    Good afternoon I had lower back surgery the L4&L5 fused after surgery I use a catheter and has problems with passing stools. What can I do

      It sounds like you have severe nerve damage or potentially have prolapse that is causing poor flow of urine and stool. He certainly need evaluation by a gynecologist to determine that.

    Dr. McQueary, I had a total hysterectomy 18 years ago. every now and then I feel like my bladder has prolapsed, but it seems to go back?? Recently, like the past 2 or 3 weeks, I'm beginning to have that collapsed feeling. My Gynecologist told me a few years ago that I had a cystocele. How long can you wait before surgery becomes necessary and is there any danger of the bladder compromising my vaginal wall and should sex be put on hold? Thank you for your time and attention.

      @@carolynmontgomery7815 main issues are if you have difficulty with emptying your bladder that’s when cystocele becomes a significant issue. Also, if you have start having significant urinary tract infections. As far as repair, it would be appropriate to consider it at that time. Typically intercourse is no issue and will not make the problem.

    With a rectocele surgery what is recovery like ? Is it painful? I have to have it im scared to death I have a great gyno/neuro but I’m scared it will hurt to have a bowel movement? Any advice?

      Definitely the posterior rectal side is more tender but patients seen at 2 weeks are typically doing well. Everyone is different though and it is based on pain tolerance.

      @@DrJeffreyMcQueary thank you for your input but I neglected to ask I’m assuming your first bowel movement is scary 😱 but you will go that’s my greatest fear I also have autoimmune and active RSD but it’s managed at this point I do have a excellent doctor I feel I need a antibiotics is that out of the normal to ask? Now when they do this type of procedure are you catheter? Please tell me if so they can remove it before I wake I don’t have good experience with catheters Dr.Jeff thank you for doing what you do and I apologize for all the questions but you can understand my fears I was supposed to have this last March but I had bicep tear and rotator and that was not fun so I’m 7 months out doing well it’s just the thought of another surgery it scares me! Thank you again for allowing me to ask you and yes I will be consulting with my specialist but it helps to know questions to ask him

      @@carlamarquette822 antibiotics are common, but unfortunately also is a catheter. Typically, I make sure my patients urinate after surgery even if they’ve had just a repair of a rectocele.

    When I had my first baby, he was a force up birth . No episiotomy was performed, and I had a fourth-degree tear into the anus . Years later, I had external hemorrhoids removed . One big one was missed . Now, something tore and there was blood . I think it was the rectum. I also had a vaginally hysterectomy many years ago . In order to poop, I take Fibre 4 and senorita as necessary . My GP sent a referral to a GI SPECIALIST. NOT AN OB OR GYNECOLOGIST. . I told my GP about these issues 3 years ago . He doesn't listen . Now bad things happened . I really hope I can be repaired .

      Sounds like you need to see specialist in Urogynecology to throughly evaluate and see what the real problems are. GI specialists don’t treat these issues unless it is internal hemorrhoids.

    I have been diagnosed with this, the scan showed a badly scarred Bladder. I have had Instillations which helped a bit, then acupuncture, which also helped. But what I would love to know is, why does my pain get worse after passing water.

    I had a hysterectomy,cystocele and rectocyle in 2022. I started having some issues in 2023. This year in 2024 is worse. I went to a gynecologist in Knoxville Tn,close to where I live. I’m going to check out a colorectal surgeon. Terrible surgery. Disgusted. The dr told me during exam that these surgeries are all he does. Shameful drs out there

      @@lizwaandersch3421 For an ultrasound that is done through the vaginal area you typically want to be empty, but typically have a full bladder when it is done through the abdominal area. The bladder is used as a window to see the uterus and ovaries.

    I'm a male with IC x 21 years. It's still going strong. What I can't understand is how nerves in the pelvis can continue to fire off . These sensory nerves never burn out. WTF ! The only help I get is by drinking lots of water. Nothing else helps. Drink, drink, drink water. The solution to pollution is dilution.

      That is often why we use nerve medications such as Lyrica and amitriptyline.

      I started on gabapentin early then changed to lyrica once it became less costly and generic. I've been on amitriptyline 25 mg at bedtime. Drink, drink, drink.

    After listening to this.... Why would I have my prolapse fixed. What can happen if I choose to do nothing? I gave stage 2 3 bladder cervix and uterus prolapse.

      @@robinlabranche6147 you certainly don’t have to do anything about this, and it won’t cause you any major harm. But prolapse often will give you bladder issues with difficulty with emptying your bladder. This can lead to persistent bladder infections and some bladder issues such as urinary urgency and retention. Still don’t hesitate to see an expert in, dealing with these problems as well as fixing these problems and do the right thing for you.

      You can have a normal delivery and you shouldn’t have a repair until you are done with childbearing.

      It can happen from damage from the delivery. A pessary can help hold the uterus up especially if there is desire to have another child.

