      The author claim that DHT utility has been undervalued a lot, and therefore the danger of finasteride and dutasteride vastly underestimate. Do you have any angle on that ?

      I think I'll hold off on giving my official opinion on PP405, pending a full video on the subject. As far as HMI-115, I don't think we can conclude anything from one leaked result. We don't know if it is representative or an outlier yet.

      in italy “Clemente” 1L olive oil, which is one of the highest quality commercial olive oil is 14€ or something like $18, and i think its quality is better than the Bryan’s one at 1/3 of the price

    Hi Kevin, does minoxidil gains permanent as long as i use it or after enough time i will lose all my gains and return to baseline? if so what is the point of using minoxidil (while using fin )for lifetime if it only works for 4-5 years? not to mention if you stop your hair become even worse. im nw 2 with long hair. hairlineis not that bad but my main problem is density.

      Yes, minoxidil works indefinitely. It only loses its efficacy if you use it by itself since it doesn't block dht. If you use it with finasteride it works forever.

    I’ve watched all your DUT videos multiple times. They provide great insights into Fin versus DUT, highlighting how DUT may have a similar side effect profile to Fin, if not better. I’m interested in hearing your thoughts on the 75-patient study that examined the reduction in sperm count. As a 22-year-old, this concerns me about potentially having kids in the future.

      I'm not sure if you mean this study, academic.oup.com/jcem/article/92/5/1659/2598215 . It showed mild reductions in sperm counts that reversed on stopping therapy. In the majority of men these changes would not affect fertility, but in men with borderline sperm counts to begin with it could. Sperm counts do go back up when stopping treatment in all the studies I have seen, but most of the time it is nothing to worry about.

      @@haircafekevin In a study comparing dutasteride vs placebo, 27 healthy men took dutasteride, 0.5 mg, daily for 52 weeks and completed 24 weeks of follow-up.47 At 52 weeks, compared with placebo, dutasteride treatment resulted in a reduction in sperm count, semen volume, and sperm motility (23%, 26%, and 18%, respectively). There were 2 participants in the dutasteride group who had decreases in sperm count that were greater than 90% from baseline at 52 weeks.47 At 24-week follow-up, all semen parameters were within normal range, except for sperm count, which remained decreased at 23% lower than baseline.

      @@prav077 If you read the rest of that section of the article, there is a second study where the decrease was not statistically significant at long-term follow-up. Like the article says, the decreases in sperm counts probably are not clinically important in most men and don't affect fertility, but there may some men who have a greater decrease that could cause fertility problems. If this is a concern, the data on 1 mg per day of finasteride might be more reassuring to you, so maybe you should just use finasteride.

    (theory) Im a big fan of him but I have doubts that Brian Johnson is doing much.
    I fear that it`s just his team of doctors simply doing whatever is proven to work (or what they think works)
    I`d say other researchers or even the average Redditor on r/supplements is more knowledgable than his team

    What's the most effective and safest way to switch from Fin 1mg daily to Dut 2.5mg daily. I've been on Fin 1mg daily for almost 3 months and want to switch to Dut 2.5mg daily.
    I am thinking of taking Fin 1mg 5x a week and Dut 2.5mg 2x a week then gradually dropping Fin over the course of a few months (less Fin 1mg, more frequent Dut 2.5mg) and eventually taking only Dut 2.5mg daily.
    Would this work or not, or is there a better way to do it? Have you already covered this in a video? If so, send me the video please.

      I'd maybe just try it once or twice per week with fin on the other days to see how you respond, and then switch to 2.5 mg daily if you're comfortable with it.

