You can easily get it with a small trick, just watch a speedrunning Video, there's usually a full explanation. Basically you just open up a specific file and edit the view count manually. Next time you start the game, the achievement triggers.
And no, it's not unfair, Valve removed the capability to get this achievement normally, so this is balancing justice.

When a medic amputator taunts (healing aoe taunt) you can hit everyone. Medics use this in medieval mode. The third degree can hit all. A single random crit on anyone in the aoe and boom
