I don't quite understand the part about cutting the pattern to match neck size. If the neck is 14" long, does that mean I would cut the pattern where it says 14" ? thanks!

The pattern instructs you to cut on the bias, but you are instructing differently. Do you get enough stretch when you tie it cutting with the grain?
It should be cut on the bias to have enough stretch

do you have to add the lining?
Yes, definitely use interfacing. Otherwise the bow tie would be sloppy
Question: you cut the tie on the straight of grain when the pattern showed cutting it on the bias? How come?
You can do either, but I highly recommend cutting on the bias
