The Si joint is frequently overlooked as an important cause of low back pain in about 22% of the cases. It is present in about 40% of all ow back pain patients that had spinal fusion.
What is the SI joint? This is the spine. This is the sacrum which is the lower part of the spine. The pelvis is connected to the spine through the SI joints. Why is this connection important? Because the SI joint transfers the force and the load from the spine to the legs. The Si joint has very little movement; you don’t want a lot of movement. The Si joint is limited to less than four degrees pf rotation and bout 1.6 mm of translation, so the SI joint has very little movement. The cause of the pain in the SI joint cannot be due to this little movement. Some people think it is neuroplasticity when the nervous system is very sensitive to any movement and the ligaments around the SI joint is full of nerve fibers. Neuroplasticity is rewiring of the nervous system in response to injury to function differently than before. It is a maladaptive reorganization of the nervous system to a painful site which can be resolved with treatment. In general, the cause of the pain is usually difficult to determine. SI joint is protected by strong ligaments that prevent shear forces, and it is surrounded by strong muscles for stabilization during movement. The causes of SI joint problems are several. Pain from different sources overlap. The SI joint, the lumbar spine, and the hip share the same pain. The pain may be associated with one of these areas or all of these areas. In fact, more than one source of pain may exist. So the diagnosis of SI joint dysfunction can be challenging and the patient will go through a battery of steps including injection with greater than 75% in reduction of pain. The cause of the pain is usually idiopathic and multiple risk factors may exit. Usually, the idiopathic SI joint dysfunction will not show up on the x-ray, MRI, or CT. In fact, up to 25% of asymptomatic patients over 50 years old will have abnormal SI joints on x-rays.Risk factors / causes of SI joint pain? The important risk factor is previous lumbar fusion, especially when there is more than three levels involved, similar to adjacent segment disease. So if the spine is fused, then maybe the SI joint will move more and become painful. Pregnancy can cause SI joint pain. Previous trauma to the pelvis can cause SI joint pain. 80% of patients have a history of a specific trauma, usually a twisting injury. And Si joint pain can occur from getting bone graft from the pelvis and violating the SI joint. Always obtain a detailed history of trauma, infection, inflammatory disease such as Ankylosing Spondylitis, Reiter’s Syndrome, Psoriasis, check for spinal fusion, Scoliosis and leg length discrepancy. When you examine the patient, the pain is usually not in the midline, the pain is usually to the side. The pain will be below L5 level. How do you now that? Look at that pelvis. This is the level of the iliac crest and this line corresponds to the L4 level. The pain will be below the L5 level. Only 4% of patients had pain above L5. Usually, the SI joint pain is just inferior to the posterior superior iliac spine (PSIS). Treatment of the SI joint is usually non-operative treatment including anti-inflammatory medication, physical therapy and pelvic belt. Try to do at least four weeks of non-operative treatment before doing the injection. If you suspect the SI joint to be the cause or the main pain generator, then you need to identify the area of pain exactly, and that will be done by a diagnostic injection to confirm the diagnosis. The injection might give 60% success rate in pain relief at six months. If you have more than 75% reduction of the SI joint following a single injection, that means that the source of pain is the SI joint. You have to do it under fluoroscopy or ultrasound. If you do it blind, only 22% of the needles will be in the SI joint. Injection can be diagnostic and therapeutic, but do not give more than three injections in six months or four injections within one year. The radiofrequency ablation targets the lateral branches of the sacral nerve roots, but it is unable to denervate the anterior neural structures of the Si joint. Fusion is minimally invasive. You have got to have confirmation that the pain comes from the SI joint, the SI joint is the primary pain generator, and you have to have poor response from non-operative treatment. The procedure can be done minimally invasive and as an outpatient. Result has better outcome compared to conservative treatment. Arthrodesis is good option for a patient with confirmed sacroiliac joint dysfunction and poo response to non-operative treatment. SI dysfunction is a disabling problem.