    Hey Chooms, hey Kevin !! 👋
    I started adding 2.5mg dutasteride on every sunday on top of the 0.5mg dutasteride I take as usual everyday.
    My goal is to regrow more hair (basically, despite better regrowth on dutasteride than on finasteride, I still have plenty of miniaturized hair that just won't get thicker....) and my goal is to also regrow those thousands of still miniaturized hair.
    I've also been on topical minoxidil for years so you guys know.
    I will update on HairCafe's channel in 6 months and in a year to let you guys know how it goes (whether is makes a difference or not).👍
    Might even upgrade to 2.5mg dutasteride twice a week on top of dutasteride 0.5mg everyday later if I have to (hair greed I know) .
    Wanna do that until GT20029 is out on the market to maximize my regrowth

      Dut is almost a cure. I’m thinking of hopping onto the master race myself, just scared about It’s effects on sperm counts, I’m 22 almost and that’s also a concern for me.

    Hey Kevin
    So I had a video call with a well known hair surgeon here in europe this week. After telling him my hairloss history, he suggested the following medical therapy:
    - Switching from dutasteride (after 9 years on it) to oral finasteride 1 mg, 1 tablet per day for 1 year.
    Reason: My body has most likely built up a tolerance after 9 years of dutasteride. The surgeon has also observed in other patients that the effect of finasteride/dutasteride diminishes after 5-7 years. Tolerance should be resolved with the interruption. He, calls this window therapy.
    Have you already heard of fin/dut tolerance and the effetcs after taking a break from it?

      That's based on very weak observational data from Asia. I've addressed it in some of my Rob English videos. There's no solid evidence at all that this is true.

    Mr. Kevin Mann can we get some lifting videos like the old times? I lost a little bit motivation for lifting due to an injury. And also kevin, how do you manage injuries while lifting so heavy. I'm 22 and whenever i lift even a 1/3rd of your number i just tear something. I feel like strength is very genetic and i haven't been able to bench 205 even after 3 years training.     Any idea why there has not been any progress in making synthetic hair? Like, how can we have implanted fake teeth but not implanted fake hair that does not cause infection and other negative side effects? To me it feels like it has to be a more tractable problem than e.g. hair cloning which is cloning an organ and therefore basically final boss of medicine.

    really love your content since you were the one convincing me to fight the curse rather than just letting my hair go away. I was wondering if you would do an update on your hair transplantation video.
    In your video you are on the side of Fue, however there are several points where I would be interested in your opinion.
    -FUT has better growth yields than FUE
    -FUT preserves the donor area
    There are some papers discussing this. Also the videos of Dr. Bloxham recommend starting with FUT.
    What is your stance on this topic? Clearly you were quite succesful doing FUE.

    Hi Kevin, hope you are feeling better.
    A quick question. Ive seen a commercially available exosome product here in the UK called E50.
    When I’ve looked deeper into it, the exosomes are captured/cultured from salmon testes apparently.
    I just wonder if there is any data on if all exosomes are created equal, not only in terms of parent progenitor cell type, but also species type.
    Can we expect salmon testis derived exosomes to be as effective in hair regrowth as human dermal papilla cells derived exosomes?! My guess is probably not, but that’s complete speculation.
    Any thoughts? In truth I am considering trying it out as financially I can take the hit for afew months of experimentation.

    Well chooms, after a year and a half on finasteride I am still experiencing hairloss (much slower than it should have been, I'm sure) but still progression wasn't halted. In turn I need advice - anybody know where I can order dut online outside the US? And also, a good place for hair transplants (not in Turkey) with decent prices?

      Europe itself like spain has some good ones also fairly cheap compared to the us.
      You can get dut from some hairloss surgeons even in the us.

    Me the 20 yo with dupa, retrograde and regular pattern hl (probably started at 16 yo) has experienced some hairgrowth after 6 months finasteride along my 1.5 nw hairline, so much that it regrew to 1.25 nw, but my dupa continued to worsen after 3 months into fin again, as did my retrograde. Isnt this a sign, that fin is not enough for my dupa and retrograde?

      Could just be a seasonal shed. I would give it a few more months to stabilize and if that fails maybe consider 2.5 MG of dutasteride for double finasteride's scalp DHT suppression.

      Nah, it is completely benign, though you may need to shave the top of your head for a proper fit.

