    In this video, I discuss Alcon's new Clareon lens material. Alcon Acrysof Lens implants have been associated with unwanted glistenings that form in the lens material over time. Alcon recently upgraded their lens material to resist glistenings.
    Within a healthy eye is a clear lens. As we become older, that lens becomes cloudy. The cloudy lens is called a Cataract. In cataract surgery, the cloudy lens is removed and replaced with a clear artificial lens implant.

    I had my surgery scheduled in the morning. They just called me back and said i was having the old Acrosof installed. I cancelled and then they called me back and said i could get the new Clareon - just have to wait 2 weeks. Really glad I saw this video

    I had the Claren Panoptix trifocals in both eyes 2 weeks ago. I was going to get it done last year, but the new lens had not come out in Australia so I decided to wait, and I was so glad that I did. For anyone that is worried about the surgery don’t be. It was probably the easiest trip to the doctor ever. I had one eye done on the Friday and then had to return the next day and get the second eye done on the Saturday. In Australia, they don’t let you do both eyes at the same time. It will take a good 6 weeks they tell me, but I am very happy. I already don’t notice the halos when I’m inside at home, but I haven’t been out night driving as yet. For me, the halos seem to be quite mild and I’m getting used to them very quickly where my brain is just telling me to disregard them. I am very happy so far!

    73 years of age with mild cataracts and mild astigmatism, never worn anything but readers, 2.75 strength before surgery. Distance vision started deteriorating in the last 3 years approx. I had the Clareon PanOptix installed Jan 3rd rt eye and Jan 10 left eye. 6:30 am check-in and on my way home by 9am. Unqualified success and surgery was painless. First eye drops stung a wee bit for 20 seconds or so and then just some pressure. Mild anesthesia thru an iv, conscious but didn't care (I'd love to have a bottle of that, whatever it was lol). No after effects but I did lounge around 1st day and I'm following doctors orders not to strain lifting anything heavier than a gallon of milk and am careful not to get water in my eyes.
    Vision is amazing, now able to read bottom line of eye chart w either eye and that was on the day after surgery exam. Distance vision is clear & crisp with depth perception as it should be. Great to see the dashboard clearly w/out cheaters. Halos are a thing but I don't look directly at oncoming traffic and have no problem driving - doc says halos will diminish in a few weeks. Cost was $3200 per eye plus a $75 charge for eye drops. Medicare paid for surgery but premium lens/eye drops are out of pocket. Apply drops 4x per day 1st week, 2x per day 2nd week and then once per day for 2 weeks. Eye Associates Southern Indiana - Dr Pugh

    I just had the Clareon panoptix toric lens implanted two days ago. So far, I love it! A lot of halos though, even during the day, around lights. Hopefully that will improve. But I’m happy that I was able to get this advanced amazing lens in my eye. Far vision is great, mid vision is decent, but close-up vision is a bit blurry. However, I can see close-up with my right eye very well so I have my own mono vision going on.😊

    Doctorkrad, I had both eyes done with the Clareon and from the start I have had bad opaque vision. My vision is like when one wakes up with morning mucus in one's eyes. I asked my doctor if he replaced the lens but left the cataract? It has been about 6 months and have waited for it to correct but not in the least has it gotten better. In a week they said they would correct my vision with some sort of laser procedure. I also see huge halos from headlights of cars and lights at night. Why do you the lens didn't work for me? I would caution everyone to really decide on have this type of procedure. - However, I can read up close and when floating stuff move out of the way. Thanks for any information.

    Thanks so much for your video. Very opportune as I'm booked for Lens Implant surgery for presbyopia in prague next week. I absolutely abhore glasses and want to be free of them!!! Anyway since my research I'v asked the clinic if they can use the new Clareon Panoptix instrad of their current offering of the Acrysof IQ . So far they have not responded.!!! Im prepared to cancel and lose my deposit if it comes to it.

    Thank you for posting this! I need surgery ASAP and I'm wanting the Clareon PanOptix and found most but not all of my local surgeons are using it. Unfortunately the only study I can find on Clareon is a small one on the monofocal that showed a 50% increase in glare and halos. That was a little worrying. Have you found any differences other than the lack of glistening? Are patient VA outcomes the same? Any differences in night time dysphotopsias? Can you still see the rings in certain lighting? The refraction is still 1.55 so I assume you can. One doctor told me on the phone that they've found that the Clareon PanOptix does not fix as much astigmatism for some reason. I don't believe that will be a problem for me. Sorry to bombard you with questions, but this is the first comparison I've seen between old and new. Thanks!!!

