    I’ve been taking lowering IOP drops for more than a year and I am beginning to experience the same side effects you Listed. Not sure what to do, doctor says I’m imagining things and mandates I follow up my monoprost + kosopt combination- I’m 32 yo and never had issues before that such as these.

      He mentioned the term Neural adaptation. I think this is what youre asking. "Neural adaptation is the process by which the brain adjusts to the changes in vision that occur after cataract surgery. This process can include adapting to light dispersion, such as glare, halos, or streaks, and relearning how to use the eyes effectively. The time it takes for the brain to adapt can vary from person to person, and can range from days or weeks to months or even years."

    Hi doc, I have cataract, glaucoma and until recently uveitis. Because of uveitis the cataract surgery was delayed for quite a while, while uveitis was treated with steroid tablets (prednisolone) and drops initially and then steroid drops for quite a long time. My doctor opined against any surgery before uveitis is controlled. Now uveitis is almost gone / quiet I'm told and that I should proceed with the surgery asap! My current eye diagnosis is as follows: 1. S/P LPI (both eyes) -- [Status Post Laser Peripheral Iridotomy??], 2. 360 PS (right eye), -- [360-degree Peripheral Synechiae??] 3. Complicated Cataract (both eyes), 4. PACG (both eyes) . -- [Primary Angle-Closure Glaucoma??] 5. Arthritis 6. H/O CKD. .......................... The "complaint" section says: Visual Acuity: Aided. Right Eye: 6/18 p +2.50. Left Eye: 6/9 Plano Add +2.50. Anterior chamber: OD Quiet. OS: Quiet. IOP: OD: 18 mm Hg (NCT). OS: 20 mm Hg (NCT). Iris: OD: 350PS. Lens: OD: lental opacity. OS: Early lental opacity. Fundus: OD: Looks normal. OS: Looks normal. --------------------------- "Surgery Order" says: "1. Phaco with PCIOL (right eye) local anesthesia with Synachiolysis+ iris hook under oral steroid coverage". --------------------------- I was hoping to use "Clareon Vivity IOL" (EDOF) because I work with _computers and near and intermediate vision related work mostly,_ but my doctor says premium/multifocal IOLs are not advisable in my case since (to the best of my understanding of what he said!) if "Synachiolysis+ iris hook" unravels sometime after the surgery or in future, multifocal IOL will become totally useless, even detrimental. Which is a very undesirable situation. He recommends standard monofocal. *MY QUESTIONS ARE:* 1. Is Alcon "Clareon Vivity IOL" a "multifocal" IOL really? My understanding was that it's something different due to new technologies although it covers some of the multifocussing benefits of a "multifocal" lens. Vivity/Clareon is VERY new in my country and I suspect my doctor may not be familiar with it and thinks it is "multifocal". I checked last year and it was not available in my country at that time, but it's now available (probably except Clareon though). *_So my questions are again:_* *1.* Is Alcon "Clareon Vivity IOL" a "multifocal" IOL ? Is it appropriate in my condition in your opinion, especially in case of ""Synachiolysis+ iris hook"? If you think it's okay, then what can I say to my doctor to explain / query this? *2.* What would you recommend as safe & most suitable in my case from the following (available from my Eye hosp): Monofocal Aspheric (Zeiss/Alcon/J&J/Hoya) / Monofocal Plus (Alcon/J&J/BVI/RAYNER). Also: Alcon Vivity (it looks like Clareon Vivity may still not be available here, it's probablly mostly Vivity AcrySof IQ/Restor/SP/Toric/Sert). *3.* What's your view on a Monofocal Plus or Enhanced in my condition if Clareon Vivity is not suitable or available? Please help!

    It took 2 years and 4 eye doctors to find one who could tell me I had a cataract. She even did so prior to dilating my eyes. I keep saying something was fundamentally wrong.

    Doc, may i know why you said it's only for age 40 and later? I am 33, had cataract while my refraction is normal (no need flasses for near and far). Had my left eye with panoptix (non dominant). And i want to use vibity (edof lens) for my right dominant eye. Can i do that?

    By the way I did try the glasses with the lenses set up with mono vision at the Lasik place I went to and seem to have no problem with them or that perception.

    I am a 58 year old male. Glasses since the age of 44. By the age of 50 I needed bifocals. I was scheduled to get mono vision Lasik in Connecticut on Friday. Is this blended version a proprietary treatment only done by a certain place or are most Lasik places able to do it.

    I'm having trouble finding a dr who will do this, tho I've had natural monovision all my life. I started wearing glasses for distance 10 yrs & distance eye can't be corrected anymore. Drs will do light-adjustable but they are so expensive.

