I had vaginal and rectal prolapse… surgery did not help!! Within 3 days it felt no better than before!! I was devastated as I was a waitress and a single mom!! Make a long story short… a pessary ring saved me ! I got the smallest one and 13 years later it is still working!!! I am 70!!!😀👍

Pessaries do work well if you’re willing. A lot of people really don’t want that and they want surgery right away. In my experience surgery works well but it needs to be done by an experienced surgeon.

@@DrJeffreyMcQueary I supposedly had one of the best … I went in 3 times after my surgery telling him I was feeling the same falling out as before . He would give me a quick check and say nothing is wrong… I am I excellent shape with no health problems so it really bothers me. He retired shortly afterwards and I ended up going with a pessary. I had the “ mesh sling” which is totally different than the surgery my mom had years prior and she never had a problem. But , all’s well that ends well I guess, and thankful for the pessary, it has worked out wonderfully for me! 👍

There is life after prolapse. Oh I can so relate to this. In 2018 I had prolapse surgery for uterus & bladder failing out. My back hurt and I had such pressure. I was basically home bound due to constant having to urinate. If we went out for quick meal & I could sit I could handle that. So I had the surgery 2018. So 5 weeks post op my bladder doesn’t hold up to the surgery and prolapses again. So I was having some issues with my tissues healing so I had to wait until August of 2019 to have surgery again. Then came Covid. Still not finished in 2021 I had to have a total knee replacement. I’m doing great now but I’m not going to lie it took me some time to physically ,mentally, and emotionally heal. I couldn’t do pessary they just came right out. My worst times were when there were low pressures in the area. The pressure was just horrible. The surgery wasn’t too bad and from the prolapse surgery by the 5-6 week I felt pretty decent. I had robotic surgery and yes I still had post op pain but not as bad as previous surgeries.

I’ve been battling with stage two cystocele and rectocele for about three years now I have seen a PT and it relieve the symptoms but they tend to always come back. I have lost weight but the minute I gain two or 3 pounds I feel pressure I am 53 and perimenopause, and still getting my menstrual cycle but very heavy and very irregular now my last one seemed to have pushed the prolapse out to probably a stage three scared me to death. I do not want mesh anything foreign inside me I’ve been trying to avoid the surgery I cannot pick up my grandbabies. I cannot run around too much without suffering later on having to put my feet up the pressure is unbearable, especially right before my menstrual cycle is supposed to come, I have periods where I can go a week or two without any symptoms but now I’m at a point where I welcome the surgery but do not want mesh seems to be more my uterus and cervix than my bladder. I’ve gone to a urogyno and my bladder seems to be working fine and I empty completely I asked for a hysterectomy, but they said that would not help my problem completely. What would you suggest? I am truly exhausted

3 years ago I was told that I have stage 2 bladder prolapse and a stage 1 rectum prolapse. They did mention something about a vaginal they did mention something about a rectocele but I'm not sure if that is the rectum prolapse or whether it's something different.
The surgeon 3 years ago wanted me to have a complete hysterectomy because of a cyst on my ovary and at the same time the another surgeon was going to fix the rectocele. When I refused to have the hysterectomy the other rectocele surgeon said that she didn't want to fix the rectercele.
Three years later (last month) I decided to get a second opinion and this surgeon said I didn't need hysterectomy. However,
because of severe constipation my life is miserable. Nothing that I take relieves the constipation and I have tried everything.
Miralax, enamas, fiber, colonics and of course I completely changed my diet by eliminating sugar, and dairy.
I then went to another incontinence Doctor who said that my bladder prolapse was still stage 2 and my rectum was stage 1. (The same as it was 3 years ago)
I'm a very young 76 years old and I use to be very active but now my whole quality of life has changed.
All the incontinence doctor suggested was that I do kegel exercises and use vaginal hormone cream.
I'm at a loss as what to do because I am constipated every day of the week all day. Although I don't have any pain it's very uncomfortable constant pressure.
Please can you tell me what I should do because I don't feel I'm getting any answers from the many doctors that I have already seen.

I can’t tell you exactly what to do without examining you myself, but even with the stage one rectocele you could be very symptomatic with severe constipation. These problems can be repaired and often I will do that without a hysterectomy as well. A pessary, and sometimes also help with the bladder but may not help with the rectocele. I have had some patients that it really helped though. You certainly need a doctor that will give you a plan. Kegel exercises may help but they won’t fix the problem.

Also does estrogen cream helps? It’s so frustrating but pessary helps me to feel more confident,I don’t have any pain or problem with pee , only if I am all day to long on my feet it’s comes out,

Estrogen cream can help if you are menopausal. Surgery or a pessary is not typically needed for mild prolapse. You may want another opinion on your prolapse to see if this is worse than stated and if this could be fixed.

This sounds like a very difficult problem that I cannot know exactly what’s happening with your bladder, but sometimes scarring can lead to a restricted bladder capacity. Pain in the bladder area also can be interstitial cystitis and seeing a urologist or Urogynecologist that evaluates that would be appropriate.

I have been diagnosed with w a cystoseal and hv urinary urgency . I am on vaginal hormone treatments . The only solution my Urologists suggests is an implant of a device in my hip which communicates w my brain to alert me when I shd empty my bladder to try to avoid leaks. This doesn’t sound like it would treat my issues only possibly avoid leakages if it even works. I wish I could consult with you Dr. Q.

    • @DrJeffreyMcQueary
      @DrJeffreyMcQueary  5 месяцев назад

There are lots of options for urgency, but probably a cystocele needs to be repaired because often this is leading to poor urinary flow, which can then cause urinary urgency. The device that he wanted to implant is called an Interstim or Axonics. Medication can be used as well. That can help with urinary urgency. Botox injections are also an alternative. Hope you get the right care.

I do understand how difficult these problems are, but there are definitely ways to deal with this, and even for her being at 94 years of age a Copalis, if she is medically capable could be a closure procedure that could really help. Pessaries still can be very good ways to maintain. the vaginal area but infection still do happen. Typically infections are due to poor empty and you may consider a larger pessary or a new pessary that could help resolve some of that retention issues. Good luck with these issues.

Exercises for your core muscles are the best and you can find examples all over the internet for that.

Doctor - thanks for your previous responses - very helpful . I’ve now seen 2 urogyns. I am 66 and apparently have uterus prolapse, cystocele and rectocele. Stage is 2. Both are pro removing the uterus. But one is very anti mesh and one recommends it and says she does the sacrocolpobexy 4x a week. I guess the mesh kind of scares me - but my understanding is the sacrosinios attaching to your own ligaments can fail. What’s your thoughts on the mesh option.😊😊

Mesh use on sacral colpopexy is the gold standard and the use of mesh is appropriate in this situation. If your doctor does this procedure best then it should be successful. I did mesh for years with minimal problems for patients. Good luck!

@@DrJeffreyMcQueary I realized I didn’t ask - did you stop doing for a reason? And why is this mesh ok and the old mesh is not?

@@kathleengallo3180 mesh was removed from the market only due to lack of superiority over 2-3 years. I still have patients with mesh now 18 years out from their surgery still doing well. Vaginal placed mesh was removed from the market in 2019.
Abdominal placed mesh for sacral colpopexy or suspension of the vagina to the tailbone ligament is still considered to be the greatest success rates but is more invasive than just a vaginal surgical procedure. I have great success with vaginal minimally invasive repair.
