Agree always seek advice from your doctor ,every doctor I’ve talked to has said humans cannot make B12 and that only ruminant animals can synthesise B12 in the gut but that humans cannot because we have different stomachs eg a cow has 4 stomachs which is how it can make B12 , which is why people eat meat . Doctors have also stated vegan and vegetarians must supplement or ensure they are getting their daily B12 requirement from foods fortified with B12, because Humans cannot make b12, hence the need for supplements ad fortified foods .my personal experience -B12 is not something you want to experience , it is a slow progression of loss of health and recovery time is not always a quick fix ,depending how much damage has been done to your nerves . Mostly it is reversible with treatment, just depends on how long you have waited to seek professional help . As always seek professional medical advice
278 can be regarded as low if you present with many of the symptoms. Ask you health care provider to test for Homocysteine & Methylmalonic acid. Do not supplement prior to testing.

Can you still have B12 deficiency without anemia or macrocytic anemia ?
Yes
Eat SALMON and food reach in B 12, avoid injections.
if you have pernicious anemia you have to get injections because your body can't absorb B12 through food like normal
