If you were still confused after watching this video like me, maybe this explanation can work. When I first saw the words volume of distribution, I was really focused on the volume part. But try to forget about that for a second. Focus more on the distribution aspect of it. Consider a company like Amazon. They have an incredible distribution ability, right?? I'm sure there's people that live in the middle of nowhere that have prime accounts and still get two-day shipping. Drugs with a high volume of distribution are like Amazon, they go to the deepest depths of the body. They have the ability to leave the blood and go into the interstitial spaces and into all kinds of cells. So think about that for a second. A high volume of distribution means when we give a drug it's going to go into the blood and then leave and go to many different places. Let's consider the opposite now. What is a low volume of distribution mean? It basically means if we give this drug to a patient, it's going to stay in their blood and not leave. For some drugs that's okay, but for others it's not. And we have to ask ourselves what we intend to do with the drug and where we want it to go.
Another part I was getting confused on. When we give a certain drug, It has to use the blood as a highway to get around. If we give a patient a drug and then quickly take a sample of their blood, We can see how much is just hanging out in the blood and not leaving. If that concentration is really high, It means it has a low volume of distribution, cuz it's not distributing. It's STAYING in the blood.
    I hope this helped.

The analogy seemed kind of unnecessary for this explanation…and I still don’t get the idea of high and Low vd
