Borderline Personality Disorder and Autism Spectrum Disorder

  • This video answers the question: What is the relationship between autism spectrum disorder and borderline personality disorder. When we talk about autism spectrum disorder we're talking about a disorder listed in the Diagnostic and Statistical Manual (DSM) that has deficits in social communication and social interaction as well as restrictive, repetitive patterns of behavior interests and activities. Borderline personality disorder is also in the DSM, however, this is a personality disorder, specifically a cluster B personality disorder, so it's in the same cluster as antisocial, narcissistic, and histrionic personality disorders. With borderline personality disorder we see a pattern of unstable relationships, unstable mood, impulsivity, identity disturbance, and anger, so at first glance it appears there are a lot of differences between these two disorders. We believe that they don't usually get confused with one another, for example, you won't see one of these disorders listed as a potential differential diagnosis for the other. This alone doesn't mean there's no risk of confusing the two, but it is an indicator. Usually we would expect that a diagnosis would be listed as a differential diagnosis, if there were a significant risk of confusing the two disorders. What we see with these two disorders rather is an overlap, particularly at the cognitive level. Adding to the potential confusion, of course, is that these two disorders can be comorbid. An individual can be diagnosed with both borderline personality disorder and autism spectrum disorder at the same time. We know there are a lot of differences between these two disorders. If we compare them using the five-factor model (openness to experience, conscientiousness, extraversion, agreeableness, and neuroticism), we see that when we compare borderline personality disorder to autism spectrum disorder, we would expect to see higher levels of openness to experience, higher levels of extraversion, lower levels of conscientiousness, and higher levels of neuroticism with borderline personality disorder. Both disorders tend to have the same level of agreeableness and similar levels of social avoidance, callousness, and restricted expressiveness.

    For me it's like having the two diagnosis is fighting against each other. The borderline part says I must be affectionate and connect with others or no one will like me, while the ASD part makes affection feel unnatural. The borderline part of myself makes me hate myself for not being affectionate and connecting easily with others. I start feeling reallu selfish, like something is deeply flawed inside me, and think 'no wonder nobody likes you'.

    I've heard many anecdotes of woman with ASD initially misdiagnosed with BPD. This research doesn't surprise me.

      we are now being diagnosed with both as it is now being seen as common for asd to develop bpd due to the trauma they/we receive

    I haven't had an official ASD diagnosis, but I have had traits of ASD/Aspergers since I was a child, and my struggles were overlooked, or the adults in my life chalked it up to being willfully disagreeable, or intentional "bad behavior" or acting like a "brat"-- even though I was mostly a well behaved child, I could get very emotional or overwhelmed and people around me (family, teachers) got angry and accused me of doing this intentionally, or to manipulate a situation. So basically my emotions and experiences were often invalidated growing up, and this constant invalidation gave me a lot of traits of BPD. Although I see now people are arguing about the connection between BPD and CPTSD, although to me it sounds like the same thing, it's just that the title Borderline PD has a "bad reputation" and people demonize it, the behaviors are just a reaction to sustained trauma.

      I've read that autistic ppl (Asperger) are not manipulative to others and honest. But ppl with Borderline are manipulative and are often dishonest (Relationship's). So how is it to have both Disorders?

    It's great how you describe any mental illness. You don't tell people to run away etc. Some therapists do online. I think it's unfair on someone who is suffering, not everyone with certain mental illnesses are toxic, sometimes its internal to themselves.

    I've been diagnosed with Asperger, but have never felt myself or been told by others that I'm bad at reading other peoples emotions. My experience has been that I'm quite sensitive to noticing changes in people's emotional state. I can be the first to say something about it. I still recognize myself in other traits from Asperger.

    Ive been diagnosed with BPD...and Ive been satisfied with that to a point. But I have watched numerous videos of grown women talking about their autism and I relate to them so much more. I feel I could have a co-morbid diagnosis or not BPD after all. Ive reached out to a Austism center in my area to see if my suspicions are true.

    usually some people tend to mistake asperger with narcissism because high function autism people tend to be very candid and irritate and people think they are doing it on purpose to hurt their feelings and somehow associate what they perceive as lack of empathy with narcissism. The issue with Aspies is that they have social disconnectedness and miss out on social clues as to how others feel; it's not intentional. This inability to "read" others seems something more in common, to me, with BPD. It all makes sense.