I have pain over the sacrum. It acts like a severe muscle spasm keeping me stuck in limbo sometimes. Stuck between getting up and getting down. I see these videos as a massage therapist myself, I end up so confused. It is bilateral or over the sacrum. Anyone else suffer this? What body mechanics are in play while working as a massage therapist? My si joint dx was negative I guess since it did not resolve pain. Even though I told them it was on both sides they only did one side so it adds to confusion. Chiropractic adjustments kill. Like cripple me for days.

My SI joint pain is self diagnosed from reading and videos such as this. My pain started as tightness which evolved into pain which can be on either side, usually the left side. Mostly felt after getting out of bed. The pain has never been extreme, mainly a nuisance. The pain subsides with normal movement after a while. The pains started years ago. I began doing several hip exercises to strengthen associated muscles and joints. I discovered over time that the best relief comes from doing a particular stretch before getting out of bed. Lie flat back, raise the knees, rotate knees like a windshield wiper as far left as possible, maintaining upper back and shoulders flat on bed. Maintain for one minute. Now rotate the alternate direction for the other SI joint. This stretch has made a massive improvement. I no longer have pain getting out of bed or after walking. I now do this stretch every morning as maintenance. I can also painlessly get out of bed without doing the stretches. I am 69 years old.

I dont know that is safe and can work for everybody with SI joint. This isn't like a disc issue. For SI joint, I think you need a program tailored for you because my SI joint pain and its variables could be different from another person, whereas what works for disc issues on me probably will work for another person.
One thing I'm understanding is SI joint dysfunction is like a sprain to any other part of the body. Like your ankle or knee or something. And what do you do for ankle or knee sprain? You brace it. You hold it in place. Stretching ligaments or tendons will only make it worse or never heal it.
So when I think about this exercise, I think to myself the whole back needs to stay in alignment. If I'm leaving my upper back fixed and twisting my lower body and one side is stretching more than the other, not only is twisting not helpful for SI joint but not keeping the whole back flat against the bed could cause more pain and inflammation.
I am not self diagnosed. I am actually diagnosed with SI joint ligamentopathy and I have several points of tendinopathy. This isnt a textbook case of SI joint dysfunction. It's a very complex and misunderstood issue.

I’m nearly speechless here. This is me. For so many years my small town family doctor basically told me it was in my head so I stopped asking for help. It kept getting worse. Several years later while I was pregnant with my daughter (now she’s 18) there were days that I wouldn’t be able to walk up the stairs to our 3rd floor apartment and I would have to sit and scooch up on my butt cuz I just couldn’t lift my legs to get up there. Years and years of torture. I started using opioids prescribed at first but then….. well, not. 😒 I was just trying to get thru the day. The only jobs around were at the ski resort and cleaning condos. Things were not good. More than once I nearly ended my life. Finally after waving the white flag 🏳️ and moving to a different state I actually got the chance to see a pain management specialist and he actually had to convince me that it wasn’t “all in my head” that there was a name for what was happening.
I just wish that the injections didn’t hurt so bad. It was horrible. The pain of those was equal to childbirth and pancreatitis.
I never thought to look for videos about SI joint pain/dysfunction.
Sorry for unloading all that stuff on here!! But, thanks for sharing this info with others!!

Also important underlying cause - hypermobility syndromes including ehlers Danlos syndrome, Marfan's. Look for marfanoid body type, check Beighton score (don't just ask if patient is hypermobile as many don't realise their joint range is abnormal) . Check for pelvic floor issues eg prolapse, rectocele, evacuation difficulties, incl in people who haven't had a pregnancy (could contribute to pain & good clue to hypermobility esp if no pregnancy to explain!). Fatigue maybe related to PoTs. Hydrotherapy maybe be more effective than standard physio if joints are especially unstable due to ligament laxity.

