The reason why there are two hells in Gensokyo alone is because hell is just a label you can put on any collection of realms. When you die, you will go to Higan (the place with the red flowers, see th9 VS Komachi and th17 st3), which is on the other side of the Sanzu River [= River Styx] (th17 st2). You'll be judged by the yama there and you may go to hell. Originally souls sent to hell would go to Former Hell (th11 st5), which is a finite place in the Undergroud, so it got filled after a while. The oni are employed there to keep the souls in hellish conditions. New Hell (th17 st4) has an infinite size and it's entrance is located right next to Higan, so it's a perfect place to bring new souls.
Hell got overpopulated so they had to move it somewhere, bringing everyone along with it, thus leaving the "Former Hell" with leftover spirits from the move. Since then, some oni and subterranean youkais started to move in Former Hell to create their own city of paradise and the *yama* itself game their approval. Also youkais from the above cannot enter the underground (probably because of the underground residents' implemented rules), that's why in TH 11 (Subterranean Animism), Reimu and Marisa had to rely on telecommunications from the surface.
The algorithm has brought me here, it has a decade of data on me, I must trust it. I like anime and I like bullet hell, so I must love touhou obviously. Also I do like your banter.

I mean no offense by this because you’re one of my favorite youtubers, but when its jump cut after jump cut, its kind of like repeatedly getting kicked in the nuts. This is just my opinion and i obviously don’t speak for every viewer, but it would be nicer if you included more commentary on the game (music n stuff) because sometimes the commentary feels kinda unnatural. Hope things continue improving though because it seems that you’ve been blessed by the algorithm
This is actually a conundrum I’ve thought about quite a bit, and unfortunately it’s rather complicated. So of course my style of video centers around the pacing being as fast as I can possibly make it. This is for a few reasons, the main one being that without me doing that, these videos would be waaay too long. Which is what happened when I was making my very first videos. Second main reason being that it allows for my humor to work (I think). So while I do apologize about kicking you in the nuts, I’m probably gonna keep doin that. Although maybe I can work on toning it down a bit. But the topic of commentary is interesting. When it comes to talking about the game (and this video was a big oopsie cause the sound effects were too loud) it’s unfortunately very difficult. Like I’ve already talked about before, unless bullet patterns are HIGHLY specialized, I can’t say anything that I haven’t already said. Let’s take music for example too. As I’ve already made apparent, I don’t know an inkling of music theory. This combined with the fact that touhou songs are pretty much always loud and energizing with the dynamics, talking about them for more than a sentence or two without repeating myself is basically impossible. And perhaps that’s fine, people aren’t coming here for my nuanced music takes. But what are they coming for then? This is a great question, because I have no idea. I should probably make a community post with a poll that asks this very question. As my videos are basically a mystery box of wildly different ideas. It’s like when you are drafting magic the gathering and you end up with 5-color good stuff. The draft didn’t have enough resources in one or two colors to make a good deck, so I ended up with 5-color, for better or worse. It’s true that I could’ve gotten by on just 3 colors, but Chad is a greedy bastard and likes to go all in once the ball starts rolling. Then again, my videos aren’t magic the gathering. They have things like titles and descriptions that point to the video being about touhou 17. And yet sometimes it ends up being relegated to a side dish, despite being advertised differently. I’ll wrap this up though. As you can tell, I like talking about random shit. That’s been the one common thread throughout my entire channel. The tough part is that in order to find success on the business side, touhou is the answer. But to find success on the artistic side, that’s not always the case. Looks like I’d better work on finding an answer to this abstract problem though, because my content needs to be on my A game at times like this. P.S. thx for the comment! I think that touhou 18 has an easier extra stage then touhou 17 but it could be because of personal experience. So… when are you going to play the pc-98 games? I mean, pcb is a game you have to finish someday but just replace the b with an 98 and then we have some good (in reality not good) games. Especially touhou 2, that game is just pure he- I mean good, good game.

Anyway, the reason why there are 2 Hells is because when Former Hell (the one from Subterranean Animism) got overpopulated, they decided to move it to a new place, the one you explore in this game, well, one of it's many areas called Animal Realm to be precise. Beast spirit mafia (UFO-like thingies) used to rule here, with 3 family bosses at the head - Yachie (Stage 4 Boss), Saki (Extra Boss) and Yuuma (Final Boss from 17.5) matriarchs of otters, wolves and eagles respectively. And then Keiki (Final Boss and God, who can give life to inanimate objects) moved in and chased them away. Since beast spirits are only spirits which can't harm her moving clay figure army, they started pestering Gensokyo inhabitants and brought human of flesh and blood with them. So basically, as you've already surmised, in this game you're doing yakuza's dirty work and playing as bad guys for once! 14:30 Don't forget about the scene games (9.5, 12.5, 14.3, 16.5). I'd suggest playing them on stream, since there are quite a lot of levels compared to main games, and some individual stages may take a while to figure out, but you do you.
