The Best Type of Vitamin B12: Cyanocobalamin or Methylcobalamin?

    Whoa, that video was packed to the hilt. The dosage needed to treat B12 deficiency, how oral is better than injectable, why the cheaper form (cyanocobalamin) is superior to the more expensive form (methylcobalamin), why you can’t just take it in a multivitamin because of the analogs, and you shouldn’t just take it in a pill you just swallow (because it needs to mix with that saliva protein), and then with the toothpaste thing totally coming out of left field.

    Methylcobalamin is the active form that the body can metabolize more easily. Cyanocobalamin does not take my pins and needles away and gives me heachaches. The methyl form takes pins and needles away with a few hours. I will always use Methylcobalamin until the deficiency is corrected..

      You are correct , cyanocobalamin is the cheapest and the worst of b12 , there are 2 other versions of b12 hydroxocoblalamin and adenosylcobalamin, you need to get your Intrinsic factor checked and folate , not folic ,hope this helps

      Omg it doesn’t take my pins and needles away and it gives me headaches too. We are obviously being slowly poisoned by it. Now to eliminate it from the body and chuck out those tablets.

      At 5:02 he's wrong though, they start to give 500ug instead of 100ug thats also why you see the increase... Maybe not because they chew it?
      At 4:26 Just before they mention 2 studies that show there is no difference of absorption between chewing and sublingual. Therefore this is 100% speculation, its certainly better but there's no actual proof.
      Edit: I think that Greger is right. Mixing b12 with saliva allows it to bind to the saliva's R protein, which theoretically allows a better absorption in your body.

    Another exception to using Methylcobalamin form instead of Cyanocobalamin is folks who have MTHFR gene mutations can't convert Cyanocobalamin to Methylated form. Hence Methylcobalamin, the active form must only be taken

    I'm a chemist who had been developing medicines. Any drug that is synthetically made is NOT good for human consumption: in this case cyanocobalamin is made synthetically, while methyl cobalamin is naturally made.(more expensive). Even medical professionals don't know is the side effect caused when it is used for long term. Human body is not evolved but designed so magnificently that human brain can not wholly understand the whole chains of chemical reactions in your body, even with the aid of A.I. It is mind-bogglingly complex multi-dimensional chemical reactions is in your body. Don't experiment your precious body with the puny human theory which is completely not proven!!!

      Yeah, either you're not really a chemist or you are lying for some reason. You should know that synthetic molecules are the exact same as natural ones. And what about all of the not naturally occurring medicines we have? Is aspirin not good for human consumption? No, it's fine, else we wouldn't take it if it was very bad. You also say the human body is not evolved, but it is. I'm a Christian but even I know that is a scientific fact. And you're wrong that we cannot understand chemical reactions in our bodies. If that was the case, why do we have gigantic maps of metabolic pathways? We know enough to actually do science, man. Please stop spreading misinformation.

    Thank you very much Dr. Greger for this informative video! I was confused as some said that cyanide occurs when using cyanocobalamin which is unhealthy & not safe for us to use. This video removes the question marks in my head.

    1:47 McDonald's hamburgers are also stable... for years. Cyanocobalamin contains cyanide, and is an unnatural form.

      The cyanide contained is minute it is incredibly small. By the way, lots of vegetables also have teeny weeny amounts. Your body can handle it,

      @@panes840 Yes but these vegetables are good for our health. Cyanide can be found in tobacco, I dont want more in my body even if its a bit more (I dont smoke but passive smoking happens). Of course eating animal products is even worse but I'm comparing to hydroxo for example which is stable and works great

    I read that Methylcobalamin B12 is the form existing in nature while Cyna-type is synthetic.. Has this point been discussed earlier ?

      Which form is "natural" and which form is "synthetic" is beside the point. It's about which form is best for preventing the horrible disease of B12 deficiency. And Cyanocobalamin is much better for doing that, and has no downsides for people without kidney failure.

      ​@Paul_K_ that's not entirely true considering the "cyan" in cyanocobalamin is a cyanide compound which taken over long periods is not good for you. It may metabolize more than methylcobalamin, but the methyl version has better absorbation rates oh and you aren't taking any amount of cyanide...

