Metronidazole is indeed more effective when ingested with food because it directly enters the fish’s digestive system, where many parasitic infections (like Hexamita) occur. However, it can still have some efficacy when dosed in water, especially for treating external infections or when the fish is not eating due to illness.
That said, the effectiveness in water depends on factors like concentration, water parameters, and the type of infection being treated. While metronidazole in oxygenated water might not be the most efficient method, many aquarists and studies suggest it can still be absorbed through the fish’s gills and skin to some degree.
For severe cases or systemic infections, combining medicated food with a bath treatment can improve results.

If the fish is eating, it can be given a very small portion of food. However, as the treatment is relatively short, I wouldn't feed them during those days. The food you give them may not be consumed and will pollute the water and when they eat they will defecate, which will also pollute the water.
I recommend that you don't feed the fish during the treatment. It don’t hurt the fish.

However, if you can't find these medicines, as they require prescriptions, you could try OCTOCIL from Manaus, which also treats intestinal flukes.
