Pediatric SARS-CoV-2: Clinical Presentation, Infectivity, and Immune Responses
    Massachusetts General Hospital
    www.jpeds.com/...
    Background
Asymptomatic carriers, including children, can spread infection and carry virus into their household
    Objectives
Potential role children play in the coronavirus infectious disease 2019 (COVID-19) pandemic
Factors that drive severe illness in children
    Study design
    0-22 years
    With suspected infection
Presenting to urgent care clinics or being hospitalized
    For confirmed/suspected SARS-CoV-2 infection
Or multisystem inflammatory syndrome in children (MIS-C)
    Enrolled provided
    Nasopharyngeal, oropharyngeal, and/or blood specimens
    SARS-CoV-2 viral load
    Serology
    ACE2 RNA levels
    Results
    192 children
Forty-nine children (26%) were diagnosed with acute SARS-CoV-2 infection
An additional 18 children (9%) met criteria for MIS-C
Nasopharyngeal viral load was highest in children in the first 2 days of symptoms
Some children carry very high viral loads even before symptoms develop
Viral load in respiratory secretions of children was high, despite mild or absent symptoms
Significantly higher than hospitalized adults with severe disease (P = .002)
Age of child / young person did not impact viral load
Younger children had lower ACE2 expression (P=0.004)
    Source of children’s infection
Nine (18%) did not have a known infected household contact
    26 (53%) attended grade school.
    Presentations
SARS-CoV-2 infection and non-COVID-19-related illnesses presented similarly
    In the positive group
    25 (51%) presented with fever
    Cough 23, (47%)
    Congestion 17 (35%)
    Rhinorrhea 14, 29%)
    Headache 13, 27%)
None of which were significantly different between the two groups
    Significant difference
    Anosmia 10, 20%
    Sore throat 17, 35%
COVID - 19 symptom tracker app
    covid.joinzoe....
Rash in up to 20% of cases
    Conclusion
Children may be a potential source of contagion in the SARS-CoV-2 pandemic
Contradict previous reports, children to be less likely to be the index case within a household
Children with high viral loads and non-specific symptoms including rhinorrhea and cough can likely transmit SARS-CoV-2 as easily as other viral infections spread by respiratory particles
If schools were to re-open fully without necessary precautions, it is likely that children will play a larger role in this pandemic
Potential transmission between children and families should be considered when designing strategies to mitigate the pandemic
In spite of milder disease or lack of symptoms
Immune dysregulation is implicated in severe post-infectious MIS-C
    Hypothesis
ACE2 expression in the nasopharynx increases with age
    Multisystem inflammatory syndrome (MIS-C)
IgM and IgG to the receptor binding domain spike protein were increased in severe MIS-C (P less than 0.001)
In severe MIS-C, more often a broadly elevated IgG response
    To a multitude of respiratory viruses, including;
    Other coronaviruses, 229E, NL63, HKU1, and OC43
    Respiratory Syncytial Virus
    Influenza
Several weeks after possible SARS-CoV-2 infection or exposure
    MIS-C presented more often with
Viral load the same as other infected children
    Fever
    Nausea/vomiting
    Rash
Less often with symptoms of an upper respiratory tract infection
    Criteria for COVID - 19 MIS-C
Fever more than 38oC for more than 24 hours
    Laboratory evidence of inflammation
    At least two organs involved
No alternative plausible diagnoses and a
    Positive serology or antigen test
Exposure to an individual with COVID-19 within 4 weeks prior to the onset of symptoms
Severe cardiac complications, including hypotension, shock, and acute heart failure
Understanding post-infectious immune responses in pediatric is critical for designing treatment and prevention strategies

Apparently it's news to WHO. Unless it is a hoax, apparently the WHO just released a statement that children under age 12 should not be required to wear a mask. Or specifically, children age 12 and above are required to wear a mask while those under age 5 are not. The statement I saw in Indonesian social media didn't mention ages K-6.

Kids have a much higher metabolic rate so that infections are generally killed off before they can get sick.
They also have very acute immune systems which they need to build up resistance to all the small and large infections that we all take for granted as adults.
It's not completely understood how they are so good at fighting diseases.
When I was young people deliberately made sure we all had the common diseases at a young age as we could deal with it better.
This was pre most of the current vaccines.

Philip Lindley nothing to do with metabolic rate. How can that have an effect?
They have a very sensitive and reactive immune system, older you get the less responsive your immune system is.
That’s why youngsters are more susceptible to sepsis.

