You know the part that always seemed funny to me, after the bad guys rebuild you but your team medic says the chip in your head wasn't turned on so you're still completely trustworthy -- but over the course of the next level every squadmate who teams up with you disappears or gets killed, including said medic (IIRC), and yet nobody in Rhino squad starts wondering if maybe he was wrong about that chip. Of course when you get back to base everyone does, and they don't even know that in the intervening time you actually killed your squad leader! Really feels to me like the Strogg were trying to set you up so that, if they can't have you, at least you won't be trusted enough to reach your potential against them -- but the humans are such idiots they don't even notice.
I never liked Quake 4 as compared to Quake 2. It felt more like an mediocre indie-mod. They should have just continued what Quake 2 did with the same monsters, level design/structure etc. They changed everything in Quake 4 so you can't relate anything to the previous game.

In this case it's true. Later on in game you find teleport, and find also human soldier without his legs - because teleport literally tears apart any non-stroggified human. So it's not some artificial reason for the sake of story here - besides Kane, nobody else can use strogg teleports and some other stuff.
Even though the doctor says they might not be able to repair him, I still think the reason him and everyone else isn't freaking out is because they're pretty sure their advanced medical technology can put him back the way he was.

I think the neurocyte activation process is a complex and delicate procedure and requires special equipment as well as close proximity to the subject. They do use a big ass machine to do it after all. The human surgeons also maybe did something to turn it off indefinitely when they examined him even if they weren't able to remove it.
The strogg were created as a last ditch effort to defend the unfortunate settlers of strogos from the natives after the quake incident, which meant they had to cobble together whatever they had on hand. Of course, the strogg were commanded by an AI, so when the actual settlers died off, the strogg remained and had no leaders and scientists to improve their methods, so they're stuck in a constant state of war since the AI doesn't realize it doesn't need to continue.

The strogg copied this squad aspect from humanity since it was so successful. In order to work like that some higher physical and mental capabilities have probably been left in as opposed to the common almost mindless grunt. It stands to reason that perhaps it's not as easy as controlling/activating/deactivating the common soldier. Also during the cinematic bit of the stroggification process they use a stationary big machine that looks like it needs to be in close proximity to the subject. Whether a controlled environment like that is required or not for activation we can only speculate. Perhaps they never thought any elite would actually not fully undergo the stroggification process and therefore be able to defect and so they never bothered with safety procedures I dunno.

The strogg augments offer nano bots that keep you healed and also allow them to remain attached to your flesh.
There is also health stations that were previously only used by the strogg but now kane can used them to health himself.
And Kane can go through Strogg security grids and teleporters without dying.
Maybe he can't feel pain anymore
