The frame I used on the multi needle is called "Fast Frames". Recommend googling "Fast Frames" with the model of your machine and you will see what shops sell them. This way you can make sure you also get the best price too.
I would have to figure out where the man preferred his Inital. If placed in the center of buttoning it could end up with initial on the side of his thumb area?
Years ago men working in the Casino business, floor men, dealers, personnel. A lot them would have their shirts tailored and also a small initials on the cuff.

The initials are small and placed very near the fold of the cuff so it shows just below the suit.
I have several customers ask for different sizes. I have done some smaller as well.
