    I have a question. I have pretty bad pain in the bottom back of my neck that has existed for 20+ years. My PCP sent me to Pain Management earlier this year. PM sent me for an MRI and it turns out that I have a very severe spinal chord compression between C3-C4. Here is where the fun begins! I have seen 3 neurosurgeons seeking treatment. Not one of them is worried about the condition that I presented with- pain in the back of my neck- because the the compression at C3-C4 is so bad that it could easily cause my death with any whiplash-type accident. NS #1 proposed a disc replacement at C3-C4. NS #2 proposed a 2-level ACDF at C3-C4 and C4-C5. NS #3 said that he agrees with NS #2 and I should go with him. I went back to NS #1 and he says that he is fine anyway that I go and says that his disc replacements have a great rate of success. He also pointed out something that I already knew because I have all my imaging on disk- C4-C5 looks WAY better than C3-C4 and C5-C6. <My question is, because you state in this video that a disc replacement can actually cause pain in the back of the neck- which is something that I already have in abundance- Would I be hosed with getting the disc replacement? One comment you made in this video is that ACDF has a quick recovery time and I have NEVER seen anyone else saying that (6 months or more is what I am seeing) but I have seen that recovery from disc replacement is 6 weeks or so. I am sooooo confused on what to do!

    If you or someone you know suffers from chronic back pain I strongly urge you to look into, and certainly with your pain specialist and your physician, spinal cord stimulators. It's not for everyone for various reasons but it works better than any medication without side effects or risk of addiction. It's an outpatient procedure that in many cases only takes about an hour and it gives you instant control of pain that you can dial in to specific areas as opposed to having to take a pill and wait to see if it helps and my experience is no pills stop pain. With medicine it's a question of how fuzzy do you need to be to tolerate the pain. You end up spending at least half a day waiting for the pill to work and it never does. Stimulation not only works but is instant at the push of a button and you can adjust it in so many ways to overpower the pain in whatever part hurts and at the level of the pain, 1 to 10. You can't change the strength of a pill or target a body part. I have 2 of them as a result of a head on car wreck and they changed my life. Literally. I hope this is helpful.

    I have cervical and Lombard spinal stenosis, the only thing the doctor said to me after his assessment was, (so, we doing it he Lombard or the cervical first?,), I broke into tears and found out that surgery is not necessary, I was prescribed a nerve medication to try, it worked well, and since it works, they took it away from me, to force me into surgery 😢😢

    Hi I just listened to your podcast about failed back surgery, I’m short listed for surgery due to severe spinal pain both cervical and lumbar, I have been thinking hard about this wether to go ahead or put up with the pain, I know that older l get the more painful it’s going to be but also may take longer to recover, so I’m confused about which way to go, thank you for your help and information

    Hey doctor will a spondylolisthesis warranty a surgery if xray shows unstable but there is minimal symptoms and no weakness. It's grade 1 . I am 24.had severe symptoms but all have disappeared like numbness. What will you do as a doctor? Will annual xray worth it?

      That's a good question, I should make a video about it. I cannot comment on your case in particular (since I am not familiar with the details) but in general most surgeons will not perform surgery for a patient who is asymptomatic. Perhaps annual follow-up with a surgeon will be a reasonable idea. Wishing you all the best.

    I had cervical fusion and my surgeon didn't file the plate enough. So now it overhangs to adjacent disk spaces. Long, bone spur. The pain is unbearable. Please excuse me because I have not slept in two nights and this happens quite frequently because of my pain thanks

    I’ve survived three helicopter crashes which damaged my lower back with three crushed disks this happened when I was a teenager and early twenties. Later in life I got bad headaches and went to a chiropractor which was a big mistake and he actually made it worse. Then I went on a keto diet to get off my type 2 diabetes drugs and after losing 60 lbs in two and half months and it cured my arthritis and back problems so I’m now 85 and after five years on keto and fasting diet I now feel much better with no back pain and no arthritis in any joints.

    Northwestern Memorial hospital says that they only do fusion surgery. Why is that? Is it because it is a teaching hospital or because fusion pays more?

    It failed because patients think it’s a magic pill,muscles imbalances are the problem and laziness to continue maintenance treatment of exercises which supposed to solve the issues conservatively in the first place. Proper health food eating is also main culprit western diet causing inflammation contributes to chronic pain no matter what..did the doctors and specialist address this issues?? No because no profit and they know patients won’t stick to it. It was a simple fix but people making it difficult because they want easy fix all the time. Its a behavior not a treatment effect mostly dictated by on the outcome.

    Dr I had a L5S1 13 years ago no pain after. Developed sciatica problems again mri showed bulge in same area. Had surgery 5 months ago took it easy but siatica is back I can do ok with advil Tylenol every six hours in PT now.       Cages are used commonly in spine surgery. Their use depends on the particular situation/ surgical-problem that is being treated and the surgeon's preference. If you are not sure about the recommendation of the surgeon, the best thing to do would be to get a second opinion.

