    Hall mark of copd is increased airway resistance /increased work of breathing depending on TV andRR/wasted ventilation and V/qmismatch.

      Mechanical ventilation provides positive pressure of larger airways and premature collapse of bronchioles and stabilizes alveoli.complication are barotruama rupture of lungs bleeding.vq scans available from PE.

    Surgical removal of bullae common in copd helps when increase in dead space

    Dead space means that part of airways which do not take place in gas exchange o2 and co3.anatomical DS includes upper airways +those bronchioles which do not take place in gas exchange.that,s the reason fr reducing dead space by opening up trachea.

    Physiological dead space is more important. It is ADS+those alveoli that do not take part in resp.

    Pulm embolism is type 1 failure hence hypoxic failure. Type failure however may slip to type 2.hence clinical features of pe resembles hypercapnic breathing.pulmoary edema develops quickly.

      I don't think pulmonary edema is hallmark of acute PE

    Wasted ventilation results in hypercapnia. Which is compensated by kidneys.

    Hypercapnia results in a life threatening condition called resp.acidosis. the acid base balance disturbance results in increased production of sodium bicarbonate.if either of the organs fail to compensate.the patient needs artificial ventilation like bipap to treat types I resp failure.type 2 is a hypercapnic resp.failure which needs invaive procedure like traceostomy and attachment to mech.ventillator.

      early intervention with steroids/bronchodilators +/- Abx + CPAP . stop smoking, gets your appropriate vaccination & avoid airway irritants . . .
