If you start a new game you're creating a new save file. When you load your save you can also choose to play with another family that's premade like Goths or create more families yourself so you can meet with your other families. If you create it in the new save it's a fresh start without any changes you made in the other save. Some people like to edit premade builds and save them in one save. You could share that with other people by going into your game folder and copying it (the game folder is not what you can see when you have your game open, rather where is the game saved on your computer). If you download someone's save and put it in your game files you could have a world that someone else edited, so their families, builds etc.

It's basically the game someone have already played. Like you, for example, after you create a sim a start to play with it in a household you create a savefile. The creators of savefiles use this to modify buildings and pre existed sims so that the game isn't always the same.
