Do keto diets work as well as SGLT2i drugs?

  • SGLT2i drugs are quickly gaining popularity for improving cardiac outcomes in people with diabetes. But this is despite the fact that they increase LDL. How do we reconcile this? Because they can also raise HDL, lower triglycerides, reduce small LDL particles, lower BP, and reduce inflammation. What else accomplishes all these effects! Low carb diets! Maybe it's time we start talking about nutrition more than drugs.

    So the medical industry and big pharma trashed low carbohydrate diets but now they have a drug that replicates keto they find these markers favorable but not before these new drugs 🤔

    The research evidence suggests that semaglutide (and possibly 5-HT3 antagonists) increases ketone body production. This needs to be studied, especially in lean controls, obese non-metabolically ill people, premenopausal women, and people of different ages and metabolic compositions.

    Early man ate Keto. He did not tune his TV to corporate drug commercials intoning "ask your doctor" about some new big pharma med with a contrived name.

    💯 the problem is that big pharma makes big bucks on an Rx, and nothing on a well-formulated keto diet. 🙄 keto did all that for me, for free.

      @@angelgirldebbiejo many have been successfully doing keto, even carnivore (zero carb) for years. Best way to get rid of cravings is to stop feeding them. Then, we have to be proactive with our shopping, meals, daily routine, family gatherings, etc. but it can be done and maintained, just like sober and clean AA and NA members who have walked away from drugs and alcohol. We can do the same with junk food. But Big Pharma and doctors won't push real nutrition but only what benefits grain and snack companies. Doctors don't look at the source, they treat the problem. They aren't taught to fix the problem, but to put a bandaid on it. They are required to only take one nutrition course with outdated curriculum taught by Big Pharma. It's a great design to keep everyone just a little sick, addicted to sugar, seed oils, processed junk and highly palatable foods and dependent upon Big Pharma's drugs to treat their self-induced addiction and health problems such as blood sugar issues, diabetes, anxiety, NAFL, depression, ED, PCOS, acne, and more!

      Anyone CAN stick to it if youre not subbing out foods like bread with "keto" recipe. I was T2D and totally reversed it with lchf, OMAD. Intermittent fasting and now down to OMAD with zero cravings and never hungry. When I was eating more than 10g carbs I could not say this. I eat meat, fish with only a few veggies 2-3 times a week. My woe for life as I feel 20 yrs younger and my my energy is amazing at age 63. Labs are excellent for first time in 30 years

      @@marianne1959 I agree! Those keto versions of bread and sweets aren't helpful in the long run because we haven't changed our mindset for one. We just switched to eating lower carb junk, which can definitely trigger cravings and don't help the sugar and carb addiction.

    Where there's a choice between nutrition and medication, I will take nutrition every time. Why eat food that my body can't handle and then take a pill to mitigate the consequences when I can give my body the food it needs and let it do its job.

    Drugs are not about a cure, drugs are to keep "customers". When someone is "cured", they are no longer a customer.

    Well. Since there’s now a medication to raise ldl- the new recommendations will probably be to raise ldl 🤣

    I am one of those people that strict keto only goes so far helping my blood glucose. So I had my endocrinologist put me on an SGLT drug. At first it was wonderful. I had blood sugar in the low 100's. Then one day I just couldn't move. I was too fatigued to even stand. Thanks to a comment by Eric Westman about his worries about combining these due to the risk of ketoacidosis, I pulled out my keto mojo meter and tested my ketones. The meter registered up to 9. My reading said "HI". I stopped the drug and after 3 days barely able to move in the bed, I recovered. So that's my n=1 warning story about keto and SGLT drugs.

      I guess that begs the question. How many carbs should you consume while on Farxiga? I had a similar experience, but ate half a cherry pie lol, and over night felt better. I’m not diabetic. I’m taking Farxiga for heart related issues

      Thank you for this post. I have been on and off Ozempic, Wegovy, and Mounjaro for weight loss and pre-diabetes for over a year now due to bouts of nausea and vomiting, as well as supply shortages. After my latest bout of vomiting and gastric distress i finally decided to start a keto diet, but was unsure whether to continue on Ozempic or stop. I think you answered my question, I will hold off on the injections because of the risk of ketoacidosis.

      @@guyelluzzi2705 You are better off if you can control it with diet. I cannot. I will say that once you get accustomed to these drugs, they work well. I have worked my way up to 12.5 mg of Mounjaro and have really good blood sugar control.