    I’m on a limited budget and want the best vision I can Ferlinghetti . I have astigmatism and a good surgeoun. I’m going to talk to him about going for blended toric iols and train my brain. I have raised the money for the toric lenses thru a foundation. It’s the best option I can have for great sight. And feedback would be appreciated. I have read and listened to everything -even above my head about cones and have come to realize blended yltoricx are my best bet. I will be extremely careful with healing. I know this guy can do it. He’s excellent, and I only have Medicaid so I have one shot at this. I think that’s my shot. I’d love to hear your take. I will be doing these excersizes completely and until it works. My eyes have not been the same anyway so o I think I’m a good candidate. Plus with God all things are achievable. I’m ready.

    This guy is not with the program. The next generation is coming fast. Already in America they are inplanting the IC-8 lens but even better lenses are coming such as LensGen. I would hate to have this guy operating on me. He is one of those guys who says I can set your vision for near or set your vision for far. What do you want? The completely wrong approach!

    Kind would Like to ASK you, im 47 years old have cateract will need soon sugery , have -2 -0,25 right, -2 -1,25 left, use my handy at 35cm, laptop 55 cm, computer 65 cm. Im active with my dog, bike also to Work morning night Anne in free time also Drive Car, v das night, hiking, Would like to have good near,+intermediar seeing, working at Computer, want to have good normal contast, nice colours and want no or little halos at night.       depends on material if acrysof or clareon. I have acrysof and a year later don't notice a problem but most certainly the lens is affected. (Vivity Toric in both eyes)

    Thanks a lot for this video and your videos about LBV (laser blended vision). I'm thinking about getting LBV - if I'm not happy with the results, will it be possible to get an IOL with trifocal lenses later in life? If this is possible, would I have to "reset" the LBV?

    So I was expecting good distance and intermediate vision and would have to wear glasses for reading with lens replacement. But I got good reading good intermediate but not so good distances. This is so frustrating and I don't feel comfortable driving . Its been just 3 weeks will my distance improve?

    If you are in your sixty’s and can afford it get lol lenses , the lens of the future now , if not and you can afford it go private and be able to get the correct lens against the nhs few that do not allow you to go glasses free

    This is actually incredibly accurate coming from someone with a weird cataract story. At 13 years old I was shot in the eye with a BB gun and miraculously the BB didn't puncture my eye, but it still hit it directly. My lens shattered and a cataract started forming instantaneously. I could see nothing but yellow when I opened the eye that was hit. This was very reminiscing of that. I'm sure the reason I couldn't see anything but yellow would have to do with the lens shattering and the cataract. I also remember having a milky white ball slowly forming inside that eye as well. I had to wear an eye patch because it freaked kids out lol.

    Dr. Dylan, can you talk more about Supplementary IOLs like sulcoflex or 1st add on? I´m 37 now and for the past 10 years have had monofocal lenses with residual myopia (2.25 and 1.25) due to a caratact early developed way too soon (sadly in Brazil at that time monofocal were the only option as I was told). As a high myopic person (9ish in both eyes) I was very happy with the outcome (remained with my near vision, gained an intermediate vision and improved the distance vision). But as we always aim for perfection I´ve recently done a prk to correct 1.25 (distance correction) and the doctor explained to me the reason to not correct both eyes. Now I find myself not seeing very well neither near or far (which is really making me sad because as a myopic my near vision has always been good). Doctor said it will get better over time and I´ll learn to live with this 2.25 difference between my eyes and I´ve watched your videos about it and hopefully with training I´ll overcome this. But...if not...I was presented with the option of sulcoflex trifocal lens in both eyes (the one with previous prk only to regain near vision and the other with 2.25 correction for distance). Seems the perfect world. However, as research goes, there´s also a neuroadaptation process that may or may not happen) and other problems such as halos and glares and the posibility of removal due to problems such as glaucoma (my mother suffers from it). Would really appreciate your opinion as I like the detailed yet simple way in which you explain things.

    good info, would be better if you had pics or drawings of what he is talking about as he talks about it, but still good info. i got yag today, almost a year after my cat surgery. so far so good. all clear now. i also had my massive amounts floaters zapped twice but they came back even more. so, im going to get vitrectomy down the road.

    What you call a primitive presentation. Most of these lens mentioned are not permitted for pilots.

    Is there something you can recommend for double vision? If i close one eye i have clear vision but it is tiring especialy while driving.

      I just had cataract surgery on both eyes it has been over a month now I cannot drive unless i close one eye and when I turn to the left, I see double Doctor says to wait 3 months to see what happens I am so disappointed in the surgery!

    I thought it was interesting my dominant eye changed to my Vivity IOL after my first surgery within a couple of days. Also my remaining cataract eye is nearsighted and I neuroadapted within the first day. No glasses for reading. I will have my second Vivity IOL in a couple of weeks and the doc is planning mini-monovision/blended vision. These new IOLs are amazing.

    I had my right eye implanted with a Vivity. Focus distance in that eye about 16" and beyond. My left eye is nearsighted. Everything looks sharp at any distance. Neuroadaptation was immediate. I'm scheduled to get another vivity lens in my left eye in 2 weeks. I hope the prescription is for MiniMono/blended so I don't lose my focus less than 16".
