How I Ended Up In The Emergency Room Last Weekend!

  • This is the story behind my deteriorating health this year, culminating in a trip to the emergency room on July 18th. It turns out that the severe pain in the muscles throughout my entire body was due to a condition known as Polymyalgia Rheumatica.
    The pain level steadily increased over the week of July 12th, and when it finally maxed out at a 10-on-a-10-scale level of pain, I realized that I wasn't just going to be able to wait for my Primary Care Physician to return from vacation and I was going to have to go in to the E.R.
    Normally, Polymyalgia Rheumatica, often abbreviated as PMR, is treated by a Rheumatologist rather than a general practitioner. However, in my local area there are only two Rheumatologists and neither is currently accepting new patients.
    Polymyalgia Rheumatica takes its name from the Greek word Πολυμυαλγία polymyalgia, which means "pain in many muscles". While it is one of the most painful medical conditions you can get, it's actually fairly easy to treat. Prednisone, a synthetic glucocorticoid used for its anti-inflammatory and immunosuppressive properties, is effective in treating nearly 100% of PMR cases. Relief can begin in as soon as a few hours after the first dose, and an almost total elimination of symptoms often occurs within a day or two when treated with Prednisone.
    This video tells my personal story with Polymyalgia Rheumatica, from the beginning of symptoms in the Spring of 2020, to the painful escalation of the pain in July, and the successful termination of the pain after treatment with Prednisone on July 18th.
    A simple blood test can usually reveal clues that a patient experiencing strong pain might be experiencing Polymyalgia Rheumatica. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) levels will almost always be elevated in PMR patients. A normal CRP level ranges between 0 and .5 mg/dl. Mine was measured at 12.61 in the emergency room. A normal ESR ranges between 0 and 20 mm/hr. Mine was at 90.
    I'd like to clarify one point I bungled a bit in my video. In talking about my weight, I think a lot of people would interpret my comments to mean that my weight GAIN was a subtle symptom of my illness. I didn't mean to make it sound that way. My weight gain happened a long time ago and had nothing to do with Polymyalgia Rheumatica. What I was trying to say, but I just bungled it, was that if you notice how large I was in my videos up until this summer, and then notice in my videos from the last week or two that I was looking thinner... you'll see I've been losing weight lately, and that weight loss was a subtle clue that my health had not been good. As of now, I've lost about 20 pounds from my peak weight... and I'm not wearing my skinny jeans!

    I went through those very same symptoms for a few weeks because my PC doctor didn't know what was wrong with me. Then I went to a rheumatologist and was diagnosed with PMR that same day. I started on a low dose (10 mg) which decreased the pain about 40%. I finally had to increase to 15 mg to get better pain relief. My dilemma is I'm diabetic and the prednisone is causing many other problems internally which scares me so much, but I can't function with the pain I felt while not taking the prednisone.I just pray that God will get me through this without too many other issues.     Gosh Jim it looks like you were in an awful condition, a neighbor of mine has this same issue and she was driving home and had to pull over down the street and opened the car door and screamed in pain like you we couldn't get her out of the car she was in absolute agony. You are lucky to have a great wife to take care of you and support you getting better.

    I'm so relieved you're feeling better, Jim. Be careful with those NSAIDS. They gave me a bleeding ulcer. Just be sure you don't take any on an empty stomach.

      ha ha ha... yeah, the porch! At our California house, we really enjoy an outdoor patio lifestyle on the days it is fog-free outside. So, in designing the Washington house, we wanted to go all-out on the back yard. Because of the lack of summer fog, I'm pretty sure we're going to have at least 30 more days in a year where the weather will support us being out on that porch, compared to our California house.
      And I know the back yard doesn't look like much more than a dirt and rock patch in the videos at this point, but in my mind you would be amazed at what I am envisioning. You can see in today's video what I've done with our back yard in California. Now imagine that times about ten, and that's what I've got in mind for Washington. But, the landscaping is going to take years to come to fruition. The porch is fully ready, though! One of our first purchases will be an outdoor dining table for breakfasts, lunches, and dinners on the patio. At least until the weather cools down this Fall! By October, I think we'll be dreaming of Spring!

