Crazy I was literally just going to research what skincare to use for closed comedones today! I use retinol, vitamin c, azelaic acid, 2% BHA, routinely and still REALLY struggle with a ton of closed comedones! 😢
So I've been dealing with closed comedones on my forehead and cheeks for years and years now, and after going thru everything under the sun (retinols, retinals, differin, vitamin c, azelaic acid, bha, etc) and using them for over 6 months to a year each time, the thing that has finally FINALLYYYYY worked was to prioritize repairing my skin barrier. I stopped retinols, retinals, differin, etc. I switched from bhas (my skin doesn't respond to bhas) to ahas (only twice a week - it's very important to cut back on exfoliating). Stopped using my salicylic acid cleanser and am now alternating between a gentle cleanser and a benzoyl peroxide cleanser (I use micellar water to get rid of my sunscreen before washing my face). Added serums and toners that realllllllly focus on skin barrier repair. Layering snail mucin before my light weight moisturizer that has ceramides. Started spraying my skin with hypochlorous acid once or twice a day. Bought a blue light therapy wand thingy that I'm consistent with. Lo and behold - my closed comedones are slowly but surely disappearing. My forehead is almost completely clear, and so is my right cheek. I would also like to stress that I make sure all of my products are free from ingredients that can potentially clog pores! But yeah, tl;dr - focus on skin barrier repair, stop over-exfoliating, try ahas instead, and check the ingredients of your products before buying them! Hope this helps!!!!!!!!!
Are you by any chance double cleansing with an oil cleanser? I really saw a big improvement in my skin, when I had to cut out oil cleansing because I wasn’t allowed to get my face wet and I found out it was the oil cleanser that was causing most closed comedones. I now use Micellar water to remove makeup and sunscreen and then a regular cleanser and it has been working so well for me 😊

Kelly Driscoll is a good one to look for tips on products for that specially Koreans, if some of you are searching for. I'm actually discovering it takes a lot of trial and error and needs patience to find out what works for us, specially when we have sensitive skin which tends to easily dehydrate and disrupt the barrier. I realised for example if I layer soothing an hydrating toners and am being consistent with barrier protecting products I can tolerate some actives I thought I couldn't. I'm purchasing loads of K-beauty barrier protective products rights now, but I started with the Geek&Gorgeous 5%Panthenol toner and their Happier Barrier moisturiser and I'm AMAZED by how good they are! I also LOVE the Q+A Azelaic 8% to start, as I tried the prescription 15% and could not handle yet - I think I will soon when my barrier is repaired. UPDATE: after doing a skin booster to deeply hydrate the skin I’m now tolerating both 15% AA (every day) and Differin (2x/week).
For anyone on a budget for azelaic acid and not liking the ordinary suspension, Try Revox b77 just azelaic acid 10%. They also have 2% salicylic acid which is like the ordinary but much cheaper. I live in Europe so I am not sure about other countries but in Europe you can get Revox easily. 😊

Please may you do an updated acne safe makeup routine/recommendations (specifically foundation/concealer)? Vichy have discontinued the derma blend foundation which was perfect as a concealer as a little went a long way, and had hygienic packaging whilst being non-comedogenic. I haven’t found anything as good, especially without worrying it’s making my acne worse.
Thank you so much for this video, Dr Sam! I have had closed comedones for a year and nothing got rid of it but this video really helped me overcome this! I use a non-comedogenic sunscreen in the morning and use micellar water and a low-pH foam cleanser along with salicylic acid two times a week and retinol two times a week both on different days in my nighttime routine. My closed comedones have cleared up a lot after just two months!
I have been struggling with cc for over 6 years now. I tried everything. I was already using these products in different order or forms and I was over exfoliating my skin. I had acne for more than a decade. And i tried almost all the products that is mentioned in this video however nothing nothinggg works better than the DR SAM’s products! And it’s also the easiest to use because i dont have to layer the products myself cuz its all in there perfectly formulated in dr sam’s products! We saw dermatologist and they put her on tretinon initially 0.25 and later on 0.5%.she does not see any significant improvement and is really worried. Because i used to have both comedones and cyist , i used salicylic serum and retinol plus niacinamide for like 5 months , i dont have any cyist anymore however there are still comedones hanging around on both of my cheeks and my pore seem so enlarged .

