Koishi has a sister who is avoided by pretty much the entire touhou cast, since again, she can read minds, so Koishi decided to not use her third eye anymore so the people won’t hate her. That’s why you can see her stabbing her eye in the video (although it the og lore she just closed it, the eye stab isn’t canon☝️🤓)

I don’t know where everybody gets the impression she shut her eye with a knife either, from what I remember it’s only stated she closed her third eye, with no further elaboration. It's more likely that she used a magic seal on it though, because stabbing wouldn't do anything. Youkai are immune to physical damage and just regenerate. The exception is that if a physical attack carries significant ritualistic or metaphorical meaning, it could work on specific kinds of youkai. Maybe she _could_ have stabbed her third eye if the act of doing so counts as meaningful, like metaphorically and literally separating her unconscious and conscious.
The fact that she has no idea that her third eye is linked to her mind makes the stab so much worse. "Head Empty" doesn't mean emotions stop going BRRRRR.
