It could be an issue with your cable. Worst case scenario it can be your Bluetooth module inside your PS4. You can try different cable and resetting the controller. There's a small hole behind your controller about where the L2 is. Try that and see what happens. Otherwise, PS5 is your last option bro :(

Might be the cable, try Turing on manually or rest the ps4 by holding the power button till it beeps 3 times.
My controller did this to my ps4 press the little reset button on the bottom of the controller and hold it for 10s then reconnect.