    • @richplant2205

Followers of this channel and others like Medcram already know all about this. WHO (I remember interviews with Dr Aylwood) greatly downplayed asymptomatic spread. However, Chinese CDC published a paper in Jan/Feb, that they thought it was as high as 50% and classified it as super spreaders. But, then, other hand, its why you do have some people able to shrug off covid19 since they already had exposure to similar past viruses.

The relaxed approach or mitigation policies of the US and most of Europe doesn't really have any opposition. Same can be said in containment countries in the Asian region. Different policies leading to currently very different outcomes. The countries with very large populations and lack of resources to respond effectively in containment haven't succeeded with containment but on a per capita basis have less deaths etc. than the alternative policy by far better of countries with plenty of resources.

I taught kindergarten for 20 years, and spent the rest of my 34 year teaching career in grades 2-8. I got sick two or three times a year with various upper respiratory issues and stomach virus illnesses while teaching kindergarten. The rest of my career I only got mild sinus issues and no stomach virus symptoms. I attributed it to closer contact with the little ones, and also older children wake up and know they are sick whereas a younger one is less likely to say they don’t feel well until they feel very sick, and have been in the classroom for several hours. Often times I would notice they looked sick and the check their temp and realize it was rather high. Now you have taught me that another factor could have been a greater viral load as well.

In Finland, our "NHS" did a study with a sample size of 1, that they thought serves as evidence that children don't spread COVID-19. This particular kid just happened to be a zero-spreader. The whole study was just made for the purpose to make it seem like opening schools is safe. Also, masks are only recommended from 15 years up! And to make matters even worse, airborne transmission is not considered to be a real thing. Then importance of aerosol transmission is down-played, instead of focusing on droplet and fomite transmission. So there is no extra ventilation or air purifiers in schools. The aerosols are thought to only be formed by coughing and sneezing. Not from talking etc. Also there is not enough room for social distancing.
While we have one of the best COVID-19 situations in the whole EU area at the moment, I fear that we are going to have severe problems as the schools are opening almost to the old normal. Time will tell, I would like to be wrong with this one, but too many times already, I've been right and the "experts" in our health services have been wrong...

However the hyper hysteria of the internet era and politicians and the political scientists forcing their narrative for power relevancy are fueling irrational fear and ignorance among the mass population at a rate never before seen in the history of the world. The most obvious concept and correlational that really should need no explanation is, if there is an exponential Increase in testing of course they will find corona virus everywhere, dig a little deeper and they would also find flu virus and other viral infections far more common and rampant than anyone could ever imagine. Corona viruses are among the most common virus on the planet. There are currently 7 corona virus infections circulating the world that are hiding dormant inside a huge portion of the global population right now as everyone is panicking about this one. These types of viruses are extremely contagious and ubiquitous, in other words absolutely everywhere and impossible to contain. However the good news is, Corona/flu type viruses have a very low level virulence rate for the vast, vast majority of the population with normal functioning immune systems. In fact it is so low our front line defender White Blood Cells snuff the virus at the onset of the infection before any symptoms are even detectable, aprox 80-90% of the time. Acquired antibody T-Cell immunity (or heard immunity) is the only way these types of viral infections ever end (by virulence dilution causing what is known as an endemic) and not by masks, hand sanitizer and vaccination. ruclips.net/video/FU3OibcindQ/видео.html

The aim now is to 'trickle' the viral infections so the hospitals can cope, rather than aiming for low or no infections. Unfortunately, keeping infection rates to a trickle will likely be impossible with kids going back to school for winter... winter was always going to be a big problem with this virus because we're all indoors with largely closed windows and the heating on, so I strongly suspect that,at some point during late autumn and winter, schools may need to be closed again so hospitals don't have to turn away seriously sick patients.

    1) They compared adults in intensive care and compared virus levels in upper respiratory tract. But by the time someone is in intensive care, the viral load has typically dropped, and their illness is now more driven by lower respiratory infection, and by damage resulting from the virus itself. They weren't comparing with children in intensive care, so it's apples and oranges.
    2) The children's age range was from 0 to 22. I don't count 22 year olds as children. In fact from puberty onwards children should be consider the same as adults for these purposes.

It was ok when we went into the school for reviews recently as only a couple of people in a room and windows and drs were open for ventilation, but the weather was warm. I can’t see how they will keep classrooms ventilated when it gets cold 😟. The government are not on top with testing, tracking and tracing. They allowed the virus to get too much out of control.