    "If it's too good to be true...." - "if it's too EASY to be true..... " - something is rotten in the state of sickcare.... sounds like there will be problems down the road with these meds...

      Ah yes, there might be, but the pharmaceutical companies will produce a pill to fix those problems, and then the next and the next. Gwen Olsen was a pharma rep fir 15 years and questioned the side effects of a drug that they were being taught about, and that is exactly what she was told that they would create something to fix the side effects.

    Thanks for the Drug vs Nutrition info. The health-minded person will always go down the nutrition route. The Lazy, processed food/high carb food person will I'm sure opt for the drug route. CHOICES! I'm not making a judgment, I'm stating facts is all!
    I've personally been OMAD - Low-Carb - medium/high saturated fats - high animal protein for the past 6 years. I've never been so healthy. I'm 63 years old and counting ; ) See how EASY, HEALTHY, and TASTY it really is. You'll be amazed!

    You forgot to mention one key observation: SGLT2i increase blood ketone levels and is one of the hypothesized MOAs for their cardio-protective effects.

    I take metformin 850 mg a day, 500 mg of berberine, and am on a low carb diet. I also walk 10-20 minutes after every meal. I'm down to 166 pounds from a max of 233 a few years back. In the last year I dropped my A1C from 6.5 to 5.3 and lowered my triglycerides, and my blood pressure. I'm not eager to try any other medication.

    Sglt drugs look pretty amazing to me, but that's hilarious the cognitive dissonance between them and low carb lol. Another tool in the toolkit however I would guess price will be sky high for quite a while

    I totally agree with you Doctor when you say that most Doctors recommend us to take drugs instead of doing a correct diet, and I believe it's because drugs are a big business and once certain diets like keto does the exact same thing as a certain drug, then for some people is like : " why should I do a diet, if by taking this medication I will achieve the same results?" But here's the answer that you just gave us, the keto diet is a lot cheaper than this drug, keto ends up turning into a good life style habit, and it's addiction free from all those drugs... not sure why some doctors still recommend the meds instead of prescribing a good diet method, I lost 38 pounds and it took over a year, I'm not done yet as I still do have a long way ahead, but as soon I mentioned that I was doing keto to my family doctor and he saw my blood work results, he was super satisfied and I will never go back to weight loss meds or believe in any of those crappy drugs, all they do is empty your pockets and you still not losing the desire weight...

    All the positives you mention are my number directions exactly since carnivore! My LDL is high though so that's why I'm being referred to a cardiologist...can you believe it? Well also my EKG was different..not wrong just different looking than my last routine physical back 6 years ago. Probably cause I was laughing while the nurse was doing it

    So you never addresses the inflammatory effect with Sglt2 effect on the nervous system with lowering TNF Cytokines production? Its effect on the Vegus Nerves is also not addressed. I agree with you on nutrition but this is America?

    I live in India and am a vegetarian ( no meat).
    It's very difficult to follow Keto diet because that is totally alien to Indian vegetarian food.Also, you have to scout for some special food , if you to try to follow vegetarian keto diet.
    I have therefore struck a balance by consciously trying to choose low carb food and avoiding processed carbs.
    Still it is not sufficiently low in carbs.
    It's here that SGLT2 medicine fits in nicely. It improves my health and also I appear to eat some normal food that everyone eats.
    ( Most of the doctors in India don't encourage keto diet. They love to increase medication. SGLT2 inhibitor along with some reduction in carbs seems to the best option)

    Info on the current drug trials warrant a closer look as there were some deaths associated & some surgeries necessitated for gangrene tissues formed in the genitalia area as well as infection such as UTI’s, etc.

    Semi glutide worked good for my sugar but i didn't like side effects, really full feeling, nausea, constipation. I did mine from injections from compound labs, cheaper, was like 144 a month.

