So the sum it up was it a defective part or was there a specific reason why the cam lobe was in that position ? , crazy how that happened, good eye finding it , you and the guys do great work
Awesome find. Is there any procedure that someone may spot this when installing the head or just lobe should be close to a numerical place/time on a clock?
Definently bent valves. Rocker arms do not have manufacturing defects. Engine have piston to valve collision and the stopping force brake the rocker. Was a hard hit . Both rocker is damaged hollow dd cams is week but will not and again will not turn that much without the reason. Some will turn some if you rpm this engines over 2000 spins on the shafts just a hair creating vibrations at idle and isb 30% range. Only setting the code if rpm reach 2800. Not before. Need to pull ddec report and check the highest rpm logged in mcm. Date stamp behinds them. I don't believed the machine shop made any mistakes but tighter valve guides will do the same damage. My money is on operator error but i do not see the ddec report. If the highest speed recorded is 2300 and the day stamp is after customer picked up the truck it is the hard downshift if it is the manual. Dt12 takes to 2300 too. Alright i going to sleep. Good night's. (One more thing that wrist pin have some bang bang to )
It ended up being a faulty intake camshaft. Lots of work hours and efforts spent on this job. God bless you. You are a truly hard worker.

The engineers decided to leave out keyways or splines or anything like that anymore.....progress 🤔