      Since autistic women have been misdiagnosed for ages, I'm wondering if the diagnostic criteria is wrong from the get-go. I had the same understanding of ASD as you do. There is a difference between insensitive behavior and the toxic behavior of members of the cluster B...
      There is a huge difference in how the brains of Aspies light up compared to the brains of sociopath, and the cluster B includes sociopathy. I am wondering if professionals simply misdiagnose women in particular because they can't read minds and members of the cluster B are per definition deceiptful.

    If you have been diagnosed with BPD, how does one go to find out if they are on the autism spectrum too? Not one clinician has said anything to me about it...Most don't even seem to understand BPD.

    If autism and borderline are frequently comorbid, and the both group showed elevated autism traits.
    Perhepaps the borderline group showed high autistic traits because many have a sub clinical ASD???

    I was diagnosed with ASD aged 15, having been referred to adult mental health services suffering with low mood, anxiety and interpersonal difficulties I have received a diagnosis of BPD. I believe I have been misdiagnosed because the professional working with me had had little training on ASD. They didn't understand me, they found my presentation 'weird' and couldn't put me in a box so they labelled me BPD - but that doesn't fit me either! (not disrespect to anyone with this diagnosis, but it's not the most appropriate for me and I think having it is not having a positive effect on my life. Some professional have made assumptions as soon as they read personality disorder and I've been excluded from certain services that I feel I could benefit from) I think my issue is that I am autistic and when I get anxious or depressed it exaggerates my autistic features so whilst I am normally able to use my intellect to behave in a socially 'appropriate' way, when I am under stress and the 'autism takes over', I present like someone with a serious mental health issue - but it's just me being autistic.

    I had an awakening recently. I’ve been researching personality disorders and I realized that my son,who was diagnosed with Pdd nos
    has so many symptoms I’ve been dealing with through my childhood and adulthood were exactly like his! He is very afraid of abandonment,he likes to draw and has problems with communication.he also has a hard time with change! I’ve suffered so much of that throughout my life ,and now I feel so good knowing why I had so much difficulty with jobs and school. Forgive my grammar I’ve never been good with that! I’ve always been clumsy and had a high threshold for pain. I really realized that when my son said he got hurt. And he said”Dad I get that from you” now I can forgive myself for the past and get the diagnosis I need so bad! I still cannot make
    friends and I want to so badly but I have trouble with relationships,my poor wife has suffered by my side.God bless her soul! Now I can plan for the future and get my son the help I didn’t! Sorry I tend to babble on and on!

      Muffins Muffins this gave me a chuckle! I do hope you find some peace and the fact that you recognize your wife has been there for you and are thankful kinda takes you out of the Narcissistic Personality Disorder category. Someone with NPD simply projects all onto their spouse.
      My Asperger's son worries sometimes that his ASD Narcissistic tendencies means he has NPD. I try to reassure him it's just the ASD variety. His dad is potentially NPD and there IS a difference having experienced both.
      I too discovered I have and my family members have ASD traits, despite a lack of diagnosis since my son's diagnosis. I had never heard of Autism before then. Explains a lifetime of 🙃

    My husband was diagnosed with borderline disorder in his teens, now we know he always has being an Asperger 🧩 It’s really interesting how these diagnosis overlapped.

    Thank you for sharing this information! Overt symptoms gradually begin after the age of six months, become established by age two or three years and tend to continue through adulthood, although often in more muted form. It is distinguished by a characteristic triad of symptoms: impairments in social interaction, impairments in communication, and repetitive behavior.

    Good video.
    My take is those of us with ASD who have any Borderline traits or DX or cPTSD have this not due to ASD wiring but due to childhood abuse.
    Also, those of us who are Autistic do not have less empathy. We actually have more, lots more.
    We simply have a more challenging time expressing empathy like those who are Neurotypical.

      Unfortunately the BPD diagnosis is often made primarily on the basis of self harm or suicidality ,female gender and clinicians' frustrations regarding "treatment resistant" symptoms (even in patients with no history of abuse , trauma or fear of abandonment ). Unlike ASD a BPD diagnosis does not require extensive testing/evaluation and is often based solely on clinicians' own judgements.