    This leaves me confused as to what to believe. Even a quick search tells me that studies show that 3 level fusion runs a much greater risk of ASD than 1 level. In fact the numbers given are 15% for 1 level, 28% for 2 level & 39% chance for 3 level fusion. The exact opposite of what you state here in the video. The contradiction leaves me frustrated. I will get a follow up visit with my surgeon in a few weeks & will hopefully get him to address this discrepancy. I am very concerned with the possibility of ASD given I had a 3 level & am facing returning to a very strenuous job of wood sculpting with chainsaws. Dazed, confused & concerned.

    I had FBSS, some 15 years later I have peripheral neuropathy. I am in constant pain. I am medicated severely. I have arthritis throughout whole length of spine. There are days I just cannot walk, the pain is horrific.

    My husband had a disc replacement surgery and it completely destroyed his life. It failed and they basically blamed the failure on him. Threw a bunch of opiates at him. I'm sure you can imagine how that worked out. He was in the Air Force. He never recovered. He died at 33.

    I've had 8 back operations through the back and another 1 through the front and now I'm fused from S1 to T6 and most of my opps were because the Titanium rods and screws kept breaking and in the end the rods and screws were changed to Colbolt Chrome and I've stopped working and have been at home for the past 11 years or so now but the Titanium cages the surgeon put in the front have become unstable and causing horrific pain that the surgeons say just one more opp will sort it out but I've given the surgeons 9 time's to fix it and I've just got too the stage that I will just go to the grave with these broken and unstable cages but unfortunately I'm on 30 mg of morphine and 400 mg of tramadol and 4000 mg of panado a day with 600 mg tegratoll and sleeping tablets and still live with unbearable pain each day ...........

    I had a three level fusion and I did need it Did have to have a Neurostimulator But the biggest problem was having to understand my new normal Finally better but after twenty years the back above the fusion is now to the point of surgery at thoracic level. Did an ablation instead Because hell did not freeze over Another new normal having to accepot

    So, im in a bit of a dilemma. I have had a failed back surgery for Lumbar spondylolethisis. The surgeon who operated on me wants to do a repeat surgery. I got a 2nd opinion from a neurosurgeon who essentially said he definitely won't do any more back surgery on me. I have been referred to University of FL for a 3rd opinion. Im very scared to have any more surgery done & wonder if there is anything that can just help to ease the pain. Thanks.

    What do you do when you have cervical and Lombard at the same time, and they assured me it wouldn't paralyze me, its progressive, they did try a medication that helps the nerve pain, and now they won't prescribe it, now they saying surgery or nothing! Even though i don't need surgery

    Where is this surgeon what state and hospital I have sciatica can't walk or go anywhere I have lots of pain plus I have 3hernia disk L3 L4 L5 I need surgery I can't take the pain from the back down my leg thank you

    I'm thinking that if you were to warn patients against using tobacco products of any kind, rather than just referring to smoking, you might get the attention of people like my husband. He's used chewing tobacco for over 40 years and has all kinds of back problems, but since he doesn't "smoke" he doesn't believe there could be a connection.

    Hello there I just stumbled across your video today after I just put off a lumbar spine surgery. I’m 20 years old and have had a herniated disc and stenosis for the past 8 months, I’ve been very slowly improving but at times it seems like I plateau. I started at a constant 10/10 now I’d say I’m down to a 3 or 4 but can have flare ups that can get bad. The best way I can describe it is the pain is “livable” but not good quality of life. My surgeon told me most people will heal on their own with in 3 months but the odd few make take a year. That gave me some hope to try another 3 months of conservative treatment and if I don’t get improved quality of life in the 3 months to then schedule the surgery again so I can know I had done everything i possibly could. I guess my question for you is do you think I can be pain free and get back to my normal life if it’s been nearly 9 months and I still have pain and do you think it’s a smart move to have postponed the surgery to try a last few months of conservative treatment and if it doesn’t work to get the surgery in March or April. Your feedback would be greatly appreciated. Thank you very much!

      Thank you for reaching out. I cannot give you advice about your specific condition for obvious reasons. Having said that, most patients with a disc herniation can gradually recover over time without having surgery. Pain-management procedures can be helpful to some patients.     Can you talk more about preferences for lumbar (L4-L5) disc replacement vs ALIF? As you stated, I'm receiving competing recommendations and am torn on which way to pursue. I almost want to do NOTHING, but not sure that course of action will work for me long term.

      Lumbar disc replacements have largely fallen out of favor, but cervical disc replacements are gradually ticking up. This has to do with the long-term data after lumbar vs. cervical disc replacements. You should get an in-person consultation about your particular condition with a surgeon to discuss this matter.       Don't do the disc replacement. My husband had his L4 replaced and he ended up strung out on pain killers until he died. He never got out of pain. Edited to add: He got hurt in the Air Force. He died about 10 years later at 33.

    This is useful to any potential patient considering orthopedic surgery. The way every alternative is explored is illuminating. Altogether, the explanation gives a person a succinct answer regarding possible symptoms and outcomes. I was once considering a shoulder procedure after banging it on a sidewalk when I tripped while out running. The doctor did not think surgery would be the way to go in my case. I admit I was skeptical: it felt almost disarticulated; but I thanked him and went on with my life strengthening the area as best I could. After viewing this excellent outline by the spine surgeon, I feel like I know more than I did before about how things could go wrong. Everything is sensibly and rationally mapped out and there are valid reasons why. Thank you, Doctor! Sometimes no surgery is the best alternative.