      Hey Jeremy! So far I haven't noticed a difference between the two in terms of astigmatism reduction or dysphotopsia. I'd love to see a large study comparing the two. The newer clareon lens is not as refined on the ORA system yet (less total number of lenses implanted), and so I wonder if that had an affect on the other doctor's astigmatism corrections. I now have a few patients with Clareon Panoptix in one eye, and AcrySof Panoptix in the other. Next time I see them I will specifically ask them to compare their dysphotopsia experience in each eye.       ​@@DoctorKrad Thanks for asking. Yes, I had surgery in late Sept. My vision the day after the second surgery was 20/20 near and far. They didn't completely correct my minor astigmatism so I have some "gray shadows" around letters especially if it's light text on a dark background. The halos are a bit more than I expected and I think it's exacerbated by the astigmatism. At my January appt my distance vision in my right eye had gone down to 20/30 at best. I believe this is due to my ciliary muscle spasming from so much near work I'm doing after surgery trying to make up for lost work/business when I couldn't see. I've had issues with my ciliary in that eye for many years. The Dr. saw PCO and did a YAG last week on the eye. She thought that was the issue but my vision hasn't improved yet. That all sounds kind of bad, but I'm reasonably happy (other than the halos) and I'm confident that with some astigmatism "exercises" from endmyopia.org (I eliminated my astigmatism and all vision correction before cataracts with their reduced lens method) and getting plenty of distance vision to release the ciliary I'll have really good vision in the end.

    Age 45, high myopia and developed cataract ( after covid treatment in Dec 2020) in both eyes plus had to undergo retinal barrage laser in both eyes a month ago, i needvto operate righy eye cataract asap. I have none from diabetes, astigmatism, glaucoma, uveitis. I am convinced about using monofocal with UV filter and a lens that was launched 7-8 years ago rather than Clareon which was launched in last two years only.
    Considering all above Alcon Acrosoft or Hoya Vivinex will be better for me ? Will selecting glistening free iol model like Vivinex guarantee that i will be able to avoid its explantation ( for reason of severe glistening type of opacification) for next 30-35 years or even more ?

    With my surgery scheduled for this coming Friday, I am somewhat saddened to discover that the Clareon version of the Panoptix lens is unavailable in the UK. It was bad enough discovering that there are no centres in the UK offering ORA but to discover we are only permitted the old, more problematic AcrySof lens is infuriating. Why are we (and probably those in some other countries) denied the best system?

      Just speak with places that do the surgery and they will tell you how long and when it will be available. It was only made available in Australia about six weeks ago. I had the panoptic’s trifocals… Best decision ever, and it was worth waiting an extra six months to get it done with these lenses. Also the surgery was so easy.

    Is it safe to explant ioL?..i feel i was cheated by a doctor here, in the middle of surgery he said my lens support was weak & putting a multifocal was not possible and he inserted monofocal against my wish which i hated very much and don't wish to wear glasses at all..I'm extremely disappointed & went into depression with that i have to wear glasses again..had he told that before i wud never have gone to him, now I'm planning to go for explant and reinsert with a panoptix multifocal ioL that can get me rid of wearing glasses..now is it safe to explant the ioL?.. plz suggest..I'm 40 years Male what will be the consequences of explant and reinserting premium ioL like clareon panoptix? at the time of surgery he said my lens support was weak

    Have you ever explanted a vivity/panoptics acrysoft lens because of this? No one else seems to mention this defect.

    Dr. Krad, you had a video about a patient with a Panoptix in one eye and a monofocal in the other. Is this a good combination? Does she get the benefits that each offer...range of vision with Panoptix and sharper vision and better night vision with the monofocal? Seems like a good combo. I had one Clareon panoptix implanted 48 hours ago. Thank you

      There are some cases when putting a different type of lens in each eye works well. However, my default plan is usually to put the same type of lens in each eye. I think that it's easier for a patient to get used to one eye having a multifocal (to see both far and near) and the other a monofocal (to see far), compared to monovision where one eye is set to far and the other is set to near. At least seeing far well is in common with the two different lenses (multifocal in one, and monofocal for distance in the other).