    I'm not sure how to word this but in simple terms: You are born with autism (on the spectrum), bpd you can form. now; it differs with people who can have brain damage, as they can show traits of ASD but are not autistic. It's a confusing area. but I have (in my experience) support classic autism with bpd (took yrs for the bpd diagnosis) + it was extremely hard. esp when a person is deemed to not have mental capacity. ASD on any level is/can be a struggle + an added diagnosis on top, makes it harder. now we know you can not cure autism but can manage.....throw in something complex on top, where would you start? double edged sword 😥

      😢 for you having to struggle so much. Everyone is different of course but personally I would consider looking for a humanistic therapist. Even though BPD is usually treated with dialectical behavior therapy, I personally would be concerned about DBT therapists' capacity to appreciate the difficulties presented by the ASD especially the sensory difficulties and meltdowns that could easily be mistaken for BPD symptoms. Most importantly whatever you decide I just hope you can find the compassionate support you need and deserve.

    What I also appreciate is the way you've included the big 5, as I often feel the natural personality/human aspect gets lost in the urge to understand traits through pathology.

      Same. It's beyond confusing. I don't self harm, I am not a risk taker, I don't gamble, I am asexual, I am not suicidal. But somehow I still have both bpd and aspergers?

    What I didn't find surprising was Systemizing being relatively the same in ASD & BPD, even though you did.. What I actually found quite alarming was Empathy results presenting at similar levels between BPD & The Control Group!! Really?!! Do we all have BPD in some form or another, and a control group doesn't actually exist?! Define 'normal..' Maybe everything & nothing is normal at the same time!! And.. imperfection really is the true perfection.. Oooo! Dr Grande! I only clicked on RUclips today to learn how to put up a shelf!! Phew!! Seriously though, this video has given me a furrowed brow! Challenged the neurons a bit! Thank You.. (I think..) 😯 P.s. Are narcissists actually attracted to people with higher levels of empathy? (Genuine Question). Thanks again.

    It would definitely be one of your "less popular" videos you discussed in your recent subscriber milestone video, but Vegni, D'Ardia, and Torregiani (2021) recently published another paper on this topic, so it does seem to be gaining traction. It's a peripheral interest to me since I'm defending my neuroscience dissertation in a couple weeks on functional brain connectivity in ASD repetitive behaviors, but the thing that really sticks out to me is the extent of the negative reactions in both of these conditions to asymmetries with expectations: Neither cognitive profile is particularly associated with flexibility. Fwiw, I was diagnosed with one of the two as a child, but in my adult life, I've found the other might be better. I'll leave which is which as an exercise for the reader.

    I'm autistic and honestly dont see how they are similar but a lot of people with ASD uve talked to have been misdiagnosed/diagnosed with borderline earlier. I also dont really see the empathy thing personally, I often feel that I'm overactive to other peoples emotions, like if someone is showing signs of sadness I may start crying without being able to stop and just feel really sad for them

    I think I may have both . I haven’t been diagnosed with autism yet but I have previously been diagnosed with BPD but my cognitive abilities and intelligence are high , I also make movements with my hands when I’m excited especially when I rock climb it’s my special interest, I listen to music and see the path but when I dance up the walls I get excited it’s very fun for me, I can tune everything out around me and just focus on my thing. I miss climbing.

      a lot of people with CPTSD have BPD, such as myself and is most likely the reason why I now suffer with BPD.

      I've got both... I believe the autism element affected my perception and understanding of the experiences I've got which got me diagnosed with CPTSD. I don't have any of the personality problems associated with CPTSD but my traumas come from a few places and started at a young age, so I technically qualified for it. The autism makes it more complicated as it causes me to remember things very vividly down to what the weather was like that day, but I also have a wonderful mental universe to disappear into in order to escape it.
      The two influence each other a fair bit (probably because they're both occurring within the same brain) but IMO there's no overlap. Not for me at least.

      not to mention that lot of autistics ALSO have C-PTSD. Which unfortunately gets misdiagnosed as BPD when the symptoms from both disorders are combined.