The burning in those joints and my hip/ leg joints is so intense its unbearable. I had a Lumbar MRI recently that found 3 bulged discs and arthritis. I had the lumbar Steroidal Epidural injection and it didn't help at all. They injected at l4-l5 and l5-S1. Ive been really losing hope lately and wondering what this is and how I'm gonna continue living like this. Im only 39 yrs old too so all of this is strange for my age i think. Hi I am 78 now and have just got to understand so much more from this video . I had a trauma to my spine 33 yrs ago which has left me with chronic pain since. I know they didn't have the knowledge that they do now so it has been only presently that I have a clear picture of things. Getting off opioids would be marvelous so I'm wondering after obviously having had unsuccessful injections that they may have been put into the wrong places. Now that lately a new MRI has pinpointed the pinched nerve at the Si joint and that an injection may help the buttock pain, I will still be left with the other pain problems.
. Your video gives me the impression that all of the symptoms are coming from the S I joint. That picture of the red spot and the red streeks and the leg knee and foot have given a good picture of my situation and case explained. I can have the bone released to free the nerve but no guarantees. Doing exercises and pool Walking.😊

My physical therapist at the time treated me for back pain and didn't do the SI joint. Fast forward to me wearing a knee brace and now my pain has decreased tremendously! 3 orthopedic doctors looked at me with confusion as to why i had SOO much pain with sitting and low back pain. When you're young and in shape nobody will listen to your complaints about back pain, and it has taken me years of complaining and going to different doctors and ultimately being forced into workers' compensation and finally the doctor mentioned my sacroiliac joint. Looking this up there is no doubt in my mind that that's what it is and I'm kind of irate because it seems like with a few pointed questions and a thorough examination this could have been easily diagnosed. Makes me think doctors are clocking into their 9 to 5:00 just like everybody else. Ask to be tested for the HLA b27… if this sacrolitis is linked to reactive arthritis, or Ankylosing Spondylitis… you can also lose your eye sight due to Uveitis. Drs do not talk to one another, I had to learn this and solve my husband’s problems. Good luck ❤

Probably the most helpful and the most validating video I have seen. I have gone to my doctor's and have felt defeated every time and not being able to sleep has made me feel a bit crazy too. I think sometimes doctors forget to communicate properly. "EMG looks Normal, Unsure why you are having so much pain and discomfort. We can do injections to see if that will help" I was here thinking why the hell am I going to do a freaking injection if you don't know what the issue is?! Maybe now I can direct my questions better. If it's for a diagnostic purpose for sacroiliac joint this makes sense. I had fusion of S1-L4. Surgery was good, most of my pain went away but a new pain crept up months after surgery. A year later I still have trouble sleeping in any position especially laying on my back and I still can't feel my left glute.

Great, clear description of this painful issue. Specialists have been shooting me in the wrong areas. But my regular doctor shot my SI joint free handed and hit the spot. I was able to take a long plane ride after that injection and spent about 2 weeks pain free. I'm going back to him.
    Thanks.

I was already a retired professional ballet dancer and had born two large babies so obviously my back was an issue when in 2012 i had a major back sprain due to a weighted twist while bending over. I assumed it was a soft tissue injury since it was more toward the left side. I couldnt walk unsupported for at least a week but i never went to an Orthopedic. It is only recently that info such as this video has been available and it has opened my eyes. Thank you for this. Thank you so much for this very informative video. I don't see why this pain affects me, as I don't have any of those issues you mentioned. I suffer bouts of it a few times a year, and it takes about 2 weeks to go away each time. I'm currently on Day 10, and I'm still in a lot of pain, hard to sit, go from sitting or lying down to standing straight (hunched over is usually all I can do for the first few days), hard to find a comfortable sleeping position, and walking also hurts. My pain is in the lower back, as well as all around the front - groin, pelvis, hip bone, and outside of upper thigh - often both sides, sometimes just one. It's like a wide belt of pain that I'm wearing. It's extremely disabling, but thankfully, I work from home. I'm surviving on Aspercreme with Lidocaine and a pelvic belt. I feel like Advil does nothing, though I do take it sometimes just in case. Just walking to get the mail is extremely difficult. Btw, my pain is also self-diagnosed based on all the research I have done. The one time I saw a doctor was just for hip pain by itself, and he found nothing.

