Dopesick Breakdown: Opioid Crisis 101

  • Dr. Van (board certified pain specialist) takes you through the first episode of “Dopesick” and breaks down the hard truths about how the pharmaceutical industry tricked the healthcare industry into over-prescribing a drug (Oxycontin) that is just as addictive as heroin. Widespread substance use disorder and addiction are the result of the opioid crisis, but the healthcare system can also be part of the solution to these medically manageable issues. If doctors and their patients who use opioids chronically work together to educate themselves and trust each other, opioids can be prescribed safely and effectively.

    I’m so happy you mentioned that these days, many doctors will immediately say “NO” to prescribing opioids these days, even when clearly indicated. I know it’s a very difficult situation but it’s swung too far in the opposite way since it was swung the other way initially. I have no idea why the middle ground was never the norm but I guess that’s a larger societal and not only a medical issue.

      I agree, the pendulum metaphor has the healthcare system swinging between two different unsafe, unrealistic extremes. You're right, our society and culture has really complex issues with boundaries and comfort, but the bottom line in healthcare should always be safety and quality of life.     Just found this video after binge watching the new netflix series painkiller. Great show, i cant belive how close that story of the mechanic was to my own family.

    Wow. Having worked at a pain management clinic as medical assistant, I am SO GRATEFUL for your video. This tv series really reveals the ugly truths of big pharma and the opioid epidemic. While working at the pain clinic, I rarely ever had to bring my lunch to work because the pharmaceutical reps that came by provided catered lunch for the whole office on a regular basis...ugh now even reflecting on that feels a little gross! I am ever so grateful for the providers such as yourself who truly are interested in helping people treat their pain in a realistic, manageable way instead of falling victim to the opioid epidemic.

    And this is why I DO NOT like taking any type of opioids or narcotics for pain if I have a choice! I was prescribed oxy for a minor surgery years ago, I took about two doses and I trashed the rest. Not to mention I have several family members that over use several drugs. I'm going to have to watch this show.

      You should also check out the new series "Painkiller" on Netflix which tells the same story in a slightly different style. Personally I found it a little more entertaining and easier to watch, but it still handles the gritty realities of the opioid crisis with dignity.

    When I was watching dope sick? I literally felt like I was watching my life years ago In some of the close scenes it was unreal

    So what about those folks that are stuck taking Opioids. What are they to do to get off these meds. Can ANYBODY tell me that

      This is a crucial question that many doctors try to avoid because the answer is complicated and challenging. Optimizing a pain management strategy means diversifying and trying many different things to find what sticks. Pain doctors are responsible for helping people with this, they should be serving as guides for people who want to "get off these meds." People should work with their pain doctors to maximize their lifestyle changes, therapies, medications, and even procedures to help manage their symptoms and conditions in a way that aligns with their goals. Improving understanding of causes of pain and different treatment options helps people realize opioids are just a band-aid for pain instead of targeted therapy. This understanding helps motivate people to tap into strategies that are safer and more effective long term. If someone's goal is to not be "stuck taking opioids" then they likely can work with their pain doctor to either 1. gradually wean opioids while maximizing non-opioid strategies, 2. rotate opioids to minimize risk of tolerance and continue a gradual wean this way. There are specific medications like buprenorphine formulations (e.g. Suboxone) that are specifically indicated to help people wean full-agonist opioids safely. The goal is to find the lowest effective dose (of any medication) and maintain a safe pain regimen. These goals are best achieved when there's a collaborative team approach to pain management, which sadly can be hard to access for so many reasons. Thanks for your comment, and thanks for watching.

    Drs today only care about money and payback is coming when you get old and whatever you do never get hurt in a care accident and when you cry in agonizing pain maybe you'll get some ibuprofen!

      I hear your frustration. It's not fair that many doctors seem happy to get paid to do the bare minimum regardless of how their patients are doing. It's going to take a lot to shift the culture in healthcare and medicine. But now at least we're talking about these challenging issues instead of pretending they don't exist.       Thanks for your comment! It's a scary reality that, in our society, there are industries (both legitimate and shady) that will take advantage of our basic needs and profit off of our suffering. The best we can do is educate ourselves, demand accountability, and prioritize our safety.

      @@ThisAbilityClinicGrow Poppies, Ephedra, & Coca plants at home in our VICTORY GARDENS & we can then make all our own tinctures & medicines & drugs that we enjoy and use in the privacy of our homes. The 4th drive in humans is a desire to get high, which is 100% Natural & Normal.
      Prohibition & the criminalization of molecules is a big big problem. Prohibition never worked for Alcohol, it’s obviously never worked for Heroin, Methamphetamine, Cocaine or any other illicit drug. In fact now we have super potent drug combinations like xylazine & fentanyl being dumped onto the streets to seemingly test out upon the mentally ill & homeless street people! This is sickening….the USA is a awful experiment in mass torture techniques and biochemical/biological systems of murder…

      Easier said than done. The "just say no" campaign that Nancy Reagan started in the 80's has been widely agreed to be ineffective. Teaching kids to "just say no" doesn't actually help them practice the social, interpersonal skills to navigate situations that involve peer pressure and self-advocacy.     Tolerance and/or withdrawal are not, in and of themselves, indicators of addiction. This can happen with anything that is introduced to the body. Also, many drugs began as non addictive including tramadol and bupenorphine...HELLO! As soon as the drug begins to make money...just saying. One more thing, your 400% increase in OD deaths is not from compliant patients. I'm sorry, there's just too much to comment on here. I really hope you that as you grow you will better understand the complexity of this issue.

      Thanks for your comment. I agree that physical withdrawal is a sign of only physical dependence, not necessarily psychological dependence or substance use disorder. I learned this back in medical school, and I continue to see this regularly as a pain management physician. There are many misconceptions about the opioid crisis and the trauma and harm it has caused and continues to cause. In this video, I try to summarize the opioid crisis in terms that people can understand. You seem to be misinterpreting my breakdown as me having only a simple understanding of a very complex issue. I do my best to learn from constructive criticism, and overlook condescending or insulting tones, since those are rarely helpful. As I continue to grow, I continue to be reminded that no one should make assumptions about people they don't know. Thank you for sharing your perspective and insights.

      @@ThisAbilityClinic I have degenerative disc but I’m in my late 30s. A surgeon at Washington hospital in DC told me I’m definitely due surgery but I’m young enough that he doesn’t want to cut me open. Is this coming from your experience?