      This is great news!!!!!! I had always assumed that methycobalmain was superior and it worked really well in giving me focus, mental clarity and executive function. HOWEVER it breaks me out so bad I can’t leave the house. I’m talking big, swollen, painful boils all over the face. I will try cynocobalmain

    I've been watching and enjoying your talks for some years now. There only one area I'm confused with. I have been iron deficient and didn't realize till recently have pernicious anemia and for many years according to previous tests results.. I had been taking B12 methylcobalamin sublingually yet appearant not helping much.. I had bad neuropathy, serious balance problems and many more terrible side effects. After discovery of pernicious anemia I started getting B12 Cyanocobalamin injections that did NOT help in fact I continued to get worst. After I received Methylcobalamin injections I started feeling some relief.
    I found out through literature that if taking B12 sublingually for pernicious anemia the dose needs to be 20X what is recommended on the bottle to meet what is equal to an injection. I do believe that the UK recommends only Methylcobalamin for injections and not cyanocobalamin injections. I know everyone situation is different but taking synthetic cyanocobalamin would have been deadly for me. I know non synthetic B12 cost a little more yet when it comes to life or the alternative I'll take life.

      With either cyanocobalamin or methyl- form, you should be OK. What concerns Dr. Greger are the people who do not take the trouble to read about B12, itself, so they have no idea they need B12 regularly, on even a whole-food, plant-based diet. It is not a crisis if you miss a day or two, but making B12 a daily habit is a better approach. And make sure the young children, especially toddlers, get their B12 supplement daily.

      Please read the book "Becoming Vegan: The Complete Reference to Plant-Based Nutrition (Comprehensive Edition)" by Brenda Davis, RD and Vesanto Melina, MS RD for proper vegan nutrition guidance before becoming a vegan. Happy vegan journey!

      I have the Becoming Vegan Express Edition and it recommends 25-mcg-100 mcg for those over 14 yrs. I am 72, so this is quite different from Dr. Greger's recommendation. I am getting more confused lol.@@ponnamy

    Those beginning B-12 supplements should be aware that some people are allergic to cyanocobalamin ("CCB" for short), myself being one of them. I began taking the supplement at the recommended dosage (well within the limits of what's considered safe) and I began to get a pretty nasty case of hives. It took me a couple of days to become suspicious about the supplement but I finally learned about CCB allergies online. I stopped taking it and within a couple of days the rashes cleared up. I'm not suggesting people not use CCB, but just that they be aware that it could cause a reaction. if it does, there's always nutritional yeast and fortified cereals, although I imagine the amount of B-12 these have isn't enough to correct deficiencies.
    EDIT: I just learned from another of Dr. Greger's videos that not all nutritional yeast has B-12.

      It's my understanding that some of CCB breaks down to cyanide, hence the name cyanocobalamin.
      Some say that unless you are a smoker or otherwise unhealthy, it shouldn't be enough to hurt you, but for me, I think I'll pass on the CCB and choose methylcobalmin instead.

    So exactly which vitamins/minerals inhibit the uptake of B12? I take a multi with B12 as well as a B12 supplement.

      Vitamin C, and it's worse when there are also thiamine (b1) and copper. But it's only for cyano apparently and under certain circumstances...
      I dont think that thiamine and copper alone would destroy b12, especially in the hydroxo, adeno and methyl forms since these vitamins/mineral are found in animal products.

    great info, as always Dr. Greger! Thanks so much. However, I think you made an error at 2:46. The screenshot of the article you show says 1200ug of methyl is comparable to 1000ug of cyano, so cyano is only about 20% more efficacious, assuming that the methyl is not photo-depleted, which is unlikely since it is so photounstable.

    What about methylcobalamin for those with an MTHFR gene mutation? I have it and need to take methylfolate; I thought B12 was better for me as methyl, too.

      Emily, Methyl doesn't always work. I have had it fail twice. But you can always take Methyl and then eat fortified foods which have Cyano in them or take Methyl plus Adenosyl or Cyano or Hydroxo. Cyano tested best. But they did test combinations of the others taken together. Methyl fails and I have seen so many people have a Methyl fail that I would never again just take that alone.

      This is also my concern. I not only have MTHFR transcription errors, but also MTRR, CBS, COMT and MAOa. *sigh* I have experimented. I tend to over and under methylate. I have found adeno-cobalamin to be the safest for me. I absolutely cannot take cyano-cobalamin bc I get tremors and heart palpitations.