    Was diagnosed Asperger's, SPD back in my childhood years and going in for a re-eval as requested by my PCP. Was having problems as a child and now as an adult; interestingly enough was diagnosed with Bipolar Disorder however yet none of the BD medicine did anything but cause side effects. Note I'm definitely not anti medicine for those who need a specific type. Don't have psychosis but this is one which is also overlapped and misdiagnosed. I'm absolutely positive that I'm ASD and not BD as was noted in a few sessions which was a mere oops moment, but he was a good Dr. overall. Just had a lack of ASD knowledge which I've heard many have since focus is primarily on other disorders first from observation. I think it would be interesting to see a video like this about Bipolar Disorder, BD and ASD since I've heard of many cases to which those with ASD as adults are being seen as having BD for some reason, even without any psychosis. I can understand why in a way and see the patterns on a personal level as I've studied it a lot, but will leave that out. Nonetheless, I enjoy your videos Dr Grande and keep the data flowing. ASD needs much more information to stop the myths and show the scientific proof as of why they changed it to ASD which to me was a very very smart move. Truly is a spectrum and I also think BD should be a spectrum since 1 and 2 is oversimplification, but that's for another time during DSM revisions.

    The two disorders are very common. And now when looking at the treatment things get even more interesting. DBT which is a treatment for BPD uses Applied Behavior Analysis with cognitive behavioral therapy and mindfulness to treat the disorder. And Applied Behavior Analysis is one of the most common therapies for ASD.

      They’re two completely opposite disorders. The context of every similiar symptom (if any are similar at all, which they don’t appear to be) is very, very different. In BPD, the main aim of the individual is to manipulate to get their way because they didn’t have enough structure as a young kid. To claim ASD is like BPD is both irresponsible and incorrect. Unless you’ve had an autistic call you up at 2am to claim they’re going to kill themselves and it’s your fault like borderlines do, don’t compare the two conditions. Autism Spectrum Disorders are not mood disorders.

    BPD is a response to attachment trauma or other long term abuse. Autistic people are born with hyper-connected nervous systems and have a lower bar for traumatization (something that traumatizes an autistic person may not traumatize an NT person). We are more likely to experience complex trauma in my opinion
    I think much of the DSM criteria for BPD can also apply to traumatized autistic people (who can become easily traumatized even without attachment wounding or long term abuse) and there is a frequent misdiagnosis of BPD in autistic women or other high-masking folks (this happened to me).

    Woah, the thing about systemizing is fascinating. Finally puts words to something I have always known about myself. I have BPD but not autism. I have often wondered about the relationship between these two disorders because I have some autistic traits (mostly social deficits) but I believe they are more easily attributable to my BPD and complex trauma.

    I'm so incredibly confused.
    I suspected i have BPD: Fluctuating emotions, lack of impulse control, history of drug abuse, unstable relationships, high fear of abandonment, high level of empathy and can tell what someone feels which affects me too.
    Spoke to a pychiatrist today, she said its more likely im on the ASD spectrum since i'm senstive to (annoying) sounds, i systemize, i am very analytical and have a particular interest in computers, games and pychology, and have a need for routine (once it breaks, its all chaos).
    What i dont get is the follow: Finding a partner, having a relationship with an emotional connection is what i desire the most, i get super clingy once i have it, i overshare, etc.
    That seems very much in contrast with ASD doesnt it?
    My psychiatrist argued its more likely i interpret emotions, face expressions as some form of data, but that WHY do those emotion i see/sense affect me? Sadness, aggresion, love, joy everything. When im in a room with many happy people i get super charged with happiness. I just dont get it.
    Do i have ASD + BPD traits? If so, then where does my empathy come from? Do i Just ASD? Just BPD? Im so stuck man.

