    PART 2 of the comparison. This video was recorded approximately 3 weeks after PART 1. Not a perfect comparison by any means, but in this video, a patient shares his experience with the Tecnis Synergy IOL from J&J, and the PanOptix IOL from Alcon after undergoing cataract surgery. During cataract surgery, the cloudy lens is replaced with a clear lens. He received a different lens implant in each eye, and he describes the quality of vision of each, side by side. He has excellent vision with each eye, but there are subtle differences between the lens implant technology. Of course, every patient is unique. But it's always helpful to get feedback from patients, especially when they are utilizing the newest technology. I hope this video is helpful for you.     What is a Cataract and why do we implant artificial lenses?
    A healthy eye has a clear lens. As we become older, that lens becomes cloudy. The cloudy lens is called a Cataract. In cataract surgery, the cloudy lens is removed and replaced with a clear artificial lens implant to improve the quality of vision.

    I did not have surgery with Omar Krad. But I had cataract surgery July 7 and 14th received Synergy. I'm looking to others to see their experiences. My experience has not been good. I don't have "clear vision" Everything I look at seems blurry during the day outside, inside, looking at the tv, looking at the sky, the lights in my kitchen, it doesn't matter what time of day or night everything isn't clear but has glare and is blurry. I'm three months out from surgery and just had the right lens replaced with Eyehance. I have good distance and intermediate vision with the Eyehance but I'll need readers. I wanted a multifocal but with the Synergy I don't have good distance vision. When driving looking straight ahead cars are blurry and until they get close to me I can't distinguish what type of car. When I look right to turn out of my driveway I'm concerned I wont see a car coming. When I pan my vision from left to right or right to left I can't focus on anyone thing, it all just blurs in together until I stop moving my head and focus directly on one object but then there is glare around that object. In the office with eye exam I'm 20/30. They tell me I have 20/30 but it's not clear. I tell them I can read the letters but if they wanted me to read the letters I can see clearly I could only report I can't read any of them. I'm being told I'm one in one hundred people whose eyes won't adapt to the Synergy lens. But I suspect most people are having the daytime glare, blur and distance concerns. It's concerning that before surgery we are being told we will have "clear vision" for near, intermediate and far. I was told I would have some halos around lights at night and I do have that but I believe its the design of the lens that are not only causing the halos at night but also the glare and blur during the day.

    I got SYNERGY on March 22, 2023 - and could not be happier! It is a miracle lens, continuing focus, virtually autofocus. Bright, sharp. I am planning to get one more SYNERGY on May 3, 2023.

      @@SSK-hy3gd I am thrilled! It feels like an autofocus - any distance. Now I can read small print without glasses, and drive in comfort. Now it is 20-20. Good luck!

      @@SSK-hy3gd I did not, no halos. Btw, surgery on my right eye brought my left eye to 20-20 as well, and the surgeon decided that I had no need for the second surgery.

    So I got Panoptix in my left eye 5 weeks ago, Synergy in Right eye 9 days ago. Went for Catalys Laser with ORA.
    Panoptix same evening I could read sharp on my Laptop. Night driving has been Hellish...Halos, Circles, Spiderwebs...
    With Synergy for 24 hrs blurry, cleared out. Supersharp text on smartphone, laptop etc. Night driving NO difference. When I close left eye, some blurryness with Right eye (Synergy).
    Hoping Night driving gets bit more tolerable.
    Anyone else did Panoptix and Synergy Combo? Thanks

      When you say night driving "NO difference" are you saying Synergy has the same halos, circles, and spiderwebs that are hellish?

      Hello: 'Night driving No difference' is an independent sentence therefore I am assuming both eye lens at this stage have Halos in low light. The time assumed for optimal clarity for Synergy 1-14 days and PanOptix 1-7 day therefore Synergy lens 'blurryness' this may subside. This is great! Please keep us informed.

    Are there glasses that you can wear at night to reduce the halo effect and starburst patterns from lights over your Synergy or Panoptic lenses? Also will you be able to read road signs at night with these interocular lenses?

    I had operated alcon penoptix trifocal lens in my left eye...I m happy with my far and intermediate vision...but I m not satisfied with my near vision I had saw every word shadow

    Thanks for the great information sharing. Is Panoptix with non-bluelight version? How come it looks whiter or brighter with Panoptix than Synergy? Thanks for the helpful video again!