      @@mesmire I got the tremors on Methyl B12 but that was because it failed. Vitamin B12 deficiency causes tremors, too.

      @@mesmire Heart palpitations can be caused from B12 deficiency, too. But cyano rarely fails. It has the best record other than IV B12 or taking more than one type together for reversing deficiency.

    I had a level of 260, and after i found out that was too low I started supplementing daily with 1000mcg methylcobalamin sublingual. However after 10 months of this suppmementation, I tested again. And my b12 has gone down to 230. I'm very confused.

      Increase your Folate....avoid anything with folic acid in it. Folate good, folic acid-bad.

      @@mtvnbp yes now i am after understanding its more shelf stable. I take b-comp and 1 mg cyanocobalamin per day. Last time i tested it was 400, and that's when I had been supplement 1-3 mg per day for A YEAR of either methyl, adenosyl, or cyanocobalamin + b-comp and some other cofactors. I don't know why it won't increase more.

      I read a PubMed article a while ago and Methyl and Adenosyl can be taken together if you don't want to take Cyano. He said that he didn't consider Methyl or Adenosyl complete but when taken together it is more successful than just taking Methyl. I have failed with Methyl alone a few times.

    Sooo wrong. Cyno may be more stable, but so what if it is so poorly absorbed. The two coenzymed forms methylcobalimin and dibencozide are much better absorbed. The reason so many coenzymed forms suggest you dissolve on or underneath your tongue is because the more highly absorbed coenzymed form gets absorbed by passive diffusion bypassing the need for intrinsic factor further on the digestive tract. Small wonder then, when I took oral cyno my B-12 levels would hardly budge. However, after taking methyl B-12 after some time my levels were raised to high levels followed by very high levels given further / enough time.

      Plants can have B12 if they are grown in soil or water that is organically bacteria rich. ( b12 is only made by bacteria ) The duckweed experiments proved if b12 is present in the water plants will absorb it as a nutrient. Problem is b12 is not in any of the common chemical fertilizers that the plants tested are grown in at the testing labs. Fungi are absolutely full of bacteria produced b12. Especially in the common grocery store white mushrooms because those are grown in bacteria rich compost and horse manure. Most vegetables are grown in poor dead soil full of toxic herbicides to kill weeds and synthetic Fertilizers containing only N.P.K. B12 producing bacteria are not present in these soils especially not in synthetic fertilized hydroponic greenhouse grown food plants. Eat mushrooms, organic plants, kombucha tea, etc. My advice as a perfectly healthy 58 year vegan from birth never took ANY supplements nor needed to ate a stinking dead corpse for b12. There is a meat lobby pushing this b12 nonsense.

      I normally take it after breakfast, so in the morning but not on an empty stomach. They recommend taking it with food on my bottle of b12

      You're not. Methylcobalamin is retained better in the body. And about the stability, it's not like you're gonna be like "oh I'm gonna expose my methylcobalamin to light before taking it". Lol

      Took methylcobalamin since 2011, once a week and got B12 deficient (MMA tested) with severe nerve problems in my left leg, switched to cyanocobalamin and after 1 month the problems went away. My own experience with N=1 🤔

    Thanks - I'm taking methylcobalamin and adenosylcobalamin, which are the products of cyanocobalamin breakdown. These are available in sublingual form, but their formula weight is too great to be absorbed by the mucous membranes, so I simply swallow them. What's the bioavailability after they get to the liver?

      I just saw in another video on b12 from PlantChompers that taking b12 with b6 can actually increase risks if hip fractures among other things. So, yes, the b12 will not be effective but can also cause problems when taken alongside other vitamins.

The b12 I take is a sublingual liquid methylcobalamine. I don’t take it every day, usually once every 4 days. I think this is better than in a pill. I did a bunch of research at the time to figure this out. Can I remember any of the reasons now? No. I just know I came to the conclusion it was better this way. Idk.

    Cyanocobalamin makes my ears ring...exacerbates my tinnitus. I noticed it without looking for it and since then I've experimented a number of times and it correlates. I've switched back to methylcobalamin.

