I’m impressed…you spent more time polishing than my whole procedure…and I was a detached retina patient from 1978 and cataract surgery in 2018. I thought you did a wonderful job doctor…all the best sir!
It’s just not worth the risk, do your bit and rely on YAGing later instead of pushing too much. A fair chance the patient’s gonna need a YAG later, still.
Agree just yag it. It costs $40-$50/minute to run the OR and you’re more than likely going to YAG it later anyway. Plus you’re tempting fate by playing with the capsule so much. You’re getting no benefit adding risk and increasing cost by polishing so much.
Had surgery on my left eye last week, in my own country, the Netherlands. I can't find these kinds of videos anywhere here. What an incredibly friendly doctor in this film! 👍🤗 I wish I had such a relaxed doctor!
Fantastic! I’m in private practice and do pretty much the exact same steps as you :D. Divide and conquer with the drysdale as my second instrument. Except I have really long legs so I prefer sitting superiorly. Great case Dr Krad
From my experience, thats probably the worst technique for newbies and also for experienced surgeons. So many cataratas wont break and will make a bowl, risking a PC tear if you insist trying to make it thinner. I did stop and chop for years. I'm just 3 weeks into a high volume cataract surgery practice and discovered by myself the most efficient and safe technique is luxating to anterior chamber just half lens during hydrodissection so it gets in a vertical position. You can easily then eat 1/4 of it just in safe position (phaco probe in the middle of the capsulorrexis area and in the pupil plane. Then flip it back to the capsule and follow a chopping technique. The 1/4 you already ate helps you a lot moving the cataract and giving you space to chop. I use a fully blunt instrument to chop (not a chopper really), so I can put it below the cataract while I use phaco so capsule wont ever come up, theres no chance to break it unless you mess in these instructions. This technique works great for every single cataract unless with those which are very hard and may be risky to luxate to anterior chamber. In those cases is easy to do stop&chop (1 central groove, break, then chop). Thats my better advice. Technique used by Dr. Krad despite being valid, is risky, slow, unpredictable (depending on cataract consistency) and very frustrating for even good surgeons. No point in using it. Hope this helps.
. Not, I have had three surgeries for my RD but it couldn't save the detachment, then there came a point I was fed up with the pain and agony it was causing, so I have lost hope of regaining my vision and finally accepted it, that was the hardest part, to accept it and move on with nuclear vision at the age of 26, then it's life, sometimes you have to come into terms with life at some point of your time and face the facts. That's what I did, face the reality and move on and here am, peace with myself and my entire life, it gave me the strength to carry on. 😊
Oh my God. That was terrifying. Your RUclips video just popped up and I watched. I had retina detachments in both eyes. In my right eye in 2017. Lens replacements both eyes in January 2018 and then retinal detachment in March 2018 in my left eye. Prior to this I wore glasses ranging from minus 7 to minus 11 over the years. In 2024 I had yag laser for my right eye and probably will need yag laser next year. My eye pressure is unstable, usually near or over 20 in both eyes I am not diabetic but have a very sweet tooth. Since July 2024 I have reduced my sugar intake by about 90% and since then my eye pressure has reduced to 10 and 12 That is the lowest since 2017. I do use eye drops to control the pressure, but I think the high sugar intake was effecting the pressure. The reason I commented was on how brave that patient was Thank God I opted for full anaesthetic for all retinal and lens surgery The yag laser treatment was a breeze but the thought of going through that surgery without full anaesthetic fills me with dread
I had this done 5 days ago.I had a detached retina in Feb which took an hour to repair.I developed cateract i was awake through both surgeries. The second operation the surgeon was quite rough i thought.Im happy it's done but it's an amazing thing to be awake for
I have a cataract in my left eye and so I wanted to watch this video and see what the surgery is all about... I don't need surgery right now but this was very interesting because the whole thing only took about 10 minutes... That's pretty amazing..
I'm curious about how the patient feels when his eyes are cleaned, even though I know that before doing the operation he needs to use antibiotics and that it's done when the patient is conscious, it's really extraordinary
I wonder about the initial concept of this surgery. I wonder how many years under study before this was attempted on a live person. What was the outcome, complications, etc. Thank God for the Incredible minds, dedication and courage to those who made this happen and continue to improve!!
❤❤❤ I lost my left eye @ 26 due to a perforated injury, now 62 looking back did pretty well in life even with nuclear vision for so long time, God's great, Love Eye Surgeons, they really care. ❤
Great video. I know the techniques in cataract surgery. I had cataract surgery in both eyes at the age of 17, according to the doctor, cataracts are congenital and rare. Thank God, now I can see clearly even if I wear glasses because ordinary lenses have been implanted. In my city there is still no device for detecting nearsightedness in conditions of cataracts.
I had Retina detachment surgery on August 6th this year it went well but now my surgeon advised me that about 3 months i will have to have surgery because i got cataract after the surgery.
Wow this was really cool to watch.. I had heard that the cataract surgery basically involved getting up underneath that flap and using the instrument to break up the cataract which also acts as a vacuum and sucks all the cataract up into that tube out the eye.. I had a retinal tear in 2016 but have not had any advanced cataract as a result.. I do have cataracts slowly forming but nothing terribly impeding at this point. Part of me looks forward to having the cataract surgery so that I don't have to wear glasses for distance anymore.. but not thrilled about giving up my close-up vision which is really good right now and I don't have to use any type of readers. I know that there's an option for a multifocal lenses but I used to do multifocal contact lenses and I've also done where just one lens for distance vision in one eye and no correction in the other eye.. it works good during the day but that's about it. Can I just have some brand new eyeballs please😅😅😅
I had cataracts in both eyes. But my surgeon could only fix 1 a year ago due to also having a detached retina. I missed appointments for follow-up after getting the ok from the retina doctor to have the cataract surgery due to ill-timed covid bouts, twice. FINALLY got back in to see my eye doctor and he couldn't set it up that day because the eye he had done before had a detached retina and I needed THAT taken care of. The office called back after talking with the retina guy and making sure I was able to get by with just the laser repair. So TODAY I had the above.
Was the surgery successfil? I had cataract surgery on my left eye firt ,last November n im still not seeing clearly , went last week to get a secundaria opinión as my surgen said that thr was swelling behind the eye n i need to take injections , im diabetes but he said once i can keep it under control n swelling goes down i wont need the injections ..now my visión is en more blurry than b4 so went for a secundaria opinión coz i really dont want to take injections n the 2nd dr told me thrs nothing can be done for that eye ..so yesterday tuesday 3rd sept.qent back for appointment at the eye hospital i did surgery saw the head surgen n after examinaron he said thrs retinadetachment n i need to do surgery but thrs an 8/2 % chance that i may still lose vision , n if i dont do surgery i can lose visión so im processing it. I was told that only when doing surgery he will k if to use gas or silicone oil in the eye , researched it n found varying reviews . I have no bleeding or glocumo n with the oil i may develop those n other complicaciones. I really dontk what to do ..any advice pls?( Sorry if thrs typos as i cant so well .)
You showed all the process mainly the removal of cotaract. But you didnt show the end up process, that how you finished the operation.I was interested to see the concluded part of surgery.
Hey Doc being a biology student and as well as a teacher , I can't ever imagine such kind of surgery , wow Guys God bless you
I’m impressed…you spent more time polishing than my whole procedure…and I was a detached retina patient from 1978 and cataract surgery in 2018. I thought you did a wonderful job doctor…all the best sir!
It's rare to find cataract surgeons who spend more time cleaning the capsule than performing the phacoemulsification. Fantastic!
Just my thought too.. Kudos to them..❤❤
