Lesson from this clip... "Perfection is enemy of good" .... ❤ Few suggestions won't be out of place.... 1. Reduce the opening of palpabral aperture by loosening the speculum screw. (I believe it is not wire speculum but adjustable one) 2. Though AC is near flat, introducing visco at wound forces aqueous to go in periphery. It is repeatedly causing high iop. This is well seen in video. Actually aqueous should leak out while introducing visco. 3. While coming out of phako tip always stop the fluid by asking rotating person to pinch irrigation from bottle. Stopping the fluid by foot switch causes regurgitation and raised iop. Once you did it while coming out of I/A TIP. While coming out with phako handpiece at wound be there for a couple of seconds when irrigation line is closed. By that floppy iris or in your case floppy capsule will not happen. You were on border line of surgery went well to you performed surgery well. But I would definitely say your patient is in safe and best hands. Would love to see other eye also.