      There is no physiological pathway by which cyanocobalamin supplements can exacerbate tinnitus. You are describing a nocebo effect. No shame in that-your brain is just so powerful that its beliefs alter the way it processes physical sensations. If you have the budget to afford the extreme doses of methylcobalamin needed to prevent B12 deficiency after taking the instability of the chemical compound into account, then go with that. But in general, people with tinnitus will do fine with cyanocobalamin as long as they aren't convinced cyanocobalamin is harmful for people with tinnitus (it's not).

      Methyl dealt with neuropathy and tinnitus was greatly reduced for me only after the first injection. But I need another one. I must be quite deficient. I have been using hydroxy but it gives me headaches and the pins and needles are still present. Methyl for me.

    I have been dizzy lately and fighting a rather new potassium deficiency. This could be caused by keto dieting, so I am making more recipes with kale from now on. But I switched away from cyano to the methyl version to see how it works out, then I will switch back to the cyanide version to see if the dizziness and Charlie horses return. I am concerned about absorption because I had dangerously high levels of homocysteine. And I have swelling conditioning still one year after my DVT. So I don’t want a potassium deficiency to last any longer. I feel screwed on both sides of the equation

      There’s less cyanide in 1000 mcg b12 than in a handful of almonds. That level of cyanide is not an issue. Dr. Berg just gets confused (assuming he was the one raising the cyanide concern).

Calcium supplements are sooo controversial...

      Probably cause they'll make more money selling you more bottles. Then again it could be to cover all bases, like older folks who don't absorb it as well, so they might need more to get there levels on par.

      THIS !!! It is misinformation!! Lots of studies show that chewing does not change anything... This is the first time Greger made a big mistake
      EDIT: R-protein from saliva allows better absorption when bound to b12. So even if the study doesn't prove anything, its better to chew/mix with saliva.

    Question for you more knowledgeable peeps....I take a weekly sublingual B12 supplement like Dr Gregger mentions and separately I also take a Multivitamin, it has methylcobalamin but I take it more for the iodine and iron. It sounds like I should take them at different times, but maybe should I not take it at all?

    That's well and good, but I can't find a chewable cyanocobalamin vitamin that doesn't have several additives I would not wish to take all the time! It's so frustrating for me, as I have been looking for weeks now. :-( Having Ankylosing Spondylitis, I react to the most unexpected things. So taking in obscure additives can be dangerous for me. Does anyone have an answer? Please?

      That is a good topic. Methyl fails but it gives a Methyl donor and you want a Methyl donor (but not too many because that causes problems) but you can combine Methyl with the other types and I have personally had Methyl fail twice but I briefly used a Methyl/Adenosyl combination and that worked better than Methyl by itself. I don't have the gene mutation and Cyano works better and is cheaper so I am taking that now. But Methyl can be used as a combination and if you eat fortified Nutritional Yeast or fortified Plant Milk those can help. A few of the people I have interacted with who had Methyl fails didn't know it until they stopped using fortified foods and that is what happened with me. I thought Methyl was fine until I went off fortified foods.

      Not really. MTHFR is just one SNP in the folate cycle and it affects conversion folates to methyl folates. Processing of cyanocobalamin occurs through MMACHC enzyme. This is the step of concern when discussing cyanocobalamin.

      SoUL Under the tongue is a rapidly dissolving tablet, B12 is absorbed directly into the blood stream. Swallowing or chewing first goes to the stomach, digestive system, then gets absorbed into the blood, etc. Direct absorption gets to work faster.

      Your kidneys must excrete the cyanide from the cyanocobalamin. It’s a very small amount, but for failing kidneys every little bit of extra work adds up.

    B12 is best absorbed in low dosages. So why is there not a single doctor recommending 2x a day 10ug oder 3x a day 5ug? I'm taking 2x times a day 10ug (Methylcobalamin) and my blood levels are fine.

      That's a misconception. The percentage you absorb will decrease the higher the dosage, but the total amount absorbed will still be higher. So if you take a tiny dose like 2mcg you might absorb 50-60%, let's say 1mcg but if you take a 1000mcg dose you might absorb "only" 10%, but that's still 10mcg or 10X as much as your 2mcg dose (edit: that's actually a 1% absorption rate, which would be like someone with pernicious anemia, so it may actually be higher). Dr. Greger had tables that showed this in his previous video so just look at that for the exact numbers but the point is that higher dosages will lead to higher amounts absorbed.

