  • Surgery for chondromalacia patella is rarely done. It's only done as a last resort once you've exhausted all the other available options.
    The root problem of chondromalacia patella is that there is too much pressure on the cartilage behind the knee cap because the quad muscles are pulling the patella too much.
    If surgery cannot relieve this excessive pressure, then is surgery really needed?

    Dr David I have gained a lot of insight from your videos thank you. I'm 73 years old had ACL,MCL, and meniscus tear at 24. Suddenly about a two years ago the bothersome pain turned into cripplng pain walking with a cain and lots of pain started doing your glute exercises and knee swings. Than went to physical therapy that helped strengthen the leg muscle but still walking with cain. I had the injections before and they where not working any longer. But I came upon a treatment that incorporated your tail gate swing with ankle weights 2 months later pain greatly reduced and walking without a cain. Also added bike riding every other day

      Yes, the tail gate swinging can do a lot to help knee pain. It's such an underrated exercise!

    I am in this exact position right now and so frustrated. I was passed around to 3 different PTs at the Ortho rehab place trying to figure out the cause of my chronic knee pain. I've had pain meds, injections, graston, cupping, knee braces- and a year later, still cannot consistently run, and still get the same pain.
    I'm with a new PT now (4th one; new facility). They are giving PT 4-6wks before sending me back to the knee Dr for possible surgery.
    Your video totally makes sense in root cause- but in the past year of PT they've all focused on not just quad strength, but hamstring, glute and lateral hip strength. And yet no amount of strengthening exercises has helped. I just feel like something is being missed yet noone can figure it out.

      I had a similar experience to you and ended up with surgery, a “super easy quick scope”. It was not super easy or quick to recover. I lost even more quad strength and it’s been so hard to recover it (it’s been 4 years). I hope you find what works for you just sharing since I was in the same spot you are right now

    My dr.said I have bone on bone and need knee replacement. I do have a lot of pain and sometimes my knee will,give out on me when walking. What do you suggest
