I know you’re not aware of how much you help us as students but so many of your videos over the past two years helped me make sense of the ever evolving career and complexities of the field. 4 years in nursing school (Respiratory is my most hated topic) & almost 7 years working as one. Only every needed to understand NIV & Other types of ventilation since Covid because I work in the ED. Your explanation really blew my mind. I have been on Bi Level for over six years, I appreciate your classroom on this. One thing I will note however is that using a Pressure of 20 on I PAP is almost impossible because (at least in my case) that pressure is so great that my full mask will not keep a seal, and the headgear wears prematurely. That is my actual real life experience. We are now working with much lower pressures to achieve the same results. Not fine tunes yet currently 15 IPAP/10EPAP. Pressure support is 5. My mask now seals better and the machine does not overheat trying to overcome the mask leaks

    You are a great learning resource ; wish our local RTs are able to explain things in this manner.
    Our disabled child was using a bipap machine (Philips trilogy 100 in non invasive mode), however her recent sleep study doesn't show any apnea. She only snores. Pulmonologist is suggesting to use an APAP machine.
    Would you or anyone from this channel please tell us if an Trilogy Evo machine can be used as an APAP mode ( non invasive) ? From videos and manuals related to Trilogy Evo, we couldn't tell if it supports APAP mode.
    Thank you all ! This is a great channel 👍

    • @RespiratoryCoach
      @RespiratoryCoach  2 года назад +1

      I don't believe APAP is a mode option on the Evo, although their own auto adjusting mode, AVAPS, is. I'm not saying they're exactly the same, just stating that while the Evo may not have "APAP" it does have another mode that automatically adjust pressure based on feedback from the patient. Thank you for visiting the channel and best wishes to you and your precious child!!

    Good video, but I have a couple of small quibbles with it. Bipap is a juggling act (as you said) between Ipap and Epap---but also Oxygen and I time. I can't tell you how many times I've seen RT's put a COPD pt on an I time of 1.0, when they need an I Time of 0.8 and then they add to the problem with an Fi02 of 100%. They wind up out of sync with the machine, with an increasing V/Q mismatch from the Fi02 and it makes their breathing harder. So, your patients underlying condition is what really drives the settings. I time is important as well because the shorter the I time, the smaller the VT is going to be and I time wasn't mentioned. And it might surprise you, but I rarely draw ABG's on Bipap/Cpap pts. I just go by the 02 Sat monitor, respiratory rate and tidal volume.

    Hey Joe thanks for the video. I appreciate all your videos. They are so good I learn so much from your videos. Can you do a video on Avaps mode and how to set the minimum and maximum pressures along with epap settings.