      If it is interstitial cystitis, then you have a lot of inflammation or swelling in your bladder. There are many treatments for this and you need to go to a urologist or urogynecologist to find out how to treat this.

      @@nelmaantenero1345 may not be a uti. Make sure they get a culture of your urine when you are tested to know the bacteria that are causing it and the antibiotics that will help effectively.

      Sorry typo. Buldge knot that dropped down. It's a small very small section from the vaginal wall that has separated and fallen outside the vagina. It appears as skin that's a small roundish bulge knot. That sits very close to the urethra. It's about the size of a normal belly button. It's skin that's detached. So my rectocoele is stage B so its considered very small from the GYN perspective where Colarectal sees the issue for what it is a serious large bulge inside coming in from the rectum. It astounds me that the two different perspectives are wildly of opposite option on the seriousness of my condition. Thank you.

      @@TrudySchwartzBurrill the main issue is how you function and if you have difficulty and having a bowel movement, then it is of concern. Go to a urogynecologist and they will take you seriously.

    I am stage 4 pelvic floor prolapse. A few years ago I had my uterus removed. Now my bladder fills my hand. I am going in for some big surgery July 30. They will pull up everything and attach it to my sacrum. The doctor said it is a mess. I can't remember everything he said but basically i am a mess. I am hoping this is a safe surgery for me. He has me putting a tiny pill inside to strengthen the tissues before surgery. He will also use mesh for the bladder sling. It will be robotic. Any suggestions? I don't get along with anesthesia as I have a difficult time waking up and it takes a long time. Thanks.

      You are having a surgery called a Sacral Colpopexy. It is a very effective surgery using mesh attaching the upper vaginal area directly to ligaments on the tailbone. It’s a very effective procedure if done by the right person. The bladder sling is also very effective and should be great ways to fix your problems. I’ve seen the effectiveness for both of these procedures to be over 95%.

Dr. Ty Erickson is my doctor and he goes to Africa and does this for the poor, mistreated women over there and does a lot of them here in the US as well. I feel pretty good about it but I do not look forward to the waking up part. That always seems to be the worst part of any surgery I have had. Thanks.

      ​​⁠ good luck with your surgery and post operative recovery! There are many medications that can help with nausea after the surgery. Ask anesthesia to help.

    I have a bladder prolaspe, dr said it was level 1-2. She recommended a pessary with support, tried 2 sizes. Both were put in correctly, but both slipped to an angle and wouldn't stay horizontal. She gave no explanation as why. Can you help?

      I have had patients say they felt their at an angle but when I look The only thing I see is the pessary falling out. Do you want a pessary or to have those problem resolved? Just get another opinion about your problem, you need confidence in the doctor you see.

      Often if your doctor is not very concerned or interested in your prolapse situation, it is because they don’t have great experience in being able to fix these problems. May not be the right doctor for you and seeking out a doctor that specialist and pelvic prolapse such as a urogynecologist would be appropriate.

      @@dinapalkowski8146 realizing that your pelvis has weakness that cannot be completely repaired, it is important to not do too much heavy lifting or straining. With that a pelvic surgery can last for many years if not the rest of your life.

    I've made my appointment and have a month to wait. What is it called when the rectum itself becomes dislodged into the vagina and you're unable to defecate? The spincter seems unable to allow anything to pass but, the feces bulk is felt inside of the vagina? Sorry for TMI. I am trying to be an informed patient

      Absolutely you can have reconstructive pelvic surgery without hysterectomy in fact that can be a very strong repair. Biological skin graft is a great option.

    DO NOT LET DOCTORS LIE TO YOU AND FEED YOU BULL POOP ABOUT THE RISKS OF POLYPROPYLENE MESH IMPLANTS. I had TVTO inserted in 2011, ruined my life, lost my career, sex life, social life. I have had 2 surgeries to remove the mesh, still 11cm remnant that cannot be removed, wrapped around my obturator nerve, permenant nerve damage, permanent pain all the time. I've had 2 bladder repairs done , both failed, now my bladder is hanging out, I need hysterectomy, bladder repair and scar tissue removed. Please research pelvic mesh scandal. I cannot have a pessary inserted as there's too much scar tissue, and I can't have vaginal hysterectomy . I have grade 3 cyctocele. I'm 44 and my life is over , miserable and all because of pelvic mesh. Also used for hernia! Sling the mesh!

    I have very bad overactive bladder which make me hardly get rest i have to wear a catheter for 6 months now thank god I’m better take D-Mannose and cranberry capsule plus oxybulynin and pumpkin seed oil feel much better

    Should I hold on to a uterus with fibroids post UFE and atrophied? In what cases are surgeries not successful? Can fibroids lead to cancer?

      Fibroids typically do not turn into cancer but the uterus should be fully evaluated. I certainly have done surgery for prolapse keeping the uterus intact even though there were small fibroids after menopause.