    Hey Kevin, hope everything is nova. I was thinking about one thing, if i had a great shedding when I started fin, and i am having a considerable regrowth rn with fin/dut, how likely is it that i will continue to have gains over the next few years? Taking into account that just finasteride was enough to give me some gains, and now I'm on dut 2.5mg daily. Im 21 btw and my hairloss started at early 19.

      Your chances of regrowth are very good. In the clinical research, even finasteride alone caused some regrowth in 60 percent of users (the remainder got maintenance), and you're on a stack that is significantly stronger than that.
      Peak hair regrowth will likely happen by year 2 but there is research on finasteride showing it continuing to give regrowth even after 10 years so I think you will very likely see continued benefits for many years and you will be able to stop any further hair loss indefinitely.

    When I got my Hair Transplant done my clinic sent in some of my fair follicles to a lab to harvest and freeze the Sten cells for future use. I wonder if they can separate the exosomes as well

    If someone ask you whats the chance of sides on fin, do you say 2% or do you say relative risk is close to 1 so the chances are verry low.

      I usually quote the absolute risk, which is risk on drug minus risk on placebo. So that's about 2%, though obviously in some studies it is more, and in some it is less. Overall I consider the risk to be low.

    Hey Kevin! I recently got Finasteride perscribed and I'm thinking of microdosing it since my hairloss isn't aggressive and im still young and about a norwood 1.5 or so. I wonder, if I start off with 0.25mg a day lets say, if it works fine, should i keep taking it forever, until lets say a few years later if i start losing gains and experiment with upping it to 0.5mg a day and go on from there? And lets say if i dont lose any ground in a few years, can i take 0.25mg forever? This is just hypothetically speaking, i hope you understand. Thanks man!

      Yeah, the dose isn't that much different and it is still effective. If you are more comfortable with 0.25, then go ahead.

    Sup, Kevin! Statred taking fin 3 days ago and have no sides but today I noticed that the drug causes allergies to me. Something like little rash and a little bit hard breathing. After 1 hour everything went away. I need you help bro, this pill was my last hope to save hair. Should i try to change brand of fin or result will be the same? What can you advise in such a situation?

      It's possible it isn't related to finasteride, I would talk to a doctor to be sure.
      If need be then use dutasteride, it is related but it is a different drug so there is a good chance you're not allergic to it but make sure you talk to a doctor to know for sure it's the finasteride causing it.

    If twice weekly .5mg dutasteride hasn’t stopped my loss progression in a year would u recommend upping the frequency to daily or upping the dosage to 2.5 mg twice a week? Or could topical dutasteride perhaps work better for me? Or do I give up lol

      I would use 2.5 MG daily or every other day. Topical dutasteride is not a proven product so I wouldn't rely on that.
      Also, use topical minoxidil in case you aren't already.

    Hey Kevin! I am from India and currently 17 years old... I have a lot of body hair (so much leg hair, especially)... People online say this is a sign of high level of DHT in my body....Also, I had so much hair reduction in past two months.. I was NW1 and I am moving into NW2.... I am really worried but I cannot use finasteride due to my age....

    Hey Kevin, Witcher trainee here. I've been on fin, min and a mix of some spray (hairgen spray with pharmacort and helsinki activator mixed in) that my dermatologist suggested for about 2 months now. I only had some light recession in the temples but i started seeing new small but thick hair from the first month, had a shed during first month too, now at a bit over 2 months shedding has stopped and my hair seems way thicker all around and the temples seem almost completely restored, some of the new hair is a bit shorter but they seem like regular hair and not vellus. My question is should i expect even more regrowth in the first year or should i temper my expectations?
    Regardless i'm already quite happy with the result and started early thanks to you before things got any worse. Most doctors in my country that are older in age say fin is dangerous and they don't suggest it, thankfully after watching your videos and i found a younger dermatologist that was better informed. Thanks for spreading the truth and good luck with your crunch.

      The fact that you're seeing any results this early on is extremely optimistic. Finasteride regrowth results tend to peak by 2 years and then after that it is maintenance, so I think you definitely are going to see more great results.

