In the US alone, 70% of Americans have either deficient or suboptimal levels of vitamin D. This is a massive problem given vitamin D's impact on our health and its effects on our gene regulation and expression. Vitamin D also regulates calcium and supports the functioning of the immune system, making it one of the most important biological compounds in the body.
In today’s episode of a new series I’m calling Know Your Numbers, I dive deep into vitamin D. I discuss why vitamin D deficiency is a problem, how conventional medicine misses the mark in measuring vitamin D levels, how Functional Medicine treats vitamin D deficiency, and much more.
You can test your vitamin D levels with ‪@functionhealth‬, a company I co-founded. It has been a lifelong dream for me. We need more capability to test ourselves at home.
I hate going to the doctor. I sit in the waiting room for over an hour, then sit in a little cold room for another 25 minutes. The doctor comes in and sees me for under 10 minutes, says, “Hello, how are you?” and ignores my answer, tells me I seem to be doing fine for my age, 80, and writes me an order for a blood and urine test. (Why don’t they have me do those before the appointment, so we might have something to talk about?) My knees hurt. He gives me a prescription for 600mg Ibprophen 3 times a day. No checking about why they hurt. Then says I’ll get my test results in the mail, and I should call if I have any questions. How would I know what to ask. We are done in 9 minutes, and he has hurried out of the room. My test results come to me and what’s tested for is minimal. Vitamin D? No. Total cholesterol only. A recommendation for statins based on the one number.
RUclips docs need to know what the real world is like. They say go to another doctor. This is my third one. All alike.

Simply because vitamin d has a sterol like structure close to cholesterol, it is considered fat soluble, but, that doesn't mean that it accumulates in fat to toxic levels, that is basically a myth, fat storage is not well delineated, and it's release to provide 25OHD in times of no sun, is not well researched, and missing any mechanistic understanding, it is an assumption, a logical assumption which may or may not be true, and doesn't really seem to pan out.
There is vitamin d in all tissues, but the confusion is the term superfluously given to circulating calcidiol 25OHD as 'storage' because it has a relatively long half-life of about three weeks to two months or longer, does not make it a closet for a rainy day usage. There is a reason why calcidiol has a long half life, is because it helps with keeping ionized calcium in a tight range the most important physiofnx for life, so having a system of renal feedback mechanisms of different organs and keeping basal amount of 1,25OH2D3 calcitriol always ready in availability and use nano second to nano second, without it having to be in large amounts (as that would be toxic), the system for long term use, ensured stable ionized calcium (intestines, kidney, bone, parathyroids) we call endocrine fnx of hormonal release and signaling, a highly and tightly regulated system.
Contrasted to the greater use of vitamin d which is anything but long term, it is always being used, dependent both on diffusion as into immune cells, and muscle cells that use coupled protein uptake (muscles, kidney, etc) short term immediate use, metabolic conversions and immediate breakdown are paracrine/autocrine etc.. not reliant on circulating 1,25OH2D3.
This idea of getting toxic build up of vitamin d intake of normal physiologic amounts like 10,000iu perday, or 5000iu perday in normal weighted people of at least 100 pounds is a myth, there is a steady state of saturation, vitamin d levels pan out and do not ever increase, they reach an equilibrium of range.
Just like the idea that taking vitamin d simply increases calcium linearly so therefore taking vitamin d long term will inevitably create high calcium levels is a myth.
Toxicity of vitamin d is not because of accumulation in fat because of its fat solubility, or that it is eliminated through the fat pathways of cholesterol elimination via bile and into poo, (unlike water based vitamins which are excreted in urine), the toxicity is one of calcium levels that are increased by virtue of dynamics of superphysiogic amounts of vitamin d that override the basic normal physiology feed back mechanisms, not because of toxic build up because it's a fat soluble vitamin. It can be an acute intake especially in small weighted individuals such as frail or infants and is life threatening, or long term use of very high supraphysiologic amounts such as those used via guidelines in India injections of 3000,000iu, 600,000iu injections.
manufacturing mistakes are the majority of causes of hypercalcemia due to supplementation and food fortifications.
Fat solubility refers to its molecular construction, how it travels in blood, how it interacts with other molecules. In the intestine, depending where, vitamin d is passively diffused/absorbed, or further on down it can be absorbed as part of the sterol system, it is not a fat, streols and fats have different mechanisms of absorption, though they get all piled into the micelles, they then have different postal codes.
In the lymph vitamin d bonds with VDBP, albumin, and some lipoproteins like HDL, hdl is highly used in the immune system as well.