This is the best information as to what I am dealing with I have ever found. I have had xrays of my back 10 years ago because at first I thought it was a sciatic nerve issue. I just had xrays and an mri done of my hip because I have started to feel this really bad in my groin, and I also have a pop and a snap. I also have pain in my lower back below L5 and not centerline. It hurts to bend over and bend up, I could hardly get out of bed the other day, it hurts to sit down, stand up, or lay down. However I finally have insurance and was able to isolate the pain instead of the entire region just being in pain, and this is exactly where it's coming from. I start physical therapy tomorrow and I am going to tell them this is the treatment I need and want moving forward. It's been very depressing to deal with this because every-time I have attempted to get it sorted I was always told I was fine but knowing in reality I was in excruciating pain. I will let you all know what happens, and let you all know how physical therapy and further treatments go. Keep you head up, you're not alone. Thank you Dr. for this video. You saved my life.

They said nothing would help but the shot & no other thing could diagnose me, like MRI. I switched to HMSA & am trying to take care of it on my own with a soft tissue specialist (who's also a chiropractor), and many of my most painful symptoms have resolved. I am also trying to do PT on my own. Kaiser PT was awful, though I did not do for SI joint. After watching this, I'm not sure if the issue is my SI joint. I have more exploration to do. But I have many indicators that it is SI - like the general location, pain when sleeping/standing, etc. This was so helpful compared to my docs. Thank you!!!

I haven’t been able to walk without a cane for two years now and I’m having bladder incontinence. They say I have normal nerves and spine imaging. Im about to go to an ortho to seek a diagnosis cuz I think it’s a joint problem. I don’t know why no doctors are suggesting joint issues they just kept referring me to a neurologist which was a dead end. Hopefully I get diagnosed soon this is causing me so much disability and financial stress along with the emotional stress of being home bound.

This is so informative and for the first time I understand why I have been in such pain .
It started in pregnancy (3 and a half years ago) I was diagnosed with common pelvic girdle pain and I was told by my gp that this can last for 1-2 years post-partum however the pain only became chronic and didn't improve , it just kept returning depending on how active/inactive I was . Sometimes If i only carroed my toddler for 5 minutes it would trigger off excruciating pain in my SI joint.
Holidays abroad also would lead to the same doomed pain along with many other life activities.
I am currently seeing Chiropractor 3 times a week for treatment and I'm starting to feel like myself again.
I'm hoping the Chiropractic treatment will be all I need to heal instead of taking the injection.

I had si joint dysfunction and a chiropractor gave me a Serola SI Belt which I used for 2mo or so and it fixed the problem. I remember saying to the chiropractor I had pain in my ribs and bam he immediately knew the SI j problem. So for $40 a belt which would give immediate relief but needs to be used for a while to correct the problem seems like a plus-plus. Even if it doesn't work then no great loss. Also I was at 280# when the issue arose so the added weight caused the problem no doubt. I remember ascending a steep hill when it happened. The joint was probably comprised from falling hard on my buttocks years prior.

I've just looked into this and thinking about getting one. How often did you need to wear it?

Crystal, waking hours. Whenever you're up and about moving. I have one. It worked immediately. But after about a year the pain started to return. My job involved walking, standing, bending and lifting all day. I think that job caused severe inflammation. I'm seeing chiropractor now, it's helping. Not at that job anymore. I'm going to look for my SI belt.

@@suerop What does the s1 belt mean, and where is it located? I would be grateful to you

That’s interesting about the rib pain you mentioned. I’m having difficulty walking and have pain in my legs and hip sometimes if I step to hard it burns horribly. But one of my other symptoms included joint weakness in my arms and my torso around my rib area fees stiff and painful if I lay down flat for too long. They keep saying it might be nerve compression but all imaging came back normal. Is that what you mean by rib pain? I still have no diagnosis.

@user-sq8jg8lf1z I hate it when people don't respond to such an important question. Serola si joint belt. Do a Google search. You were just below your hips. Best wishes.