      @@haircafekevin Bit of an update, i recently noticed along with the new hair growing on my temples theres also hair growing way beyond where my juvenile hairline used to be. almost on my forehead. They are still small but dont look vellus just shorter (like the temple hair growing).
      I was careful not to apply minoxidil in that area not sure if that means minoxidil affects an overall area around the spot applied or if that hair will fall off on its own later? If not is there anything i can do like apply less quantity etc?
      A bit of a weird problem to have, since its usually too little hair, but it does seem concerning and i could not find any info online except hypertrichosis from minoxidil on women, so i thought id ask. Thx in advance.

    Hello! In the video responding to Leo and longevity, you mentioned using Cialis to counter the physical effects of finasteride. However, Cialis has a rare incidence of ototoxicity, which, although rare, can cause a 15 dB decrease in hearing that is irreversible. So, I don't think it's prudent to recommend it. Additionally, Leo mentioned that you can't replace androgen signaling with Cialis, but he didn't explain why if you knwo i would love to knoww. I would love to hear your opinion as a finasteride user and former Cialis user. Cheers!

    Hey kevin i've started using minoxidil for the first time today. And i've melted the foam into a dropper container that still had some serioxyl in it. Is this a dumb idea? I didn't want to waste the serioxyl and i had no other dropper.

    I recently got a test done showing that my testosterone is good at 782 and free test is higher than reference range at 170. Since my free test level is higher than normal does that mean there is more dht or estrogen being converted? Going to ask doctor during followup but wanted your opinion on this result.     Hey Kevin I’m so lost right now - been on dutasteride 0.25mg and minoxidil 2.5mg for the past 66 days (2 months 5 days) and I’ve literally gotten so much worse - I initially had a small bald spot at the back and little bit of diffuse thinning that was very early stage staring but now Ive shedded so much hair and the sides are thinner and shedded as well - I used only topical minoxidil for the past year before that and my hair didn’t really improve or do anything so I switch to this and it’s just been a catastrophe??? I’m 17 and just don’t know what to do - some say it’s a shed that happens but even then how long does it last? And when would regrowth even happen

      It just sounds like a treatment shed. The reason it is aggressive is because dut is very powerful. You need to be patient though. It can take up to a year to see improvements beyond baseline.

    Kevin Mann lets say one doesn't respond well to 5% topical minoxidil what would be the next route. Should he go to 15%minoxidil directly or should he try 10% and then 15% after. Also is 15% minoxidil safe? Let's say i don't want to use oral minox for heart side effects, but isn't using 15% topical minox also dangerous?

      Yeah, 15 percent minoxidil works well in people who are poor responders to 5 percent. I wouldn't worry about the cardiovascular side effects because even though the concentration is high, the rate of absorption is extremely low.

    Hey, Kevin why do some studies say that dutasteride takes 1 to 3 or 4 months to build up in serum DHT to 90% serum DHT suppression while Avodart claimed in week one of daily 0.5 dutasteride it went up to 80 or 85% serum DHT suppression? also doesn't finasteride at 1mg daily suppress DHT almost immediately in a week or two, Visual results show up months later but the important thing is how quickly do both meds already start hacking away at DHT?

      The 5AR enzyme blocking effect happens very quickly with both drugs, within a few doses. Dutasteride takes a long time to reach steady-state blood levels, literally months, but you get the 5AR blocking effect much more quickly, like within a week.

      No, it's more likely just a seasonal shed. Stress doesn't cause hair loss like that, check out my video on it because there is a lot of misinformation about stress and hair loss.

      The shed means it is working. The weaker telogen hairs are falling out to make way for stronger anagen hairs. There is no way around it, for hair to regrow a shed has to happen.
      You may want to look into concealers like Toppik if you want to make your look more dense.

      Finasteride will at least maintain what you have indefinitely. If you want to maximize your chances of regrowth though then you should also use minoxidil.

