I've always wondered if the original double-breasted coats actually buttoned on both sides. I like how they look, but I've never wanted to have an extra row of buttons that were merely ornamental.
Usually, the middle button will be buttoned on the inside as well. They way it's done is there being another button on the backside of the "overlapping flap" and a button hole in the layer below, though. Just that one button, though.
