My daughter's '07 Fusion, with the Duratec 2.3 (similar tensioner as in the video) has the same issue. We had already decided on replacing the tensioner pulley, but this helps confirm it. The new belt, too, was slipping off, though the old one didn't.
Thanks for this video, I have a Dodge Grand Caravan 2007 with a 3.3L engine and the serpentine belt was jumping up from time to time, it seems like something in these pulleys creates a drag force when starting the engine, so the engine shakes randomly and the belt jumps out. The power steering pump was replaced when I first saw this problem 4 years ago. However, this does not happen all the time. I do not think that it is a tensioner issue, as it is very stiff to rotate - good spring.
When the engineer starts, it creates the highest tension on the belt and tensioner (when fluids are cold, it is even higher) and that's when tensioner gives up.
My serpentine belt is coming off of my water pump Pulley !!, it sites a little off the water pump pulley and then comes off about 15 minutes after the car is running !!, I'm assuming that the water pump or the water pump pulley is bad !! . I have a Ford fusion 2.5 l .
Thanks for your question! Interesting... Considering 15 minutes time range, try if there is any play on water pump pulley. If there is no play replacing water pump pulley is probably your best bet. If there is a play you would need new water pump. Please let me know what you figured. Good luck!

Sounds like exact same issue. Note, you will need entire new tensioner, not just a pulley.

The thing is, my tensioner is new and I'm having this problem.
Had the same problem a few months ago. The tensioner body was defective. Ended up reusing old body with the new pulley