    Question for you haircafe. Does it matter which time of the day you take finasteride. If testosterone is the highest in the morning would it be more beneficial to take it right before bed or early in the morning?

      No, it doesn't. Just so long as you are using finasteride on a regular basis then the 5ar enzyme will be under control.
      The acute fluctuations to androgens throughout the daytime aren't dramatic enough to affect the drug's outcomes.

      Are you minoxing it? Topical minoxidil would be the next step.
      Also, maybe use 2.5 MG of dutasteride on your dut day instead of 0.5 MG. 0.5 MG only suppresses 10 percent more scalp DHT than finasteride but 2.5 MG suppresses 40 percent more.

      Yeah, I'd go ahead and add minoxidil. You just need to use it once per day and it will increase your chances of regrowth. It's worth it in my opinion.

    Hi Kevin, thanks for all your posts. I’ve been taking Fin for 20 years almost, no issues. But I’m 40 and I swear my hair thinned earlier this year. Could this be the Fin stopping working?       Blood work won't tell you anything as it is scalp DHT that is important, not serum DHT. Finasteride works indefinitely so I think it is more likely a seasonal shed.
      If you really are convinced you are losing ground though then maybe add dutasteride. I wouldn't recommend dropping a drug that has worked so well for you for so long, so maybe just add duasteride once or twice per week (0.5 or 2.5 MG on the day you use it) and use finasteride on the other days.

      You can use stemoxydine although it isn't as effective as minoxidil. It probably is just dried residue.
      Buy a shower comb, you can use it to manually remove the flakes.

    hey kevin have you looked at skull foramen calcification as a cause of hair loss. here is a clip from an article of Frederik Hoezel. who was a technician at medical school and he had to remove brain for neurology class.
    Baldness occurred in persons in whom calcification of the skull bones apparently had not only firmly knitted the cranial sutures but also closed or narrowed various small foramens through which blood vessels pass, most prominently in persons with a luxuriant crop of hair. These blood vessels are mainly veins which normally communicate with the diploic veins in the spongy tissue of the skull bones but which are evidently pinched off by calcification of the foramens.

      I don't buy it. Impingement of venous drainage is even more unlikely than impingement of arteries. If there was significant impingement the skin in the scalp would develop edema, which never happens. I don't think there is any research to support his observations.

      No, once DHT suppression reaches a certain threshold it will stop 100 percent of hair loss. In the case of finasteride, it stops hair loss in over 90 percent of people.

    Hey man love your channel, learned more here than anywhere else. Question- I am getting gynocamastia from .5 my of fin oral daily and when I hop off 3-4 days later my hair thins and falls out.       If you never took fin your hair would definitely be worse off. Androgenic alopecia can progress rapidly.
      I'd get back on fin and try a smaller dose, like 0.2 mg.
      Worst case scenario, get gyno surgery. You can have the doctor remove the entire gland so it doesn't come back.

    Kevin, what would be the next action for someone who used dut with min and didnt see good gains in 6 months. Should you add dermarolling and tret with minoxidil or what? Or should you just wait more to see peak results?

      6 months is still too soon but the next step would be 2.5 MG of dut. Give it a year though to judge efficacy. These drugs can take up to two years to fully flex their muscles.

      @@omerul-farukarslaner8531 I don't recommend derma rolling, period. Tretinoin might be a good adjunct but no matter what you do, 6 months is simply too soon to judge a hair loss stack's efficacy.
      If you're really losing ground on your current stack (highly unlikely) then stronger 5 AR inhibition is the next best option.

      They don't stack, so same as dut daily.

      Overpriced and overrated. Just stick with oral finasteride and titrate down if you get sides.

      @@haircafekevin thanks kevin for replying. Oral still gives me sides. I tried titrating doses, frequencies etc. I have been a long term user. Currently 37 and am healthy with no excess body weight. Low dose fin lowers my libido. On blood work my free t was at the lower end. Total t is in 450’s.

      Topical still works, but I wouldn't use it unless exhausting my options with oral finasteride.
      If you respond better to topical though then it is fine.
