USB Drive showing. Just 2 USB 3.1 Bus options.

What if it shows up as “composite device” under USB on system information ?
It's most likely a hardware issue if it doesn't show up in Disk Utility. You may check if it is damaged physically.
I also have the 'composite device' scenario, but I know the device works because it's fine on a windows pc.