      Correct. The Panoptix is the clear version (without the blue wavelength filter)

    What is the price different by aye , what model exactly is the Panoptix and the Synergy can you specified Thank You Dr Krad

      The right eye is a 7.5 diopter DFROOV, and the left eye is a 6.0 diopter TFAT30. Price is about the same with both (I think at the surgery centers i go to, the Synergy is about $100 more than the PanOptix)

    Thank you for taking the time to share your real life experience of your patient. Interesting... I am facing cataract surgery decisions, scheduled with Dr. for final consult with 2 August. Been putting off cataract surgery for 6 years. If you read about Synergy, it is supposed to minimize starburst/halos compared to panoptix, additionally, supposidly improvement in low light comparatively. This is all so concerning, however, must move forward, as unable to function a moment of waking hours without glasses.

      @@JeremyCouch Hello Jeremy. No, did not make decision, Synergy lens is new since June at my eye surgeon's office, they had placed about 30 pair at the time of my appointment. It was suggested that I wait a couple months and have an additional appointment to discuss what Ophthalmologist, Optometrist and patients find out about the lens experiences. I really do not like the prospect of PanOptix lens, the material can and does end up with glistening, which can be observed in the eye, my concern is "so far" the glistening that forms in the acrylic has not affected patients vision, however, PanOptix is relatively new, all things considered. With an IOL being a lifetime replacement, I am concerned about the look of glistening and if in 5,6,10 years it could affect vision. Additionally, I am significantly concerned about halo/glare, some people report problems not only at night but in other circumstances. Synergy also could give nighttime vision disruption also, however is apparently a different material and will not be subject to glistening, that is a plus and additionally supposed to be brighter vision. For me, I believe if going for multifocal lens, would be Synergy, however, not yet consulted with Dr,'s. Big switch in thinking, I have been thinking perhaps Alcon Vivity EDOF lens may be the best for me to have a better result with less night vision disruption. Apparently lights at night problems should and do diminish with brain adaptation, if night vision problems do persist, imagine that being the way vision is for lifetime. My only concern with Vivity, actually is my ONLY concern with Vivity lens is diminished contrast, as a photographer/photo editor, contrast vision is a definite concern. My thoughts about Vivity lens are seeing clearly, without specific focus zones, with limited near vision correction, may be a better lifetime vision situation than Synergy or PanOptix. What say you? What do you hear from your eye care professionals and through your research?

      @@murielandersonspychala6898 You've certainly done your research and have the same concerns as I do. Everything you said is spot on. I was hoping to be able to hold out for Alcon to change the Panoptix and Vivity to their newer material found in their new Clareon lens. It's supposed to remove the glistening concern. Apparently they will change the material, but I don't know the timeline for that. Their are also a couple lenses coming to market over the next couple of years hopefully that are fully accommodating like the LensGen and PowerVision lenses. Unfortunately I cannot wait any longer.

      @@JeremyCouch Is certain there will be advances in development! I, as possibly you have, have delayed cataract replacement lenses as long as possible... I can NOT function one single minute without my glasses. My ability to see near, intermediate and far has deterioated over the last couple of years, I could not drive without corrective lenses, can not watch television, certainly can not use computer nor function doing anything even close to arms length. Monovision contact lenses are no longer a solution, correction for distance in dominant eye and near in non dominate eye is a complete compromise in vision, can not see clearly at distance no near, so I only wear contacts for underwater photography then change to glasses several time throughout the day. I am up to +3.5 for near vision and need +1.75 for intermediate and far vision, I guess I have true hyperopic "far sighted" vision, I have slight astigmatism in eyes, nothing that needs special IOL's. Am in need of new prescription, however, is over $1000, so figure why waste that amount of money for attempt at some improvement, and completely wasted investment if having lens replacement surgery soon. I am thinking Vivity IOL would at least get me on my way to functional vision without glasses with significantly reduced halo/glare effects. I bet it will be another 24month or more till USA approval of additional lens approval. Are they using the new PanOptix material in Europe or elsewhere yet, think it is 2+ years after European market introduction for approval in USA. Maybe accelerated a bit by much of the footwork done on design and introduction will be new material only, who knows? Where are you located, what is your age, I am in Arkansas, 63.

    Great video. Just got a email from you and this video is updated one. He likes the panoptix because of the halos/starbursts. Picking one over the other is a hard decision. If you read J&J's review you would pick the synergy. Because they say that the halos/starburst are the same. Also they say that synergy is better close, intermediate and far. So I'm not sure if they are telling the true...

