Not sure what incident you are referring to but in high sea states normally 6 torps will often run under smaller ships. It's just part of torp and weather dynamics.
@@DublinMarley The sub strikes on the CV task force. You often attaked almost directly from the front, my guess is the angle of impact was too shallow hence so many duds, that and them striking at the very limit of their reach.

@@MegaDobieDog Not sure. I am not aware angle of attack causes duds to happen. I will ask the modder and see if that is a cause.