      @@JimZim I love gardening and my mind spins thinking of the potential you have with that lot. We bought, on move into our current home, a built in fire place. Main gas and log burning. Between kids visiting, friends staying and hubby and i, got alot of pleasure from that. Was worth the investment v std fire pit concept. Can email you photo if interested.
      Excited and happy for you both. We are 19mths to 2 years behind you. With all this going on, decided we are retiring 3 years earlier than planned. Stage 1 of our 2 year plan has started. Your vlogs are certainly motivating with this in mind👍🥂

    I was just diagnosed with this yesterday. Watching your video of getting up and down in that chair made me want to cry.
    That pain is so fresh in my mind. I couldn’t walk and couldn’t raise my arms because PRM really targets hips and shoulders. I was alone and after 3 days, I didn’t go to the ER because I figured they would just give me Tramadol and I had that and it was doing nothing. My regular doctor was useless and made me wait 9 days to see him.
    I am so grateful you posted this because I realize I’m not alone and it felt so alone. I saw an arthritis doc yesterday and I have issues with steroids but they are getting me on something else. Thanks again for this supportive video because it helped me a lot.

    Know how you feel. Have had Rheumatoid Arthritis 34 years. Remember in the beginning when my mom had to bathe me and dress me. I was in my 30's when diagnosed. With good treatment and following directions I was able to do active nursing for 43 years. Am now retired and cruising as much as I can. Be strong and do as your instructed. There is life after a bad diagnosis

    wow, you have described what my husband has gone through the past few months, all the physicians he has gone to has diagnosed him with arthritis, but it was a sudden pain and it is in every part, both hands, arms shoulders legs and feet. his pain has gotten so bad that he has a rough time dressing. Thanks Jim for this update and eye opening video.

My little guy loves to watch all your train videos and frequently wakes me up in the morning and asks if we can watch Jim Zim. Things are always hardest in the beginning and you will adapt and improve. You are awesome.

      Thanks, Ryan. I'm already much better... basically back to normal now thanks to the meds they prescribed for me. Everything looks good for our upcoming move, and once we're in the new house there will be some amazing new model train videos. Also, towards the end of this week there will be one last model train video from our California house, but things are going to look VERY different than you have seen in previous videos shot here!

      There should be a new one about one week from today, if all goes according to plan over the coming week. This will be the last model train video I'll make here at our California house, before finally packing up the last of the trains in preparation for our move to Washington next month. I've actually already packed up most of the model train stuff, but have saved just enough to make one last video before closing the door on the California chapter of my life.

    Jim: One thing I can add to your Prednisone experience - via my personal experience it is imperative to wean yourself off of it when ending treatment. I learned this through personal experience and the cold turkey rebound from stopping this treatment; Never have I felt so bad! I was later given a rx for another problem and successfully weaned myself off without any complications. One other thing - No doctor ever forewarned me of this rebound effect, but when I googled "prednisone rebound" I found many similar experiences. I genuinely enjoy your cruise and train experiences and I pray for your swift recovery

      I can imagine that would be nasty. I had that happen with a different medication that needed to be tapered rather than stopped cold turkey. I don't know if I didn't read the literature, or what... but it definitely got me. I am aware of that for Prednisone, so won't be making that mistake!

    Your description of the sudden onset of polymyalgia rheumatica mirrors my own. On Sunday afternoon I contacted my doctor who said to go to the hospital emergency unit. I managed the walk to the car with my husband half carrying me. They only gave me painkillers and sent me home. The pain on moving got worse so on Wednesday morning we called an ambulance. At the hospital they didn't mess around. I had painkillers then X-rays and CT scan then I was admitted under a rheumatologist. Home now after 8 days and mobile - that Prednisone really does work but it took a few days to feel it.

      It's complicated. I was fine for quite a long time after they put me on Prednisone. Every month we lowered the dose of the Prednisone a little bit... with a goal of eventually just getting to a zero dose. But once I got down to just a few milligrams of Prednisone, I started noticing some pain in all the places that were painful when I had my bad episode originally. Eventually, I did get all the way off Prednisone for about two months, but it seemed like the pain was increasing... very much like it was in the months prior to what you saw in this video.
      So, it looks like I'm one of those people that can't just wean off the Prednisone and be fine. For an intermediate solution to the problem, I'm back on a small dose of Prednisone, to keep me mostly pain free. But the next step of the journey is going to be to start on an entirely different medication... which I've been told to expect to be on for approximately two years. I haven't started it yet, but will soon. Unfortunately, with the new medication, I will have to not drink alcohol, avoid long periods of time in the sun, and my immune system will be compromised... which means I will have to wear a face mask in public even after it is no longer required for Covid.

    Oh my gosh. I can relate. First off, I want to say how happy I am that you feel better.
    I basically felt the way you did if only I had seen my Rhumatologist earlier. I waited 2 1/2 mos and in chronic pain, like you described everyday.
    I also suffer with carpal tunnel from time to time. Luckily, I go through bouts where it really bothers me snd then subsides for bit.

