The medium is where you went stealthy and also killed people. I'm not saying there is one but it depends on how you played it If you kill, lets say...7 people and went stealthy the rest of each level doing that same thing...it'll turn out to be high chaos because you killed many people in rapid succession. There is a "medium" ending but it just depends on how and what you did in your playthrough. I hope I helped you and corrected myself ^-^