      @@aniketnavalkar5974 My reading, intermediate, and daytime/driving vision are great. But I don't feel my night vision is very good. It's not the halos that are my problem. I do see them. My vision seems "dim" at night. I go for my 2nd eye tomorrow and will discuss this with my surgeon.

      @@farmgirl9245 even with Symfony there has been an instance where I made a turn in the opposite lane at night just because I could not see anything and I still have one natural eye.

    Clareon monofocals have now been around for 2 years. How do the glistenings look for them?

      Only been 2 years around is a turn off. Hoya Vivinex has been around for 7 years.

      Best of luck Jim! I haven't noticed a difference from overall patient feedback. I do have a few patients with Clareon Panoptix in one eye, and AcrySof Panoptix in the other. When I get the chance I'll specifically ask them to compare their dysphotopsia experience in each eye.

      13 days later getting the left eye done. And still real blurry.. go back to the doctor March 1st maybe hopefully find out what's going on then.. but they did instill my eye with trimax which is a antibiotic instead of using eye drops.. postponed the right eye surgery till further notice..

      That's 13 days today from having the left eye done. They did tell me the medicine could make me blurry for up to two weeks or longer but at this point I'm beginning to think something's wrong I sure hope this thing don't have to come out of my eye...

    I'm getting the Clareon Vivity. I'm not expecting miracles and frankly almost anything would be better than my current blurry cataract vision. If I can drive/bike/hike without glasses that would be amazing.

      @@doras.3178 Sure. I'm 66 years old. Cataracts in both eyes. My eye doctor is in his late 40s/early 50s and has done over 5K cataract surgeries. For premium lenses, he only uses Vivity (refractive), not Panoptix (diffractive). My right eye was my first eye and had astigmatism so they put in a Toric Clareon Vivity. I had same day surgery at a hospital so general anesthesia via IV so I was not awake during the procedure. No pain. Eye patch worn until check up next day. Immediate good vision in eye. Take 3 types of eye drops for 4 weeks after surgery. Left eye surgery 3 weeks after my right eye. Same results. I guess the most interesting part is how the doctor gave me the option for monovision (blended) so one eye is corrected for near vision and the other eye for far vision. I said sure, so he did the lens calculations, and decided to give me "mini-mono" which is a slight amount of monovision. My brain neural adapted right away (apparently it can sometimes can take months). I can see clearly from about 12" to far distance without glasses. Reading a book/using phone/tablet/computer no problem. I don't drive much at night but my night time contrast is fine. No blur/halo/rings/stars. Taking eye drops 3 or 4 times day for 4 weeks after surgery is a hassle but not bad. I would say the only downside was that it was an extra cost of about $5K since Medicare only covers monofocal IOLs. No more glasses is worth it. I *think* the follow up optometrist said 20/20 far and 20/30 near.

      @@tom95521 I was under the assumption “premium” means multi focal but you got mini mono vision so each eye is a mono focal set at slightly different distances,correct?

      @@Fireneedsair I have the premium multifocal EDOF Clareon Vivity IOLs plus a tiny amount of monovision added for even more depth of field. I think the normal Vivity is supposed to be 16" and beyond, so the doctor added about 4" closer vision by changing the depth of field of my left eye. I don't know if this is better than regular monfocal monovision, but I am happy not wearing glasses.

    Hi, my Dr. recommended the Clareon monofocal for my LE (I also had a vitrectomy in my LE).
    I read some people complain about a cosmetic reflection of the iol lens that their friends can see in their eye that looks like a twinkle/diamond sparkle. Are there any brands that this can be avoided or is that all the IOL lenses? I was also considering Tecnis monofocal?
    If both Clareon and Tecnis are good mono lenses then I would prefer the lens that people can’t see? Can you please advise. Thank you!

      Then tecnis is the way to go. No twinkle and tecnis does not have the glistening problem which is different from the twinkle. Glistening is micro vacuoles that develop in the iol.

      Yes, mild to moderate glistenings are still common with the Acrysof platform. Severe glistenings are not common.