      I am! The prednisone has completely eliminated all the inflammation of my muscles. It also has an odd manic effect of making me feel extremely energetic... so I'm getting SO MUCH done around the house, sometimes at odd hours of the night when I really should be sleeping. But I'm definitely better off now than before, and feeling very well overall.

      It really was! And to simply describe the pain to someone... I don't think it does it justice. That's why I set up that camera in my dining room that morning. I wanted to show my doctor the kind of pain I was in first thing in the morning, even doing something as simple as sitting down or standing up.
      One thing I learned about myself is that I can hold it together for pain up to about a level of 8 on a 10 scale. But at a level of 9 or 10, it elicited quite a few words that had to be bleeped! So, 8's my limit, apparently.

      @@JimZim I was very lucky also and found a young Pain Management Physician that helped me. I have actually been pain free since November 15, 2019!! Had to cancel my 60th cruise because of covid 19 but we have another cruise booked in February to celebrate my husbands birthday!! Best wished and ENJOY YOUR RETIREMENT!

    I get a 10 out of 10 pain when I get in a Camry. And I like Toyotas. And I don't have pmr. Now I have to worry about getting this.

    So sorry to here you have PMR. I too have PMR my first attack was at the end of October. I’m still taking 3mg daily of prednisone. Again I’m so sorry to here this and I know what you are going thru. Travel Safe!

    Jim my brother had very similar symptoms and saw a specialist here in Canada. He has the same disease and had immediate relief when he took prednisone. I'm glad you are feeling better.

      Thanks. Yes, Prednisone is a miracle cure for those of us who end up with PMR. I am now totally back to normal. I'll be on the Prednisone for quite a few more months, though.

      I take prednisone for a medical issue. I reminded my brother to mention bone scans because it can damage your bone health. Also vitamine d is important and calcium. I enjoy your videos and we love to cruise when we can however our Canadian dollar doesn't take us far. lol take care

    Hey Jim, PMR here as well. Diagnosed Nov 2018 after same experience as you. I turned 60 and ran a 1/2 marathon the same weekend. I have participated in many full/half marathons in my life but this time the post race recovery was terrible and I felt as if I aged 30 years overnight. Fortunately the excellent docs at the Nashville-Vanderbilt VA recognized the condition and prescribed prednisone.
    Prednisone is a miracle drug and horrible drug at the same time. The relief was extraordinary but the usual side affects kicked in about 3 months later. I've tapered down to 2 mg, but have accepted a baseline of tolerable chronic pain. Studies indicate that PMR eventually "burns itself out" in a matter of time for most. Fingers crossed that during year 3 that time will come.
    Questions for you - How are ya? Any updates on your PMR progression? Improvements? FYI - Dan Patrick (formerly ESPN - now Peacock) has PMR as well. I now call it the "Dan Patrick Disease". When I try to explain what PMR feels like, I suggest Googling "Dan Patrick PMR" as Dan explains it perfectly. Oh - so do you.

    Jim, I had carpal tunnel also and a woman taught me an ancient cure for it and it is very simple and it worked for me. The cure is to take a golf ball and put it between your palms and roll it in a circle for 2 minutes or so daily. And that is it. It took two weeks to work but it worked and it worked well and it never came back. It's a 50 cent cure and without surgery. I hope it also works for you. Good luck.

    Poor Jim! I have both RA and PMR. My pain levels can get to 12 out of 10. Not being able to dress myself, turn doorknobs, and use my hands at all. I drew a picture of myself on fire. Prednisone worked miracles within hours.

    Oh I’m so sorry Jim that you had to go through this, my grandmother had the same exact thing and now that you feel better would you consider a complete lifestyle change? Which means no meat no dairy and daily exercise swimming biking walking if you lose your omentum you will feel 100% better! there’s a couple documentaries ‘what the health‘ and ‘game changers’ I hope you’re feeling better and I hope that you have a wonderful long future! 🙏🏻

      Fruits, vegetables, whole grains, low-fat meats, low-fat dairy products, and reducing the amount of salt in my diet as much as possible.

      Yes, it was a big relief! To go from pain of 9 or 10 on a 10-scale just one week ago, to feeling full of energy and nearly 100% healthy today... it's an amazing contrast!

