Squat:
1. Incorrect depth
2. Knees extending past toes
3. Back arching
4. Uneven descent/ascent
Bench Press:
1. Improper grip width
2. Feet not planted
3. Butt lifting off bench
4. Elbows flaring
Deadlift:
1. Poor posture
2. Back rounding
3. Knees bending too much
4. Losing core tension
General:
1. Insufficient warm-up
2. Inadequate recovery
3. Incorrect form
4. Over/under training