      All manufacturers tend to over promise and under deliver. Both lenses perform similarly but there are some small differences between them. From an optics and material perspective, the Synergy has the advantage. But I believe most people will tolerate the night time dysphotopsias (halos/starbursts) of the PanOptix better. This is just based on the patients I’ve had so far. We have also had a lot of patients with Symfony and PanOptix, and all of them say the Symfony starbursts are more bothersome that the Panoptix’s halos. But Symfony has better distance vision during the day. Synergy is like the Symfony, but with an extra near point of focus at about 13 inches.

    What are some of the best lens choices in a single vision toric lens for a +6.00 -2.50 089. Patient will wear reading glasses (+2.25) when necessary.

      For Toric Monofocals, I like the Eyhance Toric from J&J and the LAL (Light Adjustable Lens) from RxSight. But I can't tell from the refraction if they are available in the strength you would need (sphere and cylinder). You would need the biometry measurements and corneal astigmatism measurements. Those can't be extrapolated from the glasses or contact lens prescription. Eyhance has a wider range of availability relative to LAL in general.

    Got one PanOptix 3 weeks ago only my close reading distance vision is good intermediate and far vision is blurry. Why? I’m starting to get worried.

      It could be a variety of things, including residual refractive error, but I wouldn’t be able to know without testing you. I recommend asking your doctor what they think.

    Thank you for your videos, they are very helpful for ! Tomorrow i will get PanOptix in my eye. I am 23 years old and I got an eye injury a year ago, my native lens was removed. Vision before the injury was perfect :( I'm very worried and hope I can get a good quality of vision with this lens. Hello from Belarus !

    with all honesty to the doctor, but checking lenses on different eyes is kinda strange technique. One eye is always different from another in terms of brightness, light and clarity, even for people with perfect vision.

      Depends what the priority is. Every patient is different but in general, Synergy gives a closer reading distance, but PanOptix has less starbursts at night. Lens material of Synergy is a bit better

      @@aniketnavalkar5974 That's a great question. Glistenings is certainly something to consider. While only time will tell, I suspect that overall, glistenings will not have a large impact on vision in the long term for the majority of patients. Of course, if all else was the same, I would always choose a lens without glistenings. However, in the case of the Panoptix in 2021, I think when compared to its competitors, its benefits outweighs its negatives for the majority of patients. Glistenings have some, but relatively little weight on the scale when comparing pros & cons of lenses in my opinion. Thanks for your question.

    Oh no...my dr is giving me a Synergy lens in a few days, but based on this video, I think the Panoptix would be better because of better close up vision and night vision.

      Remember that this video only has 1 patient’s opinion. Not every patient will feel the same way as him. Definitely ask your doctor which one they think is better for you. Best of luck!

      @@JeremyCouch I did get the synergy lens and I am not happy with it because it has been 2 months now and my far vision is 20/40 but very blurry. I don’t feel comfortable driving with 20/40 vision. My near vision is good but not perfect...it’s also blurry. I was told I would have 20/20 vision. I also now have lots of floaters in both eyes, and occasional flashes of light plus halos at night. My dr said he will do another surgery in a few months to try to correct some issues. But he said the floaters will not go away and he said the best vision he thinks I will have is 20/25 or 20/30. So I am very upset. My dr is suppose to be the best in the country so I guess the lens is the problem. My dr said the shape of my eyes and the position of my pupils might be the problem...along with some scar tissue.

    If OS is set to a -0.5 target, the near point comparison is not accurate- over-reported in the left eye. Unless both corneas are identical, or both abortion free, the comparison on halos/glare is not reliable

    Which lens gives sparkle in eye? I had cataract surgery 2 years ago and that lens gives weird and scary look when light reflects. I would appreciate your reply

      Depending on the lens material of your lens implant, you may have a little or a lot of "sparkle" in the eye. That's the reflection of light off the surface of the lens implant that gives a shimmer or sparkle effect. The Synergy has less sparkle than the Panoptix.

    Unfair compare. His Synergy Eye has far less astigmatism than his Panoptix eye.Unless you have fully corrected this while testing. But even then the Synergy eye will perform better because it has less deviations.

      Of course both eyes are fully corrected. I thought it was clear but that's why he's wearing the trial glasses. In the video he shares which he likes better overall :)

      @@DoctorKrad Btw I liked the video, but more cases are needed. I guess both brands perform more or less equal and it depends on individual situation which one is preferred. Some will say the Panoptix is better and other like the Synergy more.
      At the end it all comes down to how good is the surgeon, how good were the measurements (getting to plano would be best of course). And are there any other things going on like PVD, floaters in the eye, how well is the cornea etc.
      E.g. If one eye has floaters and the other not probably the client will say that eye worse.

      also one month vs two months post op seems to be unfair. maybe a 6-7 months would be more apples to apples?