    So sorry to hear that you had such a terrible experience, Jim, but I'm relieved that you're okay. Prednisone can be quite the miracle drug - I have taken it in the past for life-threatening asthma and not-so-life-threatening skin conditions and it always worked like a charm. Only negative I saw as the weight gain and "moon face" that can come along with it....but that's a tiny price to pay for a drug that creates a major improvement in the quality of your life.
    Anyway, I really enjoy watching your videos and can't wait to see you back on ships again one day. Please keep recovering well, and I'll be thinking and wishing the best for you in the coming days!

    I just stumbled onto your video.about PMR. I'm going for blood tests tomorrow if I'm able to make it to the hospital. I suspect I have PMR although I won't know until I get the results. I imagine you're doing much better.now. Thanks for posting the video as I found it very helpful.

      Yes, Prednisone was a miracle drug for me. I was immediately better the first day they put me on Prednisone, and stayed on it for about a year until I weaning off of it. Unfortunately, once they tapered my dose down to smaller and smaller amounts, the PMR symptoms returned. So, what they had to do was put me back on Prednisone, and also Methotrexate. Then the dose of the Prednisone was slowly tapered again... and now I am fully off of Prednisone, but still taking Methotrexate. I am feeling fine, with no PMR symptoms. I will be seeing my Rheumatologist again soon to find out what the plan is for getting me off Methotrexate.

    I just seen this vid on what has been happpening to you..so glad youre feeling better now.. my brother has arthristis really bad and one of the drugs cause him to have a heart attack..to make it worse he lost a lot of weight and we his family didnt even recognise him..Yes he was on deaths door ..then they gave him Prednisone and its certainly is a wonder drug and now he's coping much better and still with us thankfully..

      It's funny you should mention that! Different people have different reactions to Prednisone. Some people go in to a depression and get a lot of anger. On the other hand, some other people get a "Prednisone euphoria" and an excessive increase in energy. One web site describes it as "a sense of wellbeing that is 'inappropriate' to improvements in physical health." But I think your term, "high as hell", is probably a better one.
      I am definitely in the euphoria and excessive energy category. I have been SO incredibly productive this last week! I wish I could get a full 6-8 hours of sleep at night, which I currently can not... but dang, I am now about three weeks ahead of schedule on packing and preparing for our move! And I've been having a lot of fun out in my garage woodworking shop at making custom enclosures for moving some of our unusually shaped or delicate items! One thing I've been doing -- a very non-traditional moving strategy -- for some of the furniture that we are not planning on taking with us to Washington, I am selling it now, five weeks before our move, and slapping together temporary replacements out of 2x4s and OSB out in my workshop. This way, I don't have the stress of trying to sell off a dozen different items on our very last week. I can sell them now at a more leisurely pace, and just live off of some really ugly looking temporary 2x4 version of the same furniture piece for the next 4-5 weeks. This is also a good way to use up some of my large stash of lumber that's been sitting out in my garage!

      @@JimZim glad to hear that! My father has stage 4 prostate cancer, and before chemo treatments he was on prednisone to help, and was really hyper. it was really swell to see him so enthused and positive from the effects of prednisone under such difficult circumstances. Ironically, the prednisone while "only" being a steroid made him feel better "physically and mentally" and it seems to have done that for you as well. Truly a wonder drug. Since you're retired Jim, you have the time (and now the energy) to do things at your own pace. There is most definitely a transition from work life to retired life, and busy people like you (and my father) need to relax and enjoy retirement at that easier pace. Its literally what you have earned and worked so hard for! enjoy! :)

    Jim/ at the onset, i said to myself "i don't want to listen to someone else's health problems; i have enough problems of my own"...well, lo and behold. i have the same health problem with psoriatic psoriasis but not so much joints problems as with terrible muscles pain making me like you say a 100 year old man...so i can relate...damn, it gets so bad at times it brings tears to my eyes...now prednisone is a wonder drug used for many things...but we can't take it a high doses or for long periods of time because of toxicity...heck many people have liver damage/problems due to just taking just aspirin, acetaminophen etc for long periods...i will research Polymyalgia Rheumatica...now, just give me my preds and i will be okay...thanks for your video...blessings...

      Prior to my emergency room visit, I thought my immune system was attacking my joints, not my muscles. But the E.R. doc did a simple thing that helped me to see it was the muscles, not the joints. I mentioned to him that bending my knees brought tremendous pain. He ran his hand over the front of my knee, and it didn't hurt at all. Then he ran his hand around the back side, where all the muscles are, and with just one light touch there was a big shot of pain. It was obviously not my knee joint causing the pain... it was the muscles running along the back side of my leg. I hadn't figured that out until the E.R. doc did that!
