I think Polis Massa is a hidden map, but not of Earth. I'm fairly certain it is a map or picture of Ganymede wrapped around the asteroid's mesh. Edit: Yavin (the planet, not the moon) appears to be a map of Mars around the area of Valles Marineris.
Geonisis being in its own category is kinda funny because in 2017 battlefront 2, on the geonosis map, there’s a terminal with a map of part of the planet on it and it just straight up has the Great Lakes on it with no attempt to hide it
In the case of Tatooine I think it's a satellite photo of the Libyan desert, I don't know what you mean by a lake because I can't see it in the texture, but the mountain in the north seem to me to be the mountainous area south west of the cities of Sawknah and Waddan in Libya, rotated 90º.
Galactic Conquest is neat to me because as a kid I had no idea what I was doing and didn't appreciate the added challenges and customization to gameplay. But a few years ago when I bought OG Battlefront II backwards compatible for the Xbox One, I got totally enthralled in it. I also didn't know that each faction had a neat cutscene when you won with them

Unrelated to this game, but in Jedi Outcast you can find Earth on one of the skyboxes. You can't see it in a legit playthrough because you're travelling in hyperspace, but if you're using a cheat code to clip through that, it's there.

One of the main planets (I forget which one) in X3: Reunion (2005) has all of the coastline of India & SE Asia, which then blends into Mexico (starting at Baha California), Central America and the US Gulf coast; the Black Sea is also plopped into the middle of the continent. I've also seen similar things elsewhere too, like the (PAL?) Star Trek: Voyager DVD box set has Voyager orbiting a planet on the box, but the planet is just Earth flipped horizontally (you can clearly see the eastern Mediterranean). I think the DS9 and TNG box sets do the same sort of thing but not on the outer box.
This is just an incredibly neat video presentation, whoever worked on these planet visuals for the game would probably love to see such an attention to detail someone like you has here.

And decided to play it again like last year. I've noticed something I have never noticed before... some planets in the background have just real map places mixed up in a weird collage. Was real fun piecing it together though.
Never would have know and I love this info! I crushed this game when I was younger! I played galactic conquest with my brother & I would play this for hours when I was younger just on my own I enjoyed it so much.
I think the craziest thing is that they didn't just use the maps for the filming locations... Or copy the textures from the movies... Actually I feel like they kind of did that...at least for some of them...I've definitely seen other star wars games use those same textures for planets...
Might also be Mullet lake south of Cheboyan Michigan. I went to google earth to check out Michigan and spotted Mullet lake before I spotted Frankfort, thought it was crystal lake you mentioned until the label showed up.

I couldn't begin to speculate on where it is, but looking at Kamino it looks to me like it's based on real coastlines but with the land colored blue. The darker bits could be land, the lighter bits water, and the atolls being coastlines. Just the outline of the atoll reminds me of the coast of South America flipped upside down.
This reminds me of how they put Masterchief landing on Fort Meyers at the end of the first level in Halo CE and Basis was a white colored Mars. Since then every Halo game has had a fun geography hunt. The Yavin gas giant looks like a texture of Mars at poles and Polis Massa is Ganymede.
I've played this game frequently since the day it was released in 2005. I never noticed this stuff before, and now I'll never look at this game the same way again.

Developers of Battlefront were either super lazy with planet textures, or they did not have much time for it. In Empire at War planets textures had much more efforts into them.
We had a similar game collection! I loved the Bionicle games esp Heroes and ofc Battefront 1/2. I was mostly a pc gamer as a kid and didn’t play many shooters besides Star Wars until much later
Other media in Star Wars has used Mars for Tatooine and Dxun too; iirc in one of the movies Tatooine is just a colorgraded photo of Mars, and KOTOR2's depiction of Dxun is a heavily re-colored Mars
Take a look at the maps of the loading screens for Battlefront 1 of the classic edition. They are more zoomed in and not planet size, but I think you would recognize a few of them.
You should do this with star trek too. One of the planets from star trek enterprise was literally just a blown up picture of the island of lesbos, and one from voyager had the island of borneo on it.