Hello! I have fair, dry, thin, sensitive, flaky skin (almost porcelain-like), and I get those horrible sebaceous filaments and closed comedones too. When I was a child it was fun because I got signed as a model and lots of compliments but taking care of it as an adult (I'm 21 now) has been nothing but a literal nightmare. I'm extremely scared of exfoliating, I've tried using exfoliating products once a week and the results were terrible. My skin becomes shiny and over-exfoliated very quickly... I feel like the only thing I can go for is hyla. acid and oil cleansers, but I'm not sure about salicylic acid. After the disaster with the exfoliating products it took a full month to repair my skin barrier. Should I give up and book an appointment with a dermatologist? I'm completely lost! I'm afraid I will have to end up having to spend +500€ on skincare too, I already have to spend a ton on jewelry because anything else other than fine gold or silver gives me allergy :')

Hi, Dr Sam I’m 17 years old and I struggle with clogged pores, hyperpigmentation, uneven texture and occasional breakouts, I also struggle with sensitive, acne prone skin, I’m terrified of trying to skincare, as I breakout from everything. I have combo skin, what do you recommend?
I am honestly so confused by all the suggestions. I have comedonal acne and honestly haven´t find a perfect balance in my skincare despite being "in the game" for three years. Honestly my biggest concerns are, that you get so many different suggestions from experts. Like for example use a salicylic acid wash in the evening. And incooparte a retinol. But don´t over exfoliate and don´t use retinol with an acid. I honestly don´t know what to do.

another great dermatologist, dr Shereene Idriss, suggested that people with seborrheic dermatitis can use ketoconazole shampoo as a face wash and it should help, the same as it would on your scalp

I just eent today and purchase some CeraVe hydrating cleanser and Cereva facial lotion and CeraVe hydrating toner. Maybe I bought the wrong stuff lol. I know i have a lot of clogged pores going on. I THINK I have combination skin. Typically my forehead seems to be more oily than other spots. Sometimes a tad dry but not bad at all, when it is. I bave never been one to care for my skin. Years of sun exposure (outside and tanning beds) without sunscreen, years on consuming alcohol at a not so good rate, and being poorly hydrated has done a number on my skin. Just a lot of clogged pores going on, from what I can see. A couple extremely small and light wrinkles on side of eye but only noticeable close up in mirror. I am honestly surprised my skins not worse than it is. Anyways, I am trying to repair my skin the best I can and really want to get these pores cleaned up. I quit drinking alcohol like 2 years ago so thats no issue. I wasnt like a daily drinker by any means but a few times a week at night time. For my entire life I would go all day with maybe just a few sips of water or Gatorade and thats it so its probably dehydrated as well. My water intaks is reslly good now though. My diet is good. Only thing needs to be fixed is this horrible clogged up skin and I have watched maybe 100 videos to repair it an I am overwhelmed at what I should purchase and what steps. Obviously i know the face wash/cleanser would be first and then the moisturizer would be last. I cant decide if my skin barrier is screwed up or just a bunch of the clogged pores or both? I do have sensitive skin i feel like. Anytime I try a new fsce wash, most of the time I break out a little but its not horrible. Oh and also for years I have used Toner with alcohol which I now know isnt good for skin so I stopped that. I feel like my skin used to be EXTREMELY oily, but it seemed to go away when I quit drinking alcohol. Like my forehead feels like it may be a little oily but nowhere near what it used to be. Before, you could touch my forehead and the oil would just go all over your hand. I have always washed my face, daily but it was always with a dial mens body and face wash, which im sure isnt good. So I am a clean person so Before I could understand how all this gunk got into my pores. But here recently I learned it wasnt really dirt in the clogged pores, it was just the oil and dead skin. HELP!! I’m over 70 and have many closed comedones/milia on both cheeks. Many dermatologists have told me there is nothing that can be done to remove them. They’ve also told me mine are filled with calcium that forms little hard rocks and there’s nothing I can do to prevent it. Can somebody please tell me if you’ve ever heard of this and if so, if you’ve ever heard of treatment and prevention?
@@user-ei8rb7sj6c it is an exfoliant. Very good for keratosis pilaris. I have used it for closed comedones and it didn’t work to well for me but it may work for you. It mainly just deeply gets all the dirt and oil out of your skin.