I have been deficient for 3 years. I believe it has much to do with my bad health. I asked my doctor if vitamin D was important and related to my bone thinning (I had 6 X-ray images). He said nope and the conversation ended right there and then.
Well I am not blind anymore as I used to be. 3 years of sickness and +20 consultations gave me a wakeup call to how normal doctors work (treating symptoms and not root cause). I have learned about nutrition and the importance of eating healthier. I found a dietician who has taught me much of healing with food. She charges $100 per 1-Hr session.

This new woman gave me a check-up, an order for a blood test, and a prescription for 50,000 units of vitamin D a day. She told me she automatically prescribed that for all of her senior patients because they were always deficient.
I went to the pharmacy and asked the pharmacist if this was common practice, since the amount seemed high to me. She said yes, with some doctors. I asked if it was D3 and she said no. I asked what about K2, and she said, “I’m not here to criticize doctors.” She knew this was wrong, but she was just doing her job.

    Key points:
00:03 🌞 Vitamin D deficiency affects over a billion people worldwide, with 70% of Americans having insufficient or deficient levels, increasing risks of various health issues.
01:39 🦠 Vitamin D regulates calcium, supports immune function, and significantly impacts the risk of infections like COVID-19, playing a vital role in preventing serious complications.
03:19 💉 Testing for vitamin D is crucial, yet often missed by conventional medicine. Optimal levels differ from normal ranges, with 45-75 nanograms per deciliter considered ideal, not the common 30 or lower.
06:35 💰 The cost of untreated diseases due to vitamin D deficiency can be significant, outweighing expenses related to testing and supplementation.
11:50 🏙 Lack of sunlight, sunscreen use, urban living, certain medications, obesity, aging, gut issues, and poor diet significantly impact vitamin D levels, contributing to deficiencies in modern lifestyles.
20:42 🦠 Low vitamin D can lead to inflammation and weaken the immune system, causing frequent sickness.
21:24 💪 Adequate vitamin D levels reduce flu risk by 75% and can alleviate bone/joint pain, mood issues, and fatigue.
22:31 🩺 Low vitamin D is associated with cancer risk due to its anti-inflammatory and immune-boosting properties.
28:43 🦠 Severe COVID risks are higher in those with low vitamin D levels; supplementation may reduce complications and hospitalizations.
30:18 🧠 Vitamin D influences mood, link to depression, anxiety, and cognitive disorders; optimal levels crucial for overall brain health.

I agree with how crucial vitamin D is and how incredibly difficult it is get to from food. The sunshine vitamin cannot really be synthesized since we don't really go out in the sun during the critical hours esp naked enough to get it. Here in the US and ither advanced countries we have access to natural supplements and people go thru medical content. What about countries like India where the supplement awareness at least for something like vit D? Though we have sun almost all the year round thru out the country, we are not really out in the sun naked. Most of the time they eat plant based meals. At least my parents were out in the mid day sun half of the year playing as kids and me and my sis during school breaks but that was decades ago. I got my first vit D testing done 11 years ago and it was 25 or something. I didnt supplement back then. Though I was in the normal range, it was not optimal. Do we also need to consider ethnicity into consideration for some supplements? Are the Indian suncontinent genes or near equator populations different in metabolizing stuff differently than someone from higher longitudes?

He should mention that Yes there is a difference, because the races are not alike. Blacks and darker skinned people need at least 4 times the amount of sun than very fair skinned people do. Fifty percent, not naked, skin needs exposure. It is mind boggling to me to see blacks slathering up in sunscreen in the US.