A friend of mine has only just been able to get an appointment with an emergency dentist, because she was in pain. Nothing to do with mask wearing 🤣. The message headlines, is that there is a very low risk of severe illness from COVID in children (I cannot argue with that). However, very silent on the risk of children spreading infection to others. ......... At least it won’t take long for the implications of this to take effect (or not) in the community. The danger is that schools link multiple cohorts together. Each family is a cohort but a working parent links this cohort to co-workers which again link with their families and of course children's at schools do the same but across multiple professions and as some professions are more at risk than others, greatly increase their risks. College age children are a more isolated but rather large cohort group and often bring the disease through part time jobs to service industries. I'm surprised no one has thought of collecting the parent employments and locations of employment to assess and limit risk in dividing the cohorts at schools. The students of parents that work at a given factory or meat-packing plant are obviously best separated from other cohorts etc. Such classifications could be seen as class discrimination but hey this is a pandemic that we're fighting...

Sensible idea. For those under exam years, no reason you can't keep the children on the same bus/area in the same class. Harder to apply to exam years as they have already chosen subjects, and are moving around the corner.
Alternatively, teach bus children (under the age of 14) locally at community centers, Church halls, sports halls, etc. Use every substitute teacher/qualified teacher you can lay your hands on. You've automatically achieved social distancing in secondary schools. Work can still be set by the school for the sub to follow, and get each year group in one day a week for practical subjects.
Those 14+ are working towards GCSE, A Levels, Btec, etc so should attend, in smaller classes.

    • @johnwang9914
      @johnwang9914 4 года назад +2

@@sharonsloan Presumably, the older students with variable schedules would require greater time between classes and staged release and entry to spread out the load in the hallways which probably means altering the schedules or adding time to the day. If the students requests are early enough, computers could sort out the schedule to optimize the traffic in the hallways. To a certain extent, such students being older can assume more of the responsibility to wear masks and proceed in designated lanes with social distancing. No solution will be perfect, the question is what measures will make the public feel that enough measures has been taken. Of course, regardless of what's done, there will be some spread of the disease at schools and this may adversely affect community spread and we have to be prepared for that with testing, and contact tracing.

    • @sharonsloan
      @sharonsloan 4 года назад

@@johnwang9914 the problem with those years working for GCSE and A Levels are shared education, two schools sharing resources, so could be in one school and class cohort, then a different class cohort for a different subject. Staggered classes will sort the traffic in hallways, but you can't be in the same cohort all day if going to three or four different cohorts over two buildings for different subjects. Masks probably will be worn in corridors, but effectively the bubble is year group sized, not class sizes for those 14+ due to how the system works.
Normally good, as sharing resources between schools doubles choice of subjects. Now though, not so good.

    • @johnwang9914
      @johnwang9914 4 года назад +2

@@sharonsloan Yes, the greatest danger if returning to schools are that cohorts are being linked. Even with just elementary schools where the children remain in one class and are taught by one teacher, it is still linking classes to families and to the professions of the parents. Yes, there are progressively more cohort linkage as students choose their own schedule and between schools as resources are shared but that doesn't mean we can't limit the cohort sharing simply by including additional information from outside the schools such as where the parents work. If a student uses another schools resources hence linking a cohort at one school with a cohort at another, had the students in both of these cohorts had parents working at a given factory then they were already linked to begin with so really linking them on the education front should not increase the risk as much. Yes, all this linkage is bad, I'm just saying that by increasing the information available, the cohort selections could then minimize the linkage of cohorts to overlapping linkages hence no real increase in linkage. It does require more personal information be made public so there are societal risks in this optimization but we are talking about trying to make a bad situation better. The only safe decision from the perspective of COVID would be to remain with distance learning but were we to accept the risks of reopening schools, it's about how to reduce the risk not about making it absolutely safe as that can only be done with distance learning.

Yet schools here in the Netherlands reopen like normal, with 1.5m from teachers, stricter hygiene, coughing and sneezing in the elbow, hand sanatize upon entry and some even use facemasks.
Aerosoles are discussed and proven has been that 'children' of 15+ can still infect adults as much as adults eachother, between them they dont have to keep 1.5m because of it being so mild and 1.5m from adults must be kept.
Edit: ventilation will be a must thing tho, otherwise the RIVM says reopening won't be smart or safe enough.