      Dr. Gregor recommends an intake of 50ug (once a day). According to this formula your body will then absorb 1.9ug B12. Taking 2x a day 10ug, your body will absorb 2.8ug B12. So now im just wondering why nobody recommends to take 2x a day 10ug.

    Hi Dr. Greger, Please address the issue of cyanide in cyanocobalamin; there happens to be another doctor featured at RUclips who, because of the cyanide in cyanocobalamin, recommends avoiding it.
    Also, although I don’t know if B-complex would be considered a multi-vitamin, is it alright to get my B12 via that route? Does B12 interact well with other B vitamins? My B-complex happens to include vitamin C (Nature’s Truth brand). Thank you so very much Dr. Greger.

      the amount of CN- on CYANOcobalamin is so small, IIRC it's around 1% of total vitamin b12 weight. So it's only 10 mcg of CN on 1,000 mcg cyanocobalamin.

      @@OboeDaCaccia if you take natural forms then take a mix of hydroxyl, adenosyl and methyl. Or at least both methyl and adenosyl. Theoretically your body can convert methyl in adenosyl but maybe some people struggle to do it, I dont know. It looks like some people fail with methyl supplementation alone. Cyanocobalamin is not bad, its less natural but has good bioavailability.

    It has a cyanide molecule that most likely can be absorbed, however accumulative toxicity is possible when you factor in energy drinks etc. which contain sometimes 1000% of daily. Not to mention that the hydrogen cyanide that they source for the making of cyanocobalamin comes from our sewage plants. enjoy.

    B12 is produced by bacteria that make it on plants. An animal or human can eat that same plant and get enough B12 (2.4mcg). More importantly though, B12 deficiency is only a problem if you lack a chemical called the "intrinsic factor," which causers people to be unable to absorb B12. Intrinsic factor production is reduced as dietary fat increases. This is by far the most common cause of B12 deficiency. Doctors can easily test whether a person has a normal ability to absorb B12. For those who do not, NO AMOUNT OF THE VITAMIN IN THE DIET WILL HELP. THUS, ALL OF US, VEGETARIANS AND MEAT EATERS ALIKE, ARE AT RISK OF B12 DEFICIENCY.
    A high fat diet increases this risk substantially, for two reasons. First, the colonies of B12 producing bacteria in our intestines utilize carbohydrates for fuel. As the amount of fat in our diets goes up, the amount of carbohydrate goes down, thus reducing the quantity of fuel available to the microbe. Less fuel results in a smaller colony and an overall decrease in B12 production. Second, the B12 uptake sites in our intestines become clogged when there is excess fat in the diet, further reducing B12 absorption When reduced B12 production is couple with impaired absorption, the likelihood of B12 deficiency becomes predictable.
    The situation is further complicated by the fact that many refined starchy foods are "enriched" with a synthetic form of vitamin B12. When doctors test for "normal" B12 levels, their results are skewed towards the high end by the fact that most people eat these foods(mostly grain products--cereals, breads, pasta, cookies, cakes, etc) on a daily basis. People who eat a grain-free diet not supplemented by this poor imitation of the natural nutrient often test "low" for B12, even if their levels are healthy and they are totally asymptomatic. This is because their B12 levels are being compared to those of people who are consuming a B12 supplement in their food at almost every meal.
    B12 deficiency can cause anemia. Symptoms of this type of anemia include: fatigue and weakness, weight loss, and sore tongue. It can also cause a neurological disorder. Symptoms of this neurological disorder include: numbness and tingling of hands and feet, memory changes, poor balance, and depression. If you don't have all of those symptoms then you don't have B12 deficiency. You have nothing to worry about. If you want to get checked for intrinsic factor levels, that is the only sure fire way to see if you're susceptible to it. As I mentioned, if you don't have enough intrinsic factor, no amount of B12 will help. Intrinsic factor is lowered by the amount of fat eaten, so watch your fat intake and you won't have to worry about B12 and many other diseases.

      Yes, but folks lacking that intrinsic factor are a total minority. I thought the B12 in the gut isn't aborped at all, unless of course one has very poor hand hygene... So you're basically saying all keto dieters or even keto lifestylers are at risk of a b12 deficiency? Interesting, I"d like to see studies confirming that, would be funny to rub their noses in.

      Believe me “Tri-Vita” makes an awesome sublingual cyanocobalamin B-12. It’s also the only one that changed the way I feel not long after taking it.