    Great video. Keto was a fad, ohhh you should do that its bad for your cholesterol, now this seems to be accepted generally low carb works(well at least at my doctors, even though i told him that yrs ago). So now we get to Wegovy, because its being abused by celebs and other non fat people(just 10-20lbs to lose i mean already at good BMI), also that there are no long term studies its being frowned on, just as Keto was. I wonder how long it will be until low carb and Wegovy are put together as a short term solution to get peoples mindset sorted. For me i believe this is a mental fight, one that does need a brain to be stimulated to say stop eating which Wegovy gives, but it also helps in my view get that person to start back to thinking about their health, exercise and focus them on the things that matter, as they lose the weight. Then they can stop and get on with the process long term, Wegovy is not a long term in any way solution, just like Keto isnt, but getting rid of that weight is a starting point 100%. Big Pharma is no different to the exercise and supplement billion dollar business, or the weight loss companies like WW, SW,JC that have people on their systems their whole lives, my mum i think was on a slimming plan until she passed away, must have been like 30yrs, she was the same weight at the start as the end as she YOYO'd for example but must have paid thousands. For me low carb is my way for the time being, updated macro's going to take the dog for a walk now for my extra 6,000 steps and gym later on, so lets see how this month goes, with the Wegovy, and then without and just the Low Carb.

    Im T2 for 25 yrs. Been low carb then keto for 10 years, not always faithful, but have held A1c 5.5 for 4 years. Chol acceptable. On Arb, statin and soliqua. Now spilling protein. Doc wants me on Farxiga but i see keto on SGLT2 risks ketoacidosis. Going to try it on Mediterranean/smart carb diet and watch my labs

    I would love for you to feature Sara Aguilar, she is a well known Health Anthropologist, CEO of Keto Supplements and brand adviser for a number of wellness companies.
    She speaks at national events too. Sara is based in England but travelled over the world to source new products to bring them to the EU market.
    It would be really interesting to have an interview on:
    Keto Supplements and healthy low carb diets for: hormonal health, weight loss, reversing insulin resistance, improving mental health, athletic recovery and performance and overall wellness. - one or more of these themes could be referenced and coincide with national awareness days such as mental health awareness week, menopause awareness month etc. There is also research on ketones for anti aging and to mitigate known aging causes such as high glucose, inflammation and muscle loss.

    So those of us who seem to be stuck… my insulin resistance doesn’t go down, constant fatigue, no weight loss yet I’m diligent with my keto & IF… could this medication potentially help me short term achieve my goals?

      Try low carb, moderate fat and high protein diet. Add some light body weight only resistance training to start along with some simple low intensity cardio like walking if not already. Don't give up just yet!

      @@hurcorh you are so kind. Been trying to get more protein & since I’m elderly & a heart patient I was able to get a physical therapist to help me to increase blood flow in my legs & arms like in resistance training- but in my home. I’m trying! I eat OMAD since I’m fat adapted.
      BTW I AM low carb… on keto for nearly 4 years so…
      Recently had angioplasty in my right leg but I also have intermittent claudication in both hips- painful to walk a distance. So hopefully the physical therapy will improve me.

      @@hurcorh I’ve done all you suggested and then some for over two years and still the numbers are not coming down. If very frustrating when you’re doing all the right things and not get the blood sugar to budge.

      I had fatigue (twice) and it was because my BP had gone too low so I reduced my blood pressure medication (twice). I'm now taking 1/4 of the dose I had been taking when I started low carb diet. I've seen the doctor today and my systolic, the top number, was 114 (didn't notice the diastolic).

      Watch out for keto acidosis if you do try SGLT2 inhibitors. It skyrocketed mine on a low carb diet.

    Hello dr can keto cure liver fibrosis?

      Fibrosis means the development of scarred, not functional, tissue. Keto is not a drug or a cure from anything. Going keto will releave your liver from excess fat and stop further fibrosis. But cure it? No, but your remaining healthy liver will be most functional (depends on the grade of fibrosis).

      @@PETROS_GGG in 2014 my liver biopsy showed level 3-4 liver cirrhosis in December 2021 my fibroscan showed level 1 cirrhosis. Don’t let anyone, including your doctor tell you that lifestyle changes won’t make a difference. After 4 years of living a low carb/ketogenic lifestyle I feel the best I have. On only one medication and off all steroids. You have to take charge of your own life. SO worth it!

      Periodic prolonged fasting might as it boost the autophagy.
      Everything that boosts autophagy might improve fibrosis anywhere in the body, the question is how big the effect is.
      Periodically fasting for a couple of days might induce enough autophagy to remove the fibrosis to some extent.
      BUT, it is important that you prevent further damage to your liver.

      Autophagy. But you either need a long time in which no further damage happens or you need something to boost the autophagial flux. Something like a prolonged fasting repeatedly.