    I’m diagnosed with BPD, but I relate to many autism traits (and feel like I learned how to mask early since I was socialized as a girl). But it feels like a lot of BPD traits and autism traits contradict each other, so I dunno…

      I have autism. I visit a female autism page, daily. There a significant number of the ladies, who were misdiagnosed with BPD, and later, this has been changed to autism, in the last few years. Especially since it’s becoming more understood that females present differently to males, for instance, we are far better at socialising than autistic men. and as children and teenagers, our 'meltdowns' can be interpreted as tantrums and moodiness. I would say, for myself especially, that one of the main differences is that I don't have a fear of abandonment. I am the complete opposite . I am more than happy in my own space and often this has been a problem in relationships. The other person can feel that I dont care for them, enough. The other difference is my restricted, repetitive behaviours. I am very asocial, and will choose my special interests over human contact, most of the time haha

      I think women are way more likely to get diagnosed with BPD than men are.
      maybe that could effect potential misdiagnoses

      I am wondering about that too. Are they even sure that their test subjects are not misdiagnosed with BPD when they really are autistic?
      I suspect that I have autism. I am not diagnosed. I've met women with borderline personality disorder and histrionic personality disorder. Those women are so incredibly annoying... The cluster B is the dramatic personality disorder. As a probable autistic person, I absolutely hate drama, mostly because I can't figure out how to handle these outbursts. I don't have the highest level of empathy in the world, I'll admit it, and I don't feel sorry for people who are so obviously manipulative and prone to exaggeration to get sympathy. I also don't feel like rewarding the behavior, but I often get sucked into listening to that load of *** because it's difficult to tell someone to stop when they are telling you a sob story and you can't prove that they are lying even though they don't sound credible at all.
      This being said, my sister is diagnosed with autism. She has meltdowns, which I don't have. Her case is worse than mine. She was a much more selfish child than me, and I can see that someone like her could perhaps be diagnosed with borderline. However, in that family dynamic, her behavior is explanable by birth order and other things. For instance, she was premature, which is a risk factor for autism. She ended up being much more nurtured by my mother, who seems to have bonded more with her because of this traumatic birth, and my sister really ended up exploiting that favorite status. She is the most spoiled of all the siblings, and that probably contributes to feelings of entitlement in adulthood. "I am weak, take care of me and give me my way."
      I am not like that at all even though I am suspecting that I share the genetics of autism... With all her flaws, my sister is still quite different as a person from these other histrionic and borderline women who are not organized, not into patterns, not rule-based at all... Just unintelligent, reckless, insecure, deceiptful, etc.

      I also know 2 women who were married to men diagnosed with Asperger's. Both men had parents who were encouraging them to think that everything they said was gold, that they didn't really have a condition, etc. They were encouraging their sons to be self-righteous and dominating in their relationship even though these 2 guys were obviously challenged. That's chauvinism, really... Old parents who wan their kids to "take leadership" by subjugating their wives.
      A lot of parents deny that their kid has a mental illness. I also know parents of a man with bipolar disorder who are like that. They encourage their son to stop taking his pills. They don't want the stigma of mental illness and prefer to live in denial.
      Since Aspies can be clueless and do lack empathy, the result of this kind of my-kid-can-do-no wrong and men-must-dominate-women mentality is to be expected. The guys didn't want to compromise or admit their wrong-doings, which is very arrogant and impossible to live with, whether or not someone has a mental condition. One of them was occasionally violent, so much that he caused permanent nerve damage in his (ex-)wife.
      Now, is this narcissistic personality disorder AND autism together? I am not a professional, but I would think that it's just autism mixed with a very misguided upbringing, denial and social expectations that these men can't possibly apply properly. They lack empathy and people smarts. What can one expect?

    Would you consider doing an ASD and Schizotypal Personality/Disorder comparison? There have been a few studies comparing the two spectrums and I'd be really interested in hearing your take on it.

      Great comment. Definitely sounds like something that should have been done a long time ago. I've never understood how schizotypal traits could be attributed to a "personality" disorder. These types of symptoms seem to clearly indicate primarily neurological rather than psychosocial problems.

    I have 2 ASD diagnosis and I’m not aware of any of the classic BPD traits however I am very big on systemising. I regulate emotions really well. However after prolonged contact with an NPD individual who cheated on me and lied about everything I did start displaying some more unusual traits (paranoia, spying, hiring a private detective, contacting people to get to the truth) - I think it was in reaction to his psychological/emotional abuse.

    Than those tests for "empathy" must be flawed. Both BPD and ASD tend to have higher empathy than neurotypicals.