Azelaic acid is well tolerated. The Ordinary has an anhydrous salicylic acid serum that I'm in love with too. My skin barrier can't handle most leave-on salicylic acid products but this one has made my skin so soft&glowy in the morning!
Thank you Dr.Sam! I'm only so surprised you recommended Toleriane Dermo-Cleanser! It has Ethylhexyl Palmitate as second ingredient, which is rated as comedogenic 4! Toleriane Hydrating Gentle Cleaser seems so much better formula with ceramides, niacinamide and panthenol! But nice to know how you suggest to add BHA in the routine, I was a bit confused with that one, wondering if it needs to be a low ph exfoliator or a serum would to the job.

Daily if you can - but no rush to get there, go as slowly as your barrier needs.

Obviously i know the face wash/cleanser would be first and then the moisturizer would be last. I cant decide if my skin barrier is screwed up or just a bunch of the clogged pores or both? I do have sensitive skin i feel like. Anytime I try a new fsce wash, most of the time I break out a little but its not horrible. Oh and also for years I have used Toner with alcohol which I now know isnt good for skin so I stopped that. I feel like my skin used to be EXTREMELY oily, but it seemed to go away when I quit drinking alcohol. Like my forehead feels like it may be a little oily but nowhere near what it used to be. Before, you could touch my forehead and the oil would just go all over your hand. I have always washed my face, daily but it was always with a dial mens body and face wash, which im sure isnt good. So I am a clean person so Before I could understand how all this gunk got into my pores. But here recently I learned it wasnt really dirt in the clogged pores, it was just the oil and dead skin. Anyways when your new to this, and a male, it’s extremely overwhelming 😂😂😂 I almost just wanna throw everything away lol not really but you know.
What should I use, during period break out?

Yes - try zoning. Use FNG where you have blemishes and Brightly everywhere else.

HELP!! I’m over 70 and have many closed comedones/milia on both cheeks. Many dermatologists have told me there is nothing that can be done to remove them. They’ve also told me mine are filled with calcium that forms little hard rocks and there’s nothing I can do to prevent it. Can somebody please tell me if you’ve ever heard of this and if so, if you’ve ever heard of treatment and prevention?
yes I think have it too, it’s called keratosis pilaris. you can use salicylic acid mainly.

@@user-ei8rb7sj6c it is an exfoliant. Very good for keratosis pilaris. I have used it for closed comedones and it didn’t work to well for me but it may work for you. It mainly just deeply gets all the dirt and oil out of your skin.

Azelaic acid is well tolerated. The Ordinary has an anhydrous salicylic acid serum that I'm in love with too. My skin barrier can't handle most leave-on salicylic acid products but this one has made my skin so soft&glowy in the morning!
Is it possible that when using a retinal it can leave some hyperpigmentation behind on the place where you had a spot?
that's due to the spot, usually (rather than the retinoid).

Thank you Dr.Sam! I'm only so surprised you recommended Toleriane Dermo-Cleanser! It has Ethylhexyl Palmitate as second ingredient, which is rated as comedogenic 4! Toleriane Hydrating Gentle Cleaser seems so much better formula with ceramides, niacinamide and panthenol! But nice to know how you suggest to add BHA in the routine, I was a bit confused with that one, wondering if it needs to be a low ph exfoliator or a serum would to the job.