I took the Function Health test. My vitamin D level was 94. Top of normal is 100. This is their reply: "Your vitamin D levels were in normal reference range but higher than optimal (Greater than 75 ng/mL). While low vitamin D is a problem, having too high levels of vitamin D can cause problems with a buildup of calcium in your blood. This can progress to kidney problems. Your calcium levels and kidney function are normal currently. Please talk to your regular doctor about possibly holding your supplement temporarily and readjusting the dose of your vitamin D supplement". I was taking D3-10,000 / K-2 190 per day. I went back to D3-5,000 / K-2 90 per day except two days a week I also take an additional D-3 5,000 and a K2-100.

    Dr. Hyman,
I am very interested your claim that our Vitamin D status affects angiogenesis. I do understand that we don't want to encourage angiogenesis in tumors, but we do want to support it for Heart and Skeletal muscle fuel / oxygenation to enable CV health and exercise performance.
(I subscribe to Dr. Seyfried's work in starving cancer of fuels like glucose and glutamine.)
Any time I find a health finding which is at at cross purposes, it makes me want to dig deeper into the subject in order to understand what is really going on if possible.
    1. Could you share the Vitamin D / anti-angiogenesis study you referenced?
    2. Would you consider digging deeper into this subject so that you could do interviews and / or an explainer video about it?
You will be happy to hear that just last night when I was evangelizing about the benefits of Vitamin D and zero processed carbs for health to a customer, he shared that his doctor referred him to your diet book to help him modify his nutrition in order to control / reverse pre-diabetes.
It seems that he is one of the lucky few to have a competent Doctor in these terribly misguided times.
I forwarded the above video to him to help get him onboard the Vitamin D path to health.
    Thanks for all you do.
May you and yours be well.

I found out I had vit d deficiency (along with iron deficiency and b12 deficiency) last July. Had a hell of a time supplementing with anything. My body was so deprived at the point of me trying to fix my deficiencies that supplementing caused panic attacks (from Vit d) and horrible muscle aches (from iron supplement). I am now using that dminder app to track my sun from outside since I’m having so many issues supplementing. My last labs were at 42.3ng/mL and my Dr thought this was fantastic. Meanwhile I’m still having so many issues including terrible anxiety and depression. Optimal my ass. I think I’ve been low for years because of constant sunscreen use. 42 to one person is not the same for me when I’ve been deficient for so long.

I have/had the same reaction. I knew I was low in vitamin d. Last time I got it tested my levels were 30. My doctor told me it was good so i stopped taking vitamin d. After a while I had panic attacks, brain fog, dizziness, bone and back pain, heart palpitations 24/7. I thought I was going crazy. Then I started taking vitamin d/k2 with magnesium. The first week I had heart palpitations and bone pain from taking it but after a while it got better. I guess my body was so deprived it reacted. I knew it was working because couple of hours later I felt good. Also I started sleeping better. Before that I couldn't sleep at all which made me suicidal.

Funny living in BC Canada my GP said everyone is deficient in vitamin D, and therefore they don’t test for it. She didn’t recommend any supplementation, though . I have dark skin and take vitamin D supplements as I can imagine that I am at risk of being deficient, particularly in the winter time . I am being referred to a rheumatologist, and the GP believes that the rheumatologist can request a vitamin D test, but it’s not something that she as a GP can do. I think that’s a bit ridiculous. It is also ridiculous that if it is known how important vitamin D is to prevent other diseases, why there is such an apathetic approach to just accepting “that everyone is deficient”. Lack of focus on preventative healthcare in Canada is quite alarming.

Heck no. If you aren't living at the equator, or if you have any disease at all...consider taking 5K to 10k to start. Then test your blood D3 level again. It should be 60-80 in US metrics or more. I shoot for a level of 100-150 personally, and it takes more than 10K of D3 to get there.

A good Vit D level is between 50-100 ng/ml. For healthy people, the appropriate dose is generally determined by body weight.
For your reference: 1,000 iu daily per 10 kg of body weight gives approximately a level of 70 ng/ml.
When supplementing with vitamin D, never forget the important cofactors magnesium and vitamin K2.
