So the longer Eileen goes without taking damage, and the more room transitions you do without leaving Eileen alone, then the more she can get hit without getting possessed. Eileen can literally fall to her knees from getting hit too many times without actually getting possessed if you've built up her immunity enough. The simplest way I've found to comprehend this is to think of Eileen as a sponge that soaks up energy when around Henry. This energy fuels an invisible damage shield around her. But this energy gets sapped away when away from Henry, thus depleting her shield! Some bonus info- The submachine gun doesn't damage Eileen on All-Weapons Mode. Also, the submachine-gun won't actually effect how fast Eileen walks towards the death pit at the end (even though she'll look possessed). One more thing, if you unequip the submachine Gun, the possession it did to Eileen will be reversed. Such a weird weapon. UPDATE: If Eileen is too far compromised, door transitions will stop healing her. So if you leave her alone for too long, door transitions just won't work anymore. It is possible to heal Eileen from max possession (that would take forever) but doing so will be impossible if you kept leaving her alone. Also, keeping Eileen with you for extended periods will stagnate possession that happens upon her. So remember how I said max possession was 1.0? Well it's possible to make the max amount Eileen will hit to be 0.65 if you stay with her. I'm unsure how long you need to stay with her or how many room transitions it will take, but I'm guessing it's RNG and somewhere between 50-60 room transitions (pulled this number out of my ass). So, contrary to popular belief, leaving Eileen in the center of Forest World is a horrible idea. I was going for a Hard difficulty escape ending and Eileein went from healthy to possessed in a second once a ghost got nearby. Didn't know it was a fake out so I stopped protecting her and screwed up the run. Thanks for the tip in the description too. So much false info we had for years now...
I just want to say that this guide was sent from the heavens, and you are the absolute MVP for figuring this stuff out. On my last playthrough, Eileen was very "possessed" (she started asking about mommy and daddy, and walked with a limp). I thought my playthrough was over. Then I found this video and decided to try the "door transition heal" stuff. And holy shit, it WORKED. Not only Eileen's curse marks began fading, but the childish talk stopped and now she began walking normally. Yes, it took MANY door transitions, but I managed to get the good ending. It also helps if you are playing on en emulator since you can speed up the game, and thus do many door transitions in a very short (real life) time. I was a firm believer in the "candle" method until now. Thank you so much for this!
You are very lucky! If Eileen is too far gone, door transitions will cease to heal her. Which means you probably started healing her just in time ;) I'm glad the video was helpful^^

So I went back to the safe room and used the emulator to speed up the game. I entered and exited the door quite a lot of times, and eventually Eileen's curse marks began fading a little bit, she also began to walk normally and the baby talk stopped altogether. On the final boss, I just barely made it before Eileen went into the death sphere. Thanks again for the video, it helped me to get rid of false techniques like the candle one!
To cure Eileen permanently just enter and exit a door continuously, the recommended world is in the Room of the past because the loading times are very short, if she is very possessed just make 300 transactions from the same door, I know it's boring but it's the only way to heal her permanently.
That's great advice for players in a rut. Another thing is to reenter the room you get Eileen from in Hospital World over and over again to build up her immunity so she can buffer damage if she happens to get it.
I left her alone for whole forest, over half of water prison, and left her immediately after getting out of room 302, she was fine on normal difficulty. No possession at all.
This is so useful omg. Been wondering for years what the games exact rules are on this, but I think a big part of it is the ambiguity. They want you to feel anxiety and worry because you DONT know the rules. But this eases my anxiety. It’s a lot more lenient than I thought it was.
Finally, I've found the TRUE EILEEN GUIDE. I was able to pass all scenarios of the game without Eileen getting possessed (I even didn't know this was possible). I had to rewatch 300 times to understand this complex mechanics. Happy to know there is still news useful stuffs and discovery about SH4 after so many years. Thanks!

What Eileen needs instead of guides is a remaster that patches her possession for hard difficulty to something not rage inducing. Never managed to save her on hard because giving a shit about her is just so frustrating.
So, TL;DR the only way to heal Elieen is spamming door transitions when she isn't hit in said rooms, which also gives her hit threshold damage value. Loading screen medic meta confirmed.
New Water-Prison Twin Victim room meta: Spam door transitions with Eileen beforehand, making her practically invincible, then make a b-line dash towards the exit past the twin victims 🏃♂️ 🏃♀️
One small detail worth mentioning - each location (e.g. water prison) has a threshold of maximum heal Eileen can receive from door transitions. This threshold is getting smaller near end of the game.

I'm guessing "any method to heal her is permanent" is a typo since door transitions are the only permanent way to heal her. Either way, the answer is yes. The best method to save Eileen is to stay with her as long as possible and have her go as long as she can without taking damage- this will build up her immunity, allowing her to be left alone for longer periods of time, as well as allowing her to take more damage without getting possessed.
This is definitely good to know for my 10 star run in SH4. I'm pretty sure why she got so possessed in my last run is because I left her alone for the entire forest area 😂🤣😂🤣😂🤣... And probably all those times where I transitioned too soon when she was 1 foot too far away, and she didn't follow me through the door when I was running from monsters... I made a macro using DS4Windows and left Henry to open doors all night. Here are the features I was able to discover in the morning (for the high difficulty level): 1. Healing Eileen using the doors is possible. 2. Completely healing Eileen is not possible. 3. The sound of a dying monster is not a victory! Comments: 1. But here it's probably better to use macros to enter and exit the room and control them for a while, especially if you write them using DS4Windows. I used room 106 at the end of the game to shorten the time to get in and out of the room, it has the least amount of loading time compared to the others I tested (not all of them) 2. But the health level is enough to ensure that Eileen doesn't die in the final battle. However, the final battle should be run almost perfectly. I spent about 6 full pickaxe charges and got shot about 1-3 times. One pickaxe miss and the battle has to start again. Tested late game before picking up the umbilical cord (Henry will walk alone after that). Perhaps early in the game Eileen can be healed completely. 3. If Eileen dies between after killing the monster and before the video starts, it will count as a loss.
In my last playing Eileen was 100% possessed when I went to silent hill jungle. I wanted making her to read what was written in stones by blood. But she did not read them! Wonder why
That's not how that works. The Submachine Gun will cause Eileen to appear possessed whether or not she kills things with it, but the possession is only visual and not real (she won't walk faster at the end).
Eileen was possessed as hell for me and i did this transition trick in the room to pick up umbilical cord and it worked. Just saved me off doing another sh4 run. Thx Ps: It seems that i was below a threshold limit that prevents u from "healing" her, but in any case still worth to try.

The "door trick" works on all difficulties. There doesn't need to be any enemies in the room for her to be healed. Any door transition will heal her as long as she doesn't take damage in the previous room from Henry or enemies.
@OliverNorth-b5q I should have mentioned this in the video, but you can only tell how possessed she is once you beat Subway World 2nd Time. After the level she'll show her true possession state, and the more she pulsates red the more possessed she is. If she starts reciting ritualistic text she's around 60-80% possessed. If she starts spazzing and giving off ghost auras, she's over 80% and there's no going back. You can tell she's at full possession if her standing posture is slouching like an emo person. You can go in the same door over and over again. It also builds up her immunity, so going in the same door over and over again is a good way to prevent Eileen from taking physical damage later.

Looking at memory addresses, it's a tedious process since the address for Eileen's possession level changes with every room you leave and enter.

Have you been wearing Saint Medallions around ghosts or using the unlimited submachine gun?
I used a holly candle too right before the boss fight but she again started getting possessed.