    I am on the spectrum but do not have any personality disorder. I have a friend that has borderline. I find it very hard to determine why certain actions are taken as they often seem very illogical at times and likely to cause her pain long term. I also am not prone to being sensitive to rejection. No one likes rejection....but I find I dont mind provided I have been clear and there has been no miscommunication. I also am very emotionally stable...I dont get angry quickly, I am more naturally prone to pulling back and I can be fine by myself for very long times.
    I feel very much for those with BPD. It sounds very exhausting emotionally. Take care everyone.

    My ex definitely has bpd traits. She told me at one point, her parents & teachers growing up thought she was on the Autism spectrum. She was never tested, the school didn’t want to bc she was getting very good grades & it wasn’t impacting her. However it definitely impacts her social life as an adult.

    Is it possible that early neglect and attachment issues in a child already experiencing signs of asd before the age of 8 could actually enhance the chances of bpd co-occurring with high functioning asd later in life.

    May I ask what kind of empathy was measured in the study? Cognitive or emotional? I am curious about the social aggression both groups manifest. Would assume BPD would be a lot more controlling, manipulative, etc. Asd folks are socially naive.

      I was wondering about exactly the same question. People with BPD can have strong cognitive empathy (the capacity to understand another's perspective or mental state) with some suggesting it can be particularly well developed in someone with BPD. However, research suggests that people with BPD suffer from low emotional empathy; that is they have trouble feeling sympathy and compassion for the suffering of others.
      For more information read the research findings or Dr Simon Baron-Cohen who is an expert in ASD. He has also written a book called "Zero degrees of empathy".

    Socially they act the opposing way, only the results, consequences are similar. Rather a consequence of failing in the same domain IMHO.

    Would you have any special therapeutic variation recommendations for people with a dual ASD and BPD diagnosis? CBTs? Vitamin B12? Cruciferous vegetables simulating fever and therefore alleviation of ASD? Omega 3s in plant form to help build the grey matter around the amygdala? Interesting video. Thanks.

      In cases where BPD was diagnosed 1st it may just be a reluctance by health professionals to admit to having initially made a potentially harmful misdiagnosis .In patients who genuinely fit both Dxs abuse may be an issue. I would suspect that children with ASD (especially if undiagnosed ) would be more susceptible to abuse/neglect due to parental stress and frustration and/or tendency for people with ASD to be naive regarding other people's intentions.Many people with true BPD have a history of childhood abuse .

    I'm looking for an explanation as to why certain people suffering autism already know the right answers without having to study in school or college for years!? I think it's common sense.. Anybody know what i'm talking about? I believe I have asbergers but not sure I need to be diagnosed... But when somebody asks me a question I put all the pieces together and then I have the right answer...l for example 9/11 was a controlled demolition... I don't need to study demolition or avaiation to see whats right there infront of my face.... :)

    Autism and BPD are opposite conditions and there is no way to have both conditions as symptoms are the opposite. Modern science will find that many people diagnosed of autism and other supposed "comorbid conditions" in fact are caused by pseudo-autistic symptoms which may seem the same condition of autism in the surface. But autism is caused by a lack of neuronal prunning or extra-synapsis because of the protein mTOR. And BPD have the same risk factors of schizophrenia wich is caused by excess of neuronal prunning. Character of people is in fact an expression of the neuronal development. If people doesn't learn to control emotions and impulses, the neuronal development will be abnormal and causing that some synapsis that are important for a normal development aren't "cut" but wrong synapsis are prunned. Extra-synapsis in autism implies a local hyper-connection but lack of conductivity with ther rest of parts of the brain. While BPD is a combination of an abnormal neurnal prunning and schyzophrenia is caused by excess of neuronal prunning. The conclusion is that the cognitive theraphy of more than the 90% of the professionals is still almost useless for such conditions as they still don't know the results of the last neuroscience investigations. Most people outside the autistic spectrum is in fact inside the BPD spectrum, but it will be quite hard for the society to accept this fact. There already exist "a cure" for all abnormal neuronal development and it's a triglyceride of DHA with DHA in the center such as human phisiology of human breastfeeding (check for brudylab). This tryglyceride is 5 or 6 times more powerfull and antiaging than any other omega3.
